Be at the forefront of reviewing previously unassessed and assessed periods of care, ensuring adherence to Continuing Healthcare processes. You'll collaborate with the Appeals Coordinator, autonomously gathering evidence and crafting comprehensive reports. Plus, chair local resolution meetings, providing vital support to claimants and their families. Requirements:
- NMC Registration
- Registered Nurse Degree or equivalent qualification
- Evidence of post-qualification and continuing professional development
- Postgraduate qualification or educated to master's level
Experience:
- Demonstrated ongoing personal development
- Significant clinical experience with adults in acute or community settings, or those with physical disabilities
- Experience in patient assessment, including checklist completion and participation in Multi-Disciplinary Team meetings using the National Framework for Continuing Healthcare
- Proficiency in undertaking retrospective CHC assessments with an understanding of PUPOC and responsible commissioner guidance
- Strong understanding and working knowledge of the National Framework for NHS Continuing Healthcare, as well as relevant assessment and decision-making tools, including Standards and Regulations
- Demonstrated understanding of Adult Safeguarding concepts, including the Mental Capacity Act
- Participation in relevant professional interest groups
- Experience working across organisational boundaries
- Knowledge and experience in workforce development
- Management skills, including managing Continuous Professional Development (CPD) and Personal Development Planning (PDP)
