The post holder will:

Clinically:
1. Undertake holistic assessments of patient new to the community nursing service.
2. Complete assessments such as Purpose T, MUST/MUAC, wound assessments for all patients on the caseload and ensure that all staff members are completing these assessments.
3. Manage chronic wounds such as leg ulcers, pressure ulcers using best practice and NICE guidelines.
4. Manage short term wounds such as surgical wounds.
5. Manage patients with JVAC drains, lantern drains, pig tail drains and flushes when required.
6. Care for PICC lines and chemotherapy disconnections – Full training will be given.
7. Care for other central lines such as Mid lines, portacaths – Full training will be given.
8. Undertake the administration of intravenous antibiotics in the community, bolus or infusion.
9. Manage the caseload of patients requiring daily administration of Insulin, Dalteparin ensuring patients are also taught to self-manage their long term conditions such as Diabetes.
10. Attend pressure ulcer panel to discuss patients who have developed a pressure ulcer whilst in the care of the community nursing team.
11. Share the learning from pressure ulcer panel with the team.
12. Ensure patients on the community nursing caseload have pressure relieving equipment in place or referred to the appropriate service for additional pressure relieving equipment for specialist items.
13. Liaise with the patients GP when needed and communicate effectively with the GP practice to build relationships with our GP colleagues.
14. Be part of the Integrated care team, attending weekly MDT meetings in the absence of a community matron.
15. Be the patients advocate when needed.
16. Refer patients to safeguarding when required, understand the need for safeguarding patients in the community setting.
17. Ensure staff are completing their electronic records in line with trust policy and within the NMC guidelines.
18. To adhere to the NMC standards.
19. Provide excellent end of life care to patients who wish to be cared for in their homes, ensuring communication and care is joined up with other services such as the local hospice.
20. Provide excellent palliative care and symptom management to the patients with a terminal diagnosis, sign posting, referring on to other services such as living well, local hospice, CNS.
21. Liaise with the tissue viability service, complete referrals to the TVN service for patients with chronic leg ulcers, pressure ulcers or other wounds that require the expertise of the TVN.
22. Liaise with the Diabetic CNS for patients on the caseload who require daily insulin administration or if their condition deems intervention from a Diabetic CNS.
23. Ensure all planned visits are allocated before 3pm daily, being part of the 3pm TEAMS call, asking for support with unallocated visits.
24. Have excellent communication skills, be able to manage difficult situations in patients’ homes or in other clinical areas.
25. Ensure staff adhere to trust policy with regard to clinical waste management, health and safety at work.

An opportunity has arisen for an Electrical Craftsperson to work within our Estates Services team

You will be required to undertake reactive & planned maintenance and installation work across site, maintain high technical standards of workmanship and ensure compliance with all statutory requirements by action and advice as necessary.

Experienced in all types of building maintenance, you will have proven knowledge of health and safety, good communication skills and remain calm under pressure.

As part of the Estates team you will also be required to participate in an on-call rota outside normal working hours, additional payments will be made for this.

You will be expected to provide a flexible and efficient repair and maintenance service over a wide range of electrical and mechanical plant and equipment in a wide range of locations and conditions.

We are looking for a locum consultant to join our dynamic and friendly Rheumatology team.

Our large Rheumatology department is well known for its friendliness and team-working. The consultants and excellent Nurse and AHP teams provide for Derby, East Staffordshire and the surrounding areas. The merger between Derby Hospitals and Burton Hospitals has made this an exciting time as it has strengthened and expanded our provision of Specialist Rheumatology for a population of 1 Million. We are involved in a range of MDT services including Interstitial Lung Disease, Renal, Orthopaedic, MSK Radiology, Obstetric, Dermatology and Paediatric.

We are a major teaching centre for medical students from the University of Nottingham, including the Graduate Entry School and the successful candidate would have designated time for teaching. In addition we have both SpR and GpVTS trainees working in the department.

Provision with Consultant colleagues of a comprehensive Rheumatology service to University Hospitals of Derby and Burton & surrounding areas, with responsibility for the prevention, diagnosis & treatment of illness, & the proper functioning of the department.

Responsibility for the continuing care & treatment of patients in your charge & personal performance of clinical duties as agreed in your job plan, allowing for all proper delegation to, & training of your staff.

Provision of cover for your consultant colleagues during reasonable periods of leave, including care of patients & supervision of junior doctors. (A minimum of 6 weeks notice is to be given for all leave requests).
