You will be responsible for holding a caseload of service users and for the delivery of a variety of specialist Occupational Therapy assessments and mental health interventions leading on the Occupational Therapy contribution to: assessment, interventions, care planning and evaluation. The overall purpose of the post holder is to ensure that individuals who are referred into the service have their occupational needs assessed and interventions/recommendations are identified to meet these needs and that these are incorporated into an overall formulation of need and appropriate plan of care. You will also be expected to be available for consultation, support and clinical supervision to other health professionals working within the Community Mental Health Pathway Service and other areas of the Trust where identified as appropriate.

Manager related training provision to all operation staff within the Trust. Support the department with clinical activities ensuring accuracy, legality, safety and efficacy of prescribed medication in line with local and national procedures Support with pharmacy practice-based audit and research within the department. Act as an operational lead in any relevant aspects of regulatory audit (internal and external). Support the medication safety agenda, collaborate on monitoring system and provision of information to support a reduction in avoidable harm resulting from inappropriate medicines use across the Trust. Work to deliver the medicines clinical governance plans which support safe and effective pharmacy and medicines optimisation services to patients. This involves close liaison with the medicines safety officer and multidisciplinary teams working across the Trust. Lead serious incident reviews where appropriate and provide a quality assurance function for incident reviews involving medicines. Support multidisciplinary teams to investigate incidents to enable the root cause to be understood and appropriate plans to be established to mitigate future risks.

Complete and contribute, where appropriate to robustrisk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve quality of life and maximise occupational performance/independence. Maintain an effective reporting system by observing and reporting verbally and in writing on patient conditions. Utiliseagreed outcome measures to review the effectiveness of interventions, as part of the care planning and evaluation process. Participate in the developmentof the service to ensure clinical governance processes are actively implemented at team level. Assist in ensuring that the aims and objectives of the ward/department (as set by the responsible nurse) are fulfilled and to identify factorswhich may inhibit these from being achieved. The post holderwill be required to work within the NMC Code of Practice and professional guidelines. Maintain an up-to-date level of professional competence within the specific environment. Assist in the implementation of evidence-based nursing practice and participate in research and other projects as required.

To work as a senior member of the Early Intervention in Psychosis Service and support the organisation in effectively delivering Nice Concordant Early Intervention in Psychosis Service to all appropriate referrals Providing specialist Early Intervention consultation/liaison and advice to colleagues Contributing and undertaking specialist Early Intervention assessments for those referred to the service with suspected psychosis and/or At Risk Mental State. HOMECARE SERVICE (Principal Duties) 1. Liaise and communicate effectively with all members of the homecare team involved in the prescribing, supply and delivery of Homecare medicines including Homecare providers. 2. Process invoices on the Pharmacy computer system, checking prices and contract details, ensuring compliance with the Trusts Better Payments policy. Where discrepancies are found, liaise with relevant homecare companies to resolve. 3. Input, search and retrieve data relating to the Homecare service using Trust computer systems and databases. 4. Check account statements from homecare companies and reconcile invoices from the Pharmacy computer system to the statements. Check invoices and liaise with RJAH Finance Department when necessary to ensure invoices are paid promptly. 5. Provide advice and operational support to patients, carers, homecare providers and Trust staff regarding homecare enquiries and refer to the relevant Lead Technician Medicines Homecare or specialist pharmacists if necessary. This may include communicating with patients who are distressed, frustrated and those with disabilities and being sensitive to their needs. 6. Ensure that the correct orders are raised, released, delivered, and issued via the Pharmacy computer system. 7. Support or in the absence of lead homecare technician screen prescriptions and registration forms to ensure they meet all legal requirements and that they are due and appropriate. Senior Registered General Nurse - Rapid Response

Opportunity available for a Registered General Nurse to work within the community rapid response team, Based in Shrewsbury or Shropshire

The team sits within the rapid response nursing team with the role predominantly supporting the provision of Community Nursing care within the Rapid Response and provide scheduled and unscheduled care to the house bound population. You will assist in the assessment, planning and implementation of holistic care provision and work with the MDT to prevent avoidable hospital admissions. As part of this role you will be required to use the following clinical skills: Catheter care, phlebotomy, syringe driver, IV therapy, Subcutaneous drug administration, History taking/top to toe assessment and Care of acutely unwell patients. You will also ensure at all times patients are treated with care and compassion and support the senior nurse in organisation and coordination of the overall care to meet clients' care needs, which may be complex involving other professionals and agencies in an integrated and co-ordinated way.

This role is to commence asap with the shift patterns being full time across 7 days working 3 long days per week. The hours would be between 08:00 - 22:00

The Hourly Rate for this role is:

Monday to Friday 06:00 - 20:00: £26
Saturday/Nights: £30
Sunday/Bank Holiday: £34
