Clinical Functions: To provide mental health/psychosocial nursing care throughout the 24 hour day through the operation of an internal rotation shift system. This is to include potentially working every other weekend and two weeks of nights in every eight. To undertake physical intervention techniques, that requires occasional requirement to exert moderate physical effort for several long periods. Utilise driving skills when transporting young people. To eat meals with young people at set times, and support the needs of a young person within the context of the Plym Bridge House eating disorder care pathway. Mental Effort The workload (clinical and operational) will be highly unpredictable and interruptions often require immediate action from the post holder. Supporting colleagues and families in managing difficult behaviour, verbal and physical aggression, and levels of suicidal risk in young people. Emotional Effort Frequent exposure to distressing or emotional circumstances examples include dealing with patient issues relating to child protection or risk to life. Participation in team processes and with staff issues, e.g. safety, risk management, service continuity. There is a requirement for long periods of intense concentration whilst delivering clinical practice. Working Conditions Facilitate the reasonable needs of the young person, within a managed risk environment. Regular exposure to unpleasant working conditions when dealing with aggrieved staff and patients. Exposure to incidents of self harm and the ability to provided thoughtful support and care of young people. Be able to work a full 24hr pattern of work, hours will include early shifts 07:30-15:30, late shifts 12:30-20:30, twilight duties 15:00-23:00 and waking night shifts 20:00-08:00. The needs of a 24 hour 7 day a week service will therefore incorporate working regular weekends. All duties will include a mandatory 30 minute break or longer if required dependant on the lengthy of the shift.

PHYSICAL DEMANDS OF THE JOB This section should describe the nature, level, frequency and duration of the physical effort (sustained effort at a similar level or sudden explosive effort) required for the job. Clinical supervision from another health professional can be arranged and is to be encouraged Clinical roles are supported by policies, SOPs and guidelines within which the registered professional is allowed to exercise professional discretion / interpretation Communications and Relationships Well-developed verbal and written communications skills are essential to the role Communicate medicines information (e.g. medicines doses and side effects) in oral or written form to patients and/or carers, who may have difficulties with understanding Work with the Pharmacy team to manage day to day medicines queries, including face to face consultations with patients identified to have compliance or other problems in medicine administration Be actively involved in patient counselling Develop and implement patient/carer health educational groups across the organisation Refer patients to other community services where appropriate Discuss and resolve medication problems with prescribers where appropriate, ensuring treatment is appropriate, safe and effective Input and maintain clinical and pharmacy records, for example using System-one IT, controlled drugs, unlicensed medication To provide information for summary care records to other healthcare professional if appropriate to do so Work collaboratively and effectively with all levels of healthcare staff in every area of work, Physical Demands of the Job There is a need to travel between sites, therefore ability to drive or commitment to attaining a driving license is essential, and to use own car is an advantage Good keyboard / computer skills are essential. Dispensing and checking of medication including blister packs as needed. Manual handling may include small static loads and some patient handling

The post holder will assist registered staff within the Complex Needs team at harbour Drug and alcohol services, working under supervision, in providing support in the community for people experincing mental health and co-existing substance misues issues. The post holder will work alongside the other services and staff working with people that live complex lives in the community, where they will manage a caseload under supervision from a registered member of staff. The aim is to promote recovery from addiction and improve the health and wellbeing outcomes for individuals, linking them in with Community activities and wider supports which include improving access to activities, meaningful employment, and training. The role will require the post holder to work across the complex lives system, developing in consultation with service users flexible and realistic support packages and person-centred plans within agreed guidelines and service models. This post is seen as crucial in working alongside Registered staff to develop and implement person centred care plans in order that the person using the service can optimise their level of functioning despite any barrier their complex problems may present. Care and recovery support is delivered to clients in their homes and within a variety of accommodation settings and will be monitored with the aim to optimise service users recovery needs.
