Duties will include, but not limited to:

1. Provides direct clinical social work intervention to patients and family members to include assessment and treatment as required due to complications associated with severe adjustment problems related to HIV/AIDS, other infectious diseases, and cancer diagnoses. Provides individual and family therapy required to alleviate problems and to maximize utilization of health care. Provides detailed program specific education to Veteran and families such as HIV/HEP C testing, transmission, and treatment. Performs psychosocial assessments, identifies services needs, establishes service plans, and appropriate interventions. Facilitates group sessions to include HIV, HEP C, and cancer support groups. Coordinates recreational functions/outings for patients. Provides information and referral resources related to HIV/AIDS, other infectious diseases, and cancer.

2. Manages the treatment plan of Veterans who are newly diagnosed with HIV/AIDS, HEP C, and cancer. Offers services throughout the hospital by providing direct clinical care, professional consultation, and education. Makes recommendations to inpatient team for follow-up care, discharge of patients to their homes and other specialized facilities. Initiates referrals and coordinates services with other programs such as mental health, supportive recovery services, suicide prevention, caregiver support services, and homeless services, among others.

3. Has a working knowledge of specialized benefits and services available to Veterans who are newly diagnosed or living with HIV/AIDS, HEP C, and cancer. Maintains large network of referral services and contacts for public benefits, home care, hospice, housing, and transportation. Is familiar with eligibility, referral processes, and assists in completing applications for services.

4. Acts as a liaison between patients and VA providers. Services may include coordination of community-based services, compensation, healthcare, residential placement, and other VA benefits.

5. Ensures provision of social work and health care services are delivered inclusively of all Veterans including those who are LGBTQ+

The Advanced MSA provides specialized and expert administrative patient support while working collaboratively in an interdisciplinary coordinated care delivery model. The Lead Medical Support Assistant (MSA) performs a variety of administrative functions associated with patient care in the community. In this position the incumbent will serve as the primary trainer for all MSAs assigned to the Community Care department. The incumbent will also be responsible for ensuring timely coordination of patient care in the community. As such, the incumbent must have knowledge of VA policies, regulations, guidelines and processes as they relate to patient scheduling practices, consult management, dialysis contracting, and both inpatient and outpatient authorizations for care. In addition, incumbent must have knowledge of the Community Care, Veteran Care Agreements and the Mission Act programs and processes; as well as an advanced understanding of medical terminology/abbreviations; and must keep abreast of rapidly changing regulations, directives, memorandums, software programs, and processes that are involved in the delivery of the healthcare in the community.

As the department Lead, the incumbent must be able to organize work, set priorities, and meet established deadlines. Incumbent must be able to effectively train assigned MSAs, ensuring they are highly proficient, are able to achieve established performance measures, fully comprehend duties and responsibilities, follow established standard work processes within their area, and are in compliance with established policies and regulations. The Lead Community Care MSA must complete and adhere to annual VHA training requirements, have knowledge of quality assurance and performance improvement principles, and ensure these administrative measures are achieved during the performance of their duties. In addition, incumbent must be knowledgeable of Joint Commission requirements specific to his or her job, such as patient rights, emergency preparedness, information security, ethical conduct, and environment of care.

Our Customer Delivery Drivers get the opportunity to meet and talk to different customers in their homes every day. That’s why this role is about much more than just driving: it's about helping others and delivering great service, with a smile.

Its hands on, physical and full of variety. No two shifts are ever the same and our customer’s look for help in all kinds of different ways. For most of the day you’ll feel like your own boss, delivering to your customers but there is always a team of Managers and colleagues back at base ready to support you when you need them.

You will be responsible for

Delivering your part of our customer shopping trip by:

• I ensure the van and trays I use are clean and kept tidy/organised.
• Leaving the back yard clean, tidy and clear of rubbish, and picking up litter as I see it.
• Ensuring trays are washed as part of the tray wash process.
• Loading the van as required, checking the van for roadworthiness.
• Talking to the customer about any changes to their order, including short code item and substitutions, offering a refund for anything they are unhappy with.
• Taking care of products/deliveries as if they were my own.
• Explaining to the customer that they will not be charged any more for their substitutions.
• Delivering during the customers time slot.
• Calling the customer before arriving at their property if early.
• Calling the store/centre if running 30 or more minutes late.

You will need

• Build relationships with colleagues to create team spirit, working together to make sure our store is the best it can be for customers.
• Warm and welcoming in interactions with customers and colleagues, taking time to understand the customer and say ‘Thank you’.
• Acting quickly to respond to the changing needs of our customers and embracing change within my store.
• Energetic and driven to deliver beyond expectations, integrating home and work to achieve a balance that is right for me.
• Sharing ideas with my Manager and other colleagues on how we can improve our store for customers.
• A valid UK/EU/EEA Driving licence (Full or Automatic only) for at least 12 months with no more than 3 penalty points.

Our Customer Delivery Driver rate of pay starts from £12.92 an hour; this increases to £14.05 for stores inside the M25. Additional payments may also apply depending on location.

It would be beneficial for the candidate to have completed the Diploma in Health and Social Care Level 3. Candidates must also have Literacy and Numeracy level 2 or equivalent and must complete the Trust Healthcare Assistant competencies within their probationary period (first 6 months in role). If the successful candidate does not have the required qualifications they will be expected to enrol on the Healthcare Assistant Framework which will include the acquisition of the level 3 Diploma Health and Social Care, level 2 in Literacy and Numeracy or equivalent to achieve the competencies.
