The Rapid Response Team is seeking a highly skilled and clinically motivated Senior Resuscitation Practitioner to join our service at Frimley Health covering Wexham Park Hospital.

The Rapid Response Team team supports the clinical care of all patients who are risk of deterioration or other immediately life-threatening events or risk of, suffering a cardiac arrest.

As the successful candidate you will have experience in the management of medical emergencies and in the development and implementation of resuscitation policies and procedures. You will be able to demonstrate excellent communication skills

The successful candidate will join a multi-professional clinical education team who are committed and passionate to delivering excellence in the trust.

This is a clinically focused, education, research and clinical leadership role at Frimley Health.

The post holder will be responsible for the day-to-day provision of resuscitation services across Frimley Health.

The post holder will ensure a clinically focused, clinician specific resuscitation and/or emergency care based training as directed by the Rapid Response Team Matron. The post holder will ensure effective day-to-day management of the resuscitation services.

To be a clinical member of the Medical Emergency teams covering Cardiac arrests and NEWS calls will require the post holder to work weekends, evenings and night shifts as required for a 24hr seven day service delivery.

Audit/Research

· To lead and assist in the collection and storage of clinical data pertinent to the ongoing deteriorating patient and resuscitation audit process.

· To identify local audits where areas of specific resuscitation/emergency care activity require or would benefit from a clinical review.

· Work in conjunction with the Patient Safety Team to collect data relating to

topics relevant to the deteriorating patient, Resuscitation and relevant National patient safety alerts.

· Where appropriate undertake research projects in resuscitation and deteriorating patient, which will support evidence based practise.

· Actively partake in local, national and international research projects as appropriate.


Professional and Clinical

· The post holder will be wholly accountable for their actions and will act at all times in accordance with the NMC/HCPC Code of Professional Conduct

· To have the ability to prioritise own workload

· Provide post incident debriefing to staff and identify areas for improvement

This is a very rewarding role within Adult team at East Berkshire Psychological Medicine, based at Wexham Park Hospital. The role would suit an experienced practitioner from a mental health nursing background or other similar profession.

The post holder will be joining a well-established and supportive team working with adults requiring mental health input in Wexham Park Hospital. The team supports patients within the Emergency Department as well as medical wards across the hospital. The work is dynamic and there are many opportunities to develop close working relationships with specialist departments within Wexham Park Hospital as well as with other mental health services.

The post is within a supportive multidisciplinary team of mental health practitioners, including mental health nurses, psychologists, and a full-time consultant psychiatrist. The adult service is a small team and works alongside the older adult team within the East Berkshire Psychological Medicine Service at WPH. The overall service is an established Core24 provision, and is accredited with the Royal College of Psychiatrists.

We are looking for someone who is passionate about providing a holistic approach to care and displays a high level of compassion.

An informal visit to the service is encouraged.

The role involves shift working as the service runs 24 hours per day , 7 days per week and you will be expected to work various shifts. Staff work long days.

You will respond to referrals from the Emergency Department and wards, and complete full psychosocial assessment including identifying risk and formulate a safe discharge plan. In addition you will liaise with other departments and services as necessary to progress a safe discharge.

The inclusive age range is 16 years and above. The post holder will be responsible for the formulation, execution and evaluation of a care plan, including signposting to other services. You will ensure that each individual plan is developed in consultation/agreement with the patient and informed by the wider multidisciplinary team. You will operate within a multidisciplinary team and will be responsible for the delivery of care, risk management and evaluation of care, emphasising individual service user’s recovery. From time to time you will operate within the local Acute Trust as a senior practitioner, providing advice and specialist assessments to formulate appropriate plans of care, including signposting to other services. You will direct and supervise junior staff and / or students in the delivery of care to the post holders allocated service users or as required by the clinical team lead. You will work collaboratively with CRHTT regarding gatekeepers to hospital in-patient beds and Local Crisis Beds. This is an exciting opportunity for the position of a Senior Mental Health Practitioner to join the Slough Community Mental Health Team (CMHT) at a time of new developments for the service.

Within the CMHT we have Senior Mental Health Practitioners who work as care coordinators in a longer-term way with people who live with severe and enduring mental illnesses. There are also a number of Senior Mental Health Practitioners who work in with smaller caseloads in a more intense, goal-orientated way for shorter periods. This is usually especially helpful for patients who require support to change their social circumstances.

The successful post-holder will work alongside dedicated clinicians from other disciplines: Psychologists, CBT Therapists, Psychiatrists, Occupational Therapists, Nurses and Social Workers. We work in a multi-disciplinary way, undertaking comprehensive BioPsychoSocial assessments, developing transdiagnostic treatment plans, and with a focus on reablement and positive risk taking.

Working as a joint-funded service (NHS and Local Authority) we undertake statutory roles on behalf of Slough Borough Council, such as Care Act 2014 assessments for eligibility, Safeguarding reviews etc.; as well as NHS care coordination and treatment delivery in the community.

• The post holder will be a member of the Crisis Resolution Home Treatment Team based a the main hub in Slough


• The post holder need to have a flexible approach there are time when the post holder may be re deployed to one of the home treatment teams based in Slough, Bracknell or Windsor and Maidenhead


• The post holder will provide assessments to patients 18+yrs, following appropriate induction and training.


• To maintain and develop relationships with professionals external to the area of service, service users and their significant others (where appropriate) within area of service/specialism.


• To be responsible for the assessment, planning, implementing and evaluation of specialist care, including evaluating the work of junior staff. And to be responsible for development and delivery of intensive crisis care plan, working collaboratively with multi-agencies to ensure service users’ needs are met.


• To formulate and design physical interventions as part of the service users care plan, ensuring that clinical policies and procedures are followed correctly and risks inherent in any procedures are minimised

The  “must haves” for you to be considered for this role: 
• Must hold a relevant qualification either registered mental health nurse or social worker
• Sound understanding of the acute care pathway
• Assessment and case formulation, evidence of skills utilised to manage risk
