We are seeking motivated, dynamic, and passionate Band 5 Community Nurses to work within our Mid Bedfordshire Primary Home Care Team based in Ampthill Health centre. This is an excellent opportunity to develop your knowledge and skills within the community nursing setting. You will have the opportunity to be involved in facilitating self-care for patients with long-term conditions, health promotion and innovative activities and projects that enhance patient service delivery and staff wellbeing. You will also have the opportunity to be involved in Urgent Response and Virtual Wards, which is a focus of our teams.

Experience in the field of adult community services is desirable but a willingness to learn and contribute is more important. Newly qualified nurses who are passionate about a career in the community are welcome to apply and will be supported through preceptorship.

This secondment opportunity will support you to develop your leadership skills and confidence. The community nursing teams deliver responsive, high quality nursing to patients in their own homes. You will have the support and mentorship of the Team Lead to develop into this role and be encouraged to apply for the SPQDN in due course.

Would you be interested in a career within the NHS as a Band 4 Nursing Associate, in the community nursing team? Are you passionate about providing a patient centered support service and want to deliver excellent and compassionate care to people over the age of 18 that live in Bedfordshire?



We are looking for a dynamic, motivated and enthusiastic person who has excellent communication, IT literate and problem solving skills alongside a passion for patient centered care. You will work corporately within our Primary Care Team and will be supporting the Community Nurses and the wider MDT to with provide high quality care to housebound residents who live with a wide range of physical and mental health conditions.

Our Trust is known for offering excellent training, personal development programmes, supportive coaching and competency frameworks; to enhance skills and career development which meet the goals and aspirations of the individual. Nursing Associates are encouraged to apply to support the delivery of quality community care.

The successful candidate will be expected to be flexible and assist colleagues cover service needs, which will include working across the wider integrated team. Access to a car in order to carry out service duties is essential.

The Community Nursing Service operates between 08.00 to 21.00 hours and the working pattern will include a variety of rotational shifts which will include weekend, evening and Bank Holiday working.
