Medicines Optimisation To achieve agreed priorities and targets as set by the medicines value team, to promote cost-effective, high-quality prescribing across the place, locality and PCN. To manage the implementation of agreed changes within practices as efficiently as possible and with the minimum disruption to patient care and monitor the effects to ensure benefits are maintained. Contribute to the development of an annual prescribing incentive scheme or similar programme. To review repeat prescribing systems across the locality and produce recommendations for improvement. To attend and actively contribute to regular prescribing group meetings To communicate effectively with healthcare staff, sharing and receiving complex clinical information. Develop and implement policies, guidelines and standard operating procedures as required, in conjunction with medicines value team. To contribute to collaboration across primary and secondary care and prescribing alignment. Data Analysis and Reporting Undertake analysis of prescribing trends for specific therapeutic areas at practice level and monitor the locality performance against the prescribing achievement framework. To provide the required information to the Medicines Optimisation team to track expenditure. To undertake prescribing audits in individual practices with the agreement of the relevant General Practitioners and Practice Manager. Collaboration and Engagement Negotiate with practice managers to allow work to be undertaken within practices Respond to ad-hoc queries on medicines usage from healthcare professionals and members of the public where appropriate To support community pharmacies where appropriate To liaise with secondary care pharmacy service providers where appropriate Communicate, facilitate and negotiate with a wide range of stakeholders, including senior staff and clinicians Influence others to progress medicines value and wider medicines optimisation agenda Provide highly specialised advice to healthcare teams Clinical Governance and Training To play a role in risk reduction and management relating to medicines Reduce waste, inefficiencies and variation To liaise with other members of the Medicines Optimisation team to share ideas for good practice and contribute to the achievement of Clinical Governance To help in the planning and setting up of local projects as necessary and to contribute to projects as determined in consultation with the MO team. To provide training and education to patients, practice and ICB staff where appropriate to ensure the promotion of the safe and effective use of medicines. General To be actively involved in developing the role of the Medicines Optimisation team within the ICB To undertake tasks delegated by the Principal and Advanced Pharmacists, Medicines Optimisation as required To keep updated as to current local and national policy and strategy Supervision of staff as required Work independently and as part of a team. Autonomy to act within professional competence Act as an authoritative expert in medicines quality and value Ensure value programme aligns with medicines governance strategies Purchasing, invoicing and financial processing as required Individual team members will lead on individual programme areas as agreed by the Medicines Quality and Value pharmacy team. Examples may include Optimise, high-cost medicines/ prior approvals, EMIS or SystmOne searches, PrescQIPP champion.
