Sheridan School offers an integrated programme of education and therapy for young people with social, emotional and mental health difficulties, compounded by complex disorders including Autism. Age range for students is from year 4 to year 11 and the school is comprised of Primary, Secondary and ASD provisions.
In this role you will:
Receive and respond promptly to referrals undertaking initial assessments to establish the scope, level and nature of the individual client’s therapeutic needs.
Lead and facilitate group and/or individual therapy sessions, ensuring activities and interventions continue to meet the needs of the clients, and are consistent with the provisions detailed in the child/young persons’ therapy plan.
Manage, monitor and review an assigned caseload, ensuring delivery within the agreed timescales.
Maintain accurate and up-to-date records, case file notes and computerised information, and other associated documentation; providing and presenting progress reports to case conference meetings on a regular basis.
Liaise with professional and clinical colleagues, as part of the MDT. Also, assist with the analysis and reporting of statistical data and information for research and audit purposes
Contribute to the development of policies, procedures, processes and initiatives. Ensure their clinical practice complies with statutory regulations and current legislation, meet quality standards and deliver against key performance indicators.

To work as part of a multidisciplinary team to identify the health needs of children and young people, their families and the community and contribute to developing a strategy to address individual identified needs as well as those needs within the community. To be confident to use telephone triage skills and knowledge to assess, plan, implement and evaluate packages of care. To be competent as a non-medical prescriber. To provide home visits and child health drop-ins to assess the needs of children and their families, during a rotation period into locality 0-19 teams. To support children and young people and their families with emotional and behavioural development. To advise their client group on the promotion of health and the prevention of illness. To link practice with local and national targets and deliver public health programmes based on current priorities and targets. To plan evidence based health promotion programmes, order resources, compile teaching plans with aims, objectives and outcomes, and evaluate the sessions. To act as a health link for other agencies, e.g. Early Childhood and Families Service, GPs, voluntary services, public and private agencies and work closely with partner agencies to promote the health and wellbeing of children, young people and families. To work with the locality teams to support children, young people and families who are considered to be vulnerable or at risk. To adhere to current Safeguarding Procedures, and participate in child protection case conferences and case review meetings as required. To contribute to the development of local policies and procedures relevant to their area of work. To take responsibility for their own clinical practice including clinical supervision and contemporaneous record keeping, adhering to the NMC code of conduct. To participate in meetings and attend relevant training courses to demonstrate ongoing professional development. To engage in Service Design developments. To support the digital delivery of the Healthy Child Programme and promote this with Service Users and professional agencies. Within this role you will need to travel as an independent practitioner on a daily basis, across Norfolk which involves both rural and urban travel. The nature of our work does result in travel arrangements changing at short notice. Due to the large geography our service covers and the limitations of sufficient public transport to undertake this role you must hold a full and current driving licence and have access to your own transport to meet the travel requirements of the role. Communication To communicate complex and sensitive information concerning the child or young persons condition to the childs parents/carers and young person. Able to communicate effectively using appropriate methods and recognising barriers to communication. To communicate within the multi-disciplinary team and model effective team working and communication skills, supporting change within the service. To develop links with Early Childhood and Family Service bases, schools and other agencies involved in promoting childrens well-being. To work closely with other professional teams and key stakeholders including service users, statutory and non-statutory organisations. To act as first line response to complaints working within national and corporate guidelines. To contribute to the training of students of nursing and other disciplines, during community experience programmes. Budgetary Responsibility To maintain stock control. To demonstrate responsible use of resources, with an awareness of budgetary restraints. To ensure health monitoring equipment is operating to a high standard, maintained and calibrated. People Management Responsible for day-to-day supervision and/or line management of skill mix staff within the team. To contribute to the training of students of nursing and other disciplines, during community experience programmes. Maintaining own professional development and requirement to take part in appraisal and KSF process. To delegate work to other members of the team related to their capabilities and competencies, ensuring there is clarity regarding the feedback mechanisms. Clinical and Practice Governance Observe and maintain strict confidentiality with regards to any patient/family/staff/records and information in line with the requirements of the Data Protection Act. Any data that is taken/shared as part of a phone call or transported, faxed or transferred electronically must be undertaken with regard to the Trust Information Governance and Information Security policies. The post holder must adhere to the Trust risk assessment and risk management processes. The post holder must adhere to infection control policies and procedures. It is a condition of your employment that you are currently registered with the NMC and it is your responsibility to maintain your professional registration. Undertake mandatory training and any other training relevant to the role as required by Cambridgeshire Community Services NHS Trust. The post holder must participate in clinical & Safeguarding audits as required. Provide patients and their families/carers with information on standards they should expect from the team. Participate in clinical supervision on a regular basis. The post holder is required to participate in relevant emergency preparedness process for their team.
