Working alongside a Mental Health Liaison Practitioner, you will work across the interface between Primary Care and Mental Health Services to facilitate mental health assessment in primary care and support GPs with joint decision making in relation to secondary care referrals, signposting, and transition of service users from secondary mental health services back to Primary Care. Your expertise will help inform GPs management plans for those with mental health needs in primary care.

Providing support, education and problem-solving approaches for adults of working age, you will be an active member of the primary care team with the ability to undertake very short-term interventions for clients with mild to moderate mental health difficulties.

This is not a ‘case load’ role or service, enabling you to be flexible and dynamic in a well-supported autonomous role that will allow experience, decision-making skills and service user focus to thrive and develop.

Hi, I’m Hannah and I have been working as a Band 6 Specialist Respiratory Nurse in the Community for 5 years now and honestly, I can’t imagine working anywhere else. We are looking for a Band 6 Respiratory Specialist nurse to join our team.

Our team works with patients in the community to support them at home with their chronic respiratory conditions, as well as in their pulmonary rehabilitation rehab journey. For me one of the most rewarding parts of the job is being able to support patients to improve their quality of life, because it allows you to develop closer therapeutic relationships with your patients. In our team we have Specialist Nurses, Physiotherapists and Respiratory support workers, and we work closely with the wider Multidisciplinary Team.

We have an exciting opportunity for an enthusiastic, motivated, and compassionate therapeutic Support Worker to play a core part in delivering skills underpinned by brief psychological interventions within the new Psychological Skills Service (PSS) Central in Peterborough.

The Psychological Skills Service was developed as part of the Exemplar project in Cambridgeshire and Peterborough NHS trust (CPFT). CPFT were chosen by NHS England as one of the national exemplar pilot sites for the redesign and transformation of primary and community mental health care. The goal of this transformation project was to develop and deliver a new service paradigm that seeks to provide a more seamless pathway of mental health care and eliminate current gaps between services.

PSS was developed to try and address gaps in service provision, and we currently provide interventions for those clients who would have fallen into the gaps between primary and secondary care. The PSS is a Trauma informed service, acknowledging that, the majority of clients referred to the service have experienced some form of trauma in their lives.

As a therapeutic support worker you will fill an important position offering psychologically informed support and psychological skills to a group of people previously described as "not ready" for psychological therapy.

On a day to day basis the role will involve the following tasks:

Building relationships with the client with a view to promote a sense of safety and compassion.

Delivering and teaching psychological skills. In particular ones designed to support people with the stabilisation phase of recovering from complex trauma. Including;

· Anxiety Management

· Low Mood

· Graded exposure

· To support people to make their own choices and identify recovery goals that are important to them, using empowerment skills.

We are searching for an individual with exceptional skills in self-management and organisation, who is confident in collaborating with and leading a team. Good IT skills and an understanding of data are necessary for the role. One of the current significant gaps in service provision in Peterborough is for the treatment of severe mental health illness amongst the group of people struggling with alcohol and drugs misuse.

We know that there is a significant overlap between addictions and severe
mental illness and undiagnosed, unmanaged mental health illness within this group of people makes recovery harder to achieve and maintain. Patients with a dual diagnosis often fail to get an initial diagnosis, or adequate treatment for their SMI, because they do not manage to attend appointments and, if they do manage to get a diagnosis, they do not easily follow treatment protocols.

The Dual Diagnosis Outreach Team address some of these problems, operating as an outreach service, working flexibly with service users and other agencies. Prioritising support for those rough sleeping who are unable to access service.

Successful applicants will be strong team workers with demonstrable commitment and enthusiasm for working with stakeholders. New Hire Bonus -£1500

The role:

Successful post holders will provide clinical leadership in an experienced multidisciplinary team covering the North part of the County. They would have protected time to provide specialist input into the Acute Hospital for Perinatal issues. The successful candidate would join a wider group of supportive and motivated Consultant Psychiatrists both in Perinatal and Liaison Psychiatry

Depending on the interests of the post-holder there are opportunities to be involved in medical management, under/post-graduate teaching, and to develop areas of clinical interest. There are excellent links with the academic department and the potential for Consultants to be involved in research.

The Post

The post is in Peterborough City Health Clinic with satellite clinics in Huntingdon Newtown Centre and Wisbech Agenoria House as well as one day a week clinical activity in Peterborough City Hospital. The newly appointed consultant psychiatrist will work together with a consultant colleague to provide clinical leadership of the North Perinatal Mental Health Team and offer advice, assessment and treatment for all perinatal patients with severe and complex mental health condition in the area. Consultants are expected to form close working relationships with local maternity, health visiting mental health champions as well as Liaison Psychiatry team. Consultants are expected to foster close working relationship with the local Mother and Baby Unit. The appointed Consultant with limited or no experience in the field of Perinatal Psychiatry will be offered bespoke specialist training provided by Royal College of Psychiatrist and other relevant training agencies.

The post may be assigned Specialty Trainees or GPVTS Trainees on rotation.
Key Clinical Responsibilities

The post holder will provide clinical leadership to the team. He or she will contribute to provide advice, assessment of perinatal patients referred to the service for diagnosis and treatment of mental disorders and support MDT members who carry out assessment of patients in the team. She/he will help to facilitate accurate diagnosis and timely interventions so that patients and her family receive integrated, evidence-based treatment which can be delivered in a timely manner and in accordance with the recovery model. He or she will also provide ongoing treatments to patients that are evidence based and supported by NICE guidelines maintaining a bio psychosocial approach.

Post holder is required to maintain Section 12(2) status and Approved Clinician status and have a comprehensive awareness of Mental Health, Mental Capacity and Care Act legislation. He or she will be expected to participate in Mental Health Act assessments when possible.

The post holder is expected to be committed to a multidisciplinary approach to care and actively contribute to MDT meetings and formulation meetings and work in close collaboration with team members, care coordinators, other CPFT services and statutory agencies as required for the care of community patients.

The postholder is expected to develop a one day a week perinatal clinic in the Peterborough City Hospital in close collaboration with the Liaison Psychiatry team and maternity colleagues They are to provide advice on the management of severe and or complex perinatal mental health cases in a maternity hospital setting.

The post holder has a role to foster close working relationship with key stakeholders in the perinatal pathway. He or she will lead the perinatal training programme development locally and contributes to the perinatal training of the workforce.
The service

The Perinatal Mental Health Team (PMHT) is a newly (4 years) established specialist community mental health team. The team operates in two main locations, in Peterborough and Cambridge to meet with the local families’ need and to foster close working relationship with local maternity, health visiting, primary care, statutory and voluntary organization. The PMHT is a multidisciplinary team; psychiatrists, psychologists, perinatal clinical nurse specialists, occupational therapists, social workers, nursery nurses, support workers work together. The PMHT will go through further expansion in line with NHS England Longterm Plan.
Professional Responsibilities

Training and Development

Post holder is expected to participate in the Trust’s annual Appraisal process and to attend all relevant mandatory training as and when required to do so.

The post holder will be expected to take part in regular supervision, annual appraisal and annual job planning.

The post holder will be strongly supported to receive a bespoke perinatal psychiatry training to meet with their training needs.
