There is an exciting opportunity for an enthusiastic, organised and motivated therapist to be recruited into a Band 7 position in the Urgent Community Response Team called CTT.

You will be working with a broad spectrum of professionals in forward thinking and supportive team.

This post would be suitable for either an experienced band 7 physiotherapist looking to expand their knowledge and skills in crisis management, previous community based work is desirable but not essential.

You will receive regular supervision, training and support from highly skilled colleagues to address your learning needs and work closely with other members of the MDT. Internal and external courses are supported as part of your CPD.

As a senior member of the team, your role is primarily to deliver face to face across the boroughs, including assessments, group education and post assessments in line with National guidance.

As a Band 7 role you will also be expected to provide clinical education for students regularly and further develop your management skills by conducting regular supervision and appraisal for other members of the team.

You will be joining a well-established team of supportive staff.

• To be a Registered HCPC Physiotherapist with at least 18 months post registration experience

Colchester General Accident and Emergency Department and Community North East Essex

This is an exciting opportunity for an enthusiastic and motivated senior staff nurse to join our new dynamic Integrated Urgent Crisis Response Service (IUCRS) providing holistic assessments and identifying treatment needs that can be managed within a variety of community settings. You will provide rapid specialised assessment and treatment to patients who may present with complex physical and psychological care needs and initiate services to safely avoid unnecessary hospital admission.

The multidisciplinary IUCRS team works within the wider Emergency Department of Colchester Hospital and Northeast Essex community services and identifies patients that can, once deemed as not requiring acute admission, remain at home or facilitate a rapid discharge into an alternative community setting.

You will also identify patients requiring outpatient parenteral antibiotic therapy (OPAT) within the acute setting and support a seamless discharge to the community. This new service has been developed using national evidence that illustrates the benefits of community antibiotic services, facilitating early discharge from the acute sector to a community or other appropriate setting, supporting the prevention of hospital admission. You will also support identifying patients suitable to be transferred from the acute hospital to the community hospital.

You should be highly competent and confident in the initiation, administration and teaching of IV therapy. You will be able to make autonomous decisions around patient suitability for this pathway and support patients to self-administer as well and liaise with multidisciplinary team as appropriate. Good communication, leadership and management skills are essential.

You must hold a relevant qualification with community experience or other comparable/transferable skills. You should have experience in the management of junior staff and students. As part of our Community Transformation programme, we have an opportunity for a Registered Mental Health Nurse to join our established Multidisciplinary team. The aim of the service is to provide recovery-focused treatment and support via personalised care plans for individuals who are presenting with a range of mental health disorders: supporting them from diagnosis to recovery, to enable them to lead the lives they wish to live. You will work with a Multidisciplinary Team: a spectrum of clinical professionals, which includes Nurses, Psychologists, Support Time Recovery workers, etc: as well as a range of collaborative system partners.

Provide and oversee the management and care of service users.

Produce accurate and timely documentation, conduct and manage the maintenance of records on all service users in line with your professional Code of Conduct and Trust policy.

Undertake comprehensive assessments: formulating personalised care plans which reflect the needs of the service user, their family and wider support networks

Conduct risk assessment of the individual and of the potential hazards in the service users environment

Attend the weekly Multidisciplinary team meeting

Conduct and oversee physical health care checks and act upon the findings

Act as a Care Co-ordinator/Lead Practitioner

Responsible for administration of medications, assessment of side effects, review concordance and clinical effectiveness of medication regime in line with Trust policy

Provide psycho-social interventions, education and support to develop service users and their carer’s understanding of mental illness with the aim to prevent relapse and admission

Undertake Mental Health Nursing Needs Assessments – contributing to Self-Directed Support assessments/packages

Provide comprehensive assessment, care planning, treatment and review of care in line with the requirements of the Mental Health Clusters and Care Act

Identify, assess and manage clinical and non-clinical risks/hazards in your clinical area via the completion of relevant risk assessments

Provide supervision for lower banded staff
