JOB DESCRIPTION Job Title Pancreas Transplant Specialist Nurse Base Churchill Hospital Grade AfC band 6 Hours of work 37.5 per week Directorate SUWON Responsible to Deputy Matron Transplant Accountable to Clinically: Lead Pancreas Transplant Specialist Nurse Professionally: Deputy Matron Transplant Job Summary: The Pancreas Transplant Nurse Specialist role is to assist in the coordination and management of patients within the Oxford Transplant Centre that are undergoing assessment and work up for a Pancreas transplant. The role involves conducting patient consultations addressing clinical, psychological and social needs under the supervision of the Lead Pancreas transplant nurse with clinician support. This may include clinics during the week and occasionally at weekends in outreach and referring centres as well as within OUH. The post holder also works within the wider Transplant Specialist Nurse team providing on call transplant recipient coordination 8am-8pm and at weekends (occasionally 24 hours) Specialist Nurses will: Work alongside the lead nurse in accepting patient referrals to agreed criteria and be identified as the key worker for an agreed patient/client caseload. Provide advice, education and support to patients, their carers and other health care professionals. Practice clinically in agreed care settings according to approved protocols or guidance. Contribute to the smooth running of the designated service. Work across departmental boundaries. Ensure consultant leave is communicated to relevant teams and clinical sessions closed. Process on call payments. To effectively liaise with, and support when the service requires, the wider Radiology medical secretaries across all OUH sites. To oversee and action the urgent and departmental email accounts for GP and in-house queries/communication. Action urgent findings to the referrer and MDT's. Action private patient requests from appointment to report allocation. To include relevant paperwork for payment and liaise with the Trust's private patient team. To be a departmental Superuser for the hospital EPR and Radiology CRIS patient systems. Use a variety of computer systems to include Microsoft applications, EPR, CRIS and IOV.
