The integrated care practitioner role is well established within Bracknell. If you are energetic, a team player with a good skillset for autonomous working and decision making; you will be the right person to come and work as part of the Integrated Care Team.

The role is truly situated in between the services to prevent people falling between the gaps; you will be able to guide the person on correct pathway so there are “no wrong doors”. You are working closely with community nursing colleagues as well as local authority colleagues from Bracknell Forest, discussing new referrals and completing home visits.

This is a hybrid role, whilst being able to work from home, you’ll also have opportunity to work at Bracknell CMHT OA, Church Hill House and Bracknell Forest Council, Time Square.

You will be a well-respected member of Bracknell CMHT for Older Adults and the Integrated Care Team, this is a matrix management model - your operational supervision will be provided by the service manager of the Integrated Service, your clinical supervision will be provided from Bracknell CMHTOA.

Your caseload will come from the work you undertake within the Integrated Care Team, you will work closely both Adult Social Care and CMHTOA in Bracknell, you’ll attend referrals meeting to provide steer as part of the integrated work.


• To represent BHFT mental health services at locality LAP/Cluster/Care Home meetings. To screen referrals, identify any further information which needs more exploration to be able to identify a correct pathway.
• To provide clinical leadership, and offer mental health expertise & guidance to professional colleagues and assist in formulation to support integrated decision making and care provision
• To meet the multiple and often complex needs of frail patients with long term conditions who are discussed, through on-going joint assessment of health and wellbeing, including risk assessment, developing, implementing and evaluating intervention plans.
• To support the multidisciplinary team to identify which patients discussed could benefit from mental health intervention.
• To work as an autonomous practitioner, taking responsibility for mental health assessments establishing a formulation and implementing the clinical management required.
• For those patients not requiring secondary mental health input, to support navigation of the health and social care system by recommending the most appropriate level of psychological support from the range of provisions available.
• To hold a small caseload requiring more complex mental health support.

Berkshire Community Dental Service is looking to recruit a permanent dental nurse.
The nurse will join an enthusiastic and skilled workforce with a great team spirit. The service has 7 clinics ranging from Thatcham to Langley and over 45 members of staff. The service treats patients with additional needs including children, people with learning and physical disabilities and the elderly.

You will be based at Berkshire Community Dental Clinic in Bracknell, but will also work at other clinics and in the domiciliary setting. It is therefore essential that you can drive, have a UK driving licence and a car for travel between clinics and hospitals.

There are excellent opportunities for gaining experience in treating patients under sedation and general anaesthetic. The service has hoists and a wheelchair tipper/bariatric bench. Professional development and training are encouraged. Annual leave is 5-6 weeks pro rata depending on experience.



The key duties are:
• You will work in a multi-surgery clinic, having support from other colleagues.

The PCN consists of 3 practices serving 50,000 patients spanning over Bracknell/Crowthorne and Sandhurst. We are an intermediate care rehab unit based in Bracknell for the people of Bracknell. We focus on inpatient rehab to support people to return to their homes. We are a therapy-led unit and you will be part of the rehab MDT made up of OTs, PTs, Techs, Nurses, nursing staff, Doctors and specialist services. You will receive professional supervision and be supported with your CPD and professional development.

To deliver rehab to patients, providing education and promoting independence. Working with patients with acute problems, chronic long term conditions and palliative diagnoses.

To independently manage own caseload of patients with support from Senior Physiotherapy and the wider MDT, and to prioritise accordingly.

To undertake assessment, care planning and treatment.

To support ongoing development of the services within the locality, collaborating and engaging with key partners and other services.

• To be responsible as an autonomous practitioner to undertake a significant specialist clinical workload
• To provide an assessment, treatment and evaluation service for patients referred to the service for rehabilitation with neurological, orthopaedic, respiratory and musculoskeletal conditions, including those needing an urgent response
• To undertake the comprehensive assessment of patients using investigative and analytical skills and seek guidance from senior staff as required.
• To undertake environmental risk assessments within the work environment and to document key risk areas, seeking guidance from a more senior member of staff when dealing with cases indicating a high level of risk
• To undertake individual and group rehabilitation programmes as required
• To support and facilitate safe discharge at the optimum point of care, and through in-reach discussion with acute hospitals, negotiating or following-up those patients in their own homes as needed
• To devise an individualised physiotherapy management and/or treatment plan, using clinical reasoning skills to utilise a range of treatment options in formulating a programme of care
• To be professionally and legally responsible and accountable for all aspects of your professional activities working within codes of practice and professional guidelines. This includes patient documentation
• To demonstrate a sound understanding of clinical governance, clinical risk and to demonstrate the use of treatment plans based on evidence
• To be responsible for implementing within your clinical practice the standards and policies for community working and where applicable propose changes to working practices
