Senior Mental Health Nurse

Location: Fareham, Hampshire

Hours: 37.5 hours per week, weekends to be included

Salary: £22.56 per hour

If so, we are seeking to recruit experienced, motivated and flexible RMN’s to join our Nurse led Care Home working on either a full or part time basis, the successful candidate will need a flexible approach to hours of duty in order to meet the needs of the service over the twenty-four-hour period and work flexibly regarding shift changes according to the needs of the Care Home.

Uplands House Care Home is a Nurse Led Care Home with Psychology who specialise in supporting residents with Mental Health & Physical Health needs, situated in Fareham where our multidisciplinary team offer high quality care to adults who require a safe and supportive environment to meet their potential.

In your role as a Senior RMN you will support the Registered Manager and Deputy Manager to deliver a high-quality person-centred recovery approach in line with CareTech Community Services Personalisation Strategy and Uplands House Service Improvement Plan.

Roles and Responsibilities:

• To work according to the NMC Code of Professional Conduct and relevant professional guidelines as a named nurse or key worker for a defined group of residents.
• To provide clinical leadership to a designated nursing team and ensure the provision of high standards of quality & care.
• To lead the Nursing Team by role modelling and have an active presence in the Clinical Area
• To support the Deputy Manager with audits, reviews and action plans
• To provide a high-quality individualised standard of care to a defined group of residents, relatives and carers.
• Ensure the effective supervision and development of staff to reach their potential.
• To work and act as a key member of the team, supporting each resident with achieving their planned outcomes and lead on resident review outcomes.
• To act as the primary nurse in the assessment of needs and organising of care.
• To take charge of a designated clinical area.
• To adopt a flexible approach to hours of duty, in order to meet the needs of the service over a 24-hour period.

REQUIREMENTS In order to be a successful you will be: Registered with the NMC or HCPC via a core professional qualification Applications are sought from SALT OTs, Nurses, Social Workers Experienced in risk assessment and management. Experience in supporting children and young people with mental health difficulties and often other co morbid presentations. Comfortable working in a fast-paced and varied role Experienced working within a mental health setting Experienced working in CAMHS (Children and Young Peoples mental health services) It would also be advantageous if you are: DBT Qualified You will need your own transport and be prepared to travel across the county to provide assessments at Royal Hampshire County Hospital, Winchester and Basingstoke and North Hampshire Hospital. MAIN DUTIES To provide specialist assessment and intervention as part of a multi-disciplinary approach, with specific responsibilities regarding the acute care pathway. To contribute to and facilitate evidence based assessments and individual and group interventions within home, school and community settings. To use evidence-based practices for example, professional evidence based models of practice, specialist professional evidence based assessment, treatment, and outcome measures or other recognised evidence based modality supported by post graduate training - in the conceptualisation, treatment and evaluation of complex mental health problems. The provision and organisation of crisis intervention via individual professional and generic practice and team response. To actively support the CPA process Contribution towards selection of the most appropriate psychosocial interventions for implementation by the team. Contribution of most appropriate professional assessment and treatment intervention for implementation by the team as a part of a multi-disciplinary care plan. To personally maintain accurate records and ensure that standards of record keeping are maintained. To demonstrate flexibility in working hours as agreed with the line manager. Developing and maintaining supportive professional networks across the trust. To ensure that professional standards of practice are maintained. To liaise with and actively promote excellent working relationships, communication & working practices with referring teams and partner agencies. This would include the provision of expert advice to multi-agency partners. To, where necessary, provide a single point of contact to professionals for emergency referrals and mobilise resources accordingly, in a flexible manner. To demonstrate clinical leadership skills and work to support the senior staff in service development of the Home Treatment Team and care pathway as a comprehensive, intensive, home assessmentand treatment service, with particular reference to flexible working and the development of out of hours services. To participate in the translation of policy to practice and the development of protocols and procedures for the service alongside senior clinicians and managers. To support and facilitate admission and discharge pathways to the day and inpatient service. Provide and receive appropriate managerial, professional and clinical supervision and actively participate within group supervision to support team development.
