1. To introduce and review prescribing recommendations into the Trust as required in line with local and national policies, guidelines and frameworks e.g. NICE, Patient Safety Alerts.
2. To ensure all reference sources, guidelines and protocols in Medicines Information are current and up to date.
3. To ensure seamless transfer of pharmaceutical care for patients, liaising with other acute Trusts, GPs, Community and primary care pharmacists as appropriate.
4. Toidentify and address prescribing issues across the interface and ensure seamless transfer of pharmaceutical care for patients liaising with the other acute Trusts, GPs, Community and PCN pharmacists as appropriate.
5. To promote the safe, rational and cost-effective use of medicines in accordance with recommendations of the SWL Joint Formulary, Surrey Heartlands APC and other evidence based guidelines.
6. To be responsible for the development, maintenance and evaluation of evidence based treatment guidelines, policies, procedures, shared care protocols and patient group directions (PGDs)
7. Apply specialist clinical knowledge and judgment to analyse information, at times where evidence may be lacking, in order to negotiate the optimal clinical outcome
8. To provide a medicines information service to the Trust, ensuring appropriate documentation and recording of enquiries in the Medicines Information database.
