Duties and skills needed to be successful in this position include, but are not limited to
- The Care Coordination & Integrated Case Management (CC\ICM) Social Work Program Coordinator, in collaboration with a Nursing CC\ICM counterpart, is responsible for implementation, coordination, and management of all administrative and clinical aspects of the CC& ICM framework.
- The Social Work Program Coordinator for CC\ICM will implement standardized tools designed to integrate care to create a seamless continuum experience for the Veteran.
- The CC\ICM framework is a specialized, collaborative practice among multiple healthcare professions which provides structure and standards to support inter-professional collaboration throughout the continuum of care and optimal utilization of healthcare resources. CC\ICM serves a highly complex Veteran population, whose problems require a higher level of care coordination.
- Identified Veterans involved in CC\ICM are ensured to receive the most appropriate level of care coordination support, based on their prominent need.
- The aim of CC \ICM is to provide safe, efficient, and cost-effective interventions integrated throughout the continuum of healthcare; to improve access to care, reduce duplication and fragmentation of services, ensure adherence to professional standards of practice, and support staff in their work.
- Administrative responsibility for the clinical programming and daily operations of the Montana VA Healthcare System (MTVAHCS) CC\ICM Practice.
- Developments and implements policies and procedures specific to MTV AHCS and ensures compliance with local facility, VISN, the Associate Director of Patient Care Services (ADPCS), Social Services, VA Central Office (VACO) directives and handbooks, as well as Joint Commission, and other accrediting bodies and regulatory standards as needed.
- Ensures full collaboration with nursing counterpart, for all planning, development, implementation, monitoring, and evaluation of the CC\ICM operations.
- Manages, monitors, evaluates, and reports on all CC\ICM related data to fully capture the day-to-day functioning and practice impacts on the facility and Veteran care.
- Plans, organizes, and directs CC\ICM operations, in conjunction with the CC\ICM nursing counterpart(s) and the Chief of Social Services.
- Identifies Veteran cohorts across MTV AHCS with the highest opportunity and highest need. Including development and implementation of a monitoring process for ongoing screening and identified cohorts.
- Identifies and tracks potential areas where care coordination is taking place to include but not limited to primary care, surgical care, mental health, community care, specialty clinics, geriatrics, and homeless programs.
- Ensures integration with the facility level care coordination and case management teams to include but not limited to Nurse and Social Work navigators, care coordinators, care managers, and case managers of, both inpatient and outpatient for seamless continuity of care.
Work Schedule: Monday - Friday, 8:00 a.m. - 4:30 p.m.
