Learning Disability CAMHS Practitioner
Opportunity available for a Registered Mental Health Nurse / RLND to work within CAMHS Team, Based in Herefordshire
The team sits within a CAMHS Learning Disability Team with the role predominantly developing and provide a high standard of assessment and treatment for patients referred to the service. You will take responsibility for an agreed clinical caseload, involving care co-ordination as appropriate to ensure continuity. As part of this role you will collaboratively assess and provide needs-based treatment programmes and discharge plans for patients and their families and plan and provide appropriate services in consultation with users, carers and clinical teams for patients referred, within defined resources and always where appropriate and relevant using evidence-based practice. You will
develop and maintain relevant and evidence based therapeutic activities as appropriate to the patient group served, including opportunities for cognitive, creative, education, work, leisure, interpersonal and life skills. You will also regularly review and evaluate treatment programmes with patients, carers and members of the clinical teams and participate in clinical meetings, case conferences and other patient review meetings.
The hours for this role are Full Time with shifts being Monday to Friday
The hourly rate for this role is: £26.00 Per Hour
