An exciting opportunity has arisen for a community rehabilitation band 6 Occupational Therapist to join a supportive and enthusiastic community therapy service, working across community delivering rehabilitation in patients own homes or current place of residence, following discharge from an acute bed, or supporting long term condition management.

Pennine Home Treatment Teams are split into 2 locality bases covering the Pennine Area. We have an exciting opportunity for forward thinking, highly motivated, innovative, dedicated practitioners to provide care and treatment for those service users who require support from the Home Based Treatment Team.

Practitioners in the team are dedicated to working with vulnerable service users in their own homes, preventing further deterioration to avoid the need for admission to hospital.
