The post holder will sit within a specific outreach MDT consisting of a psychiatrist, psychologist, occupational therapist, social worker and SLT. The post holder will receive professional support from a Service Manager. The role requires a Registered Nurse (RNLD/RMN) to deliver advanced nursing care and advice to clinical teams, in line with specific standards. The post holder will offer clinical support to clinical teams through excellent clinical leadership and management skills. This support will be offered to the teams caring for: Patients on trial leave where the placement is at possible risk of breaking down and the host service is considering the need to return the patient to the National High Secure Learning Disability Service (NHSLDS). Patients referred to the NHSLDS who are presenting exceptional and immediate challenges to their service. This may include advice and support regarding the safe and therapeutic use of restrictive interventions in exceptional circumstances. Patients in standard Medium Security units with challenging behaviour, while not presently meeting the threshold for a referral to High Security, would benefit from an assessment or advice from the Outreach team. Opportunities for training, supervision and support will be provided to consolidate clinical skills and develop professional practice.
