Please refer to the attached job description and person specification for full details of responsibilities -To be responsible for the assessment, planning, implementation and evaluation of the evidence-based care required, including health promotion for a named service user/group ensuring it meets Trust timescales in line with agreed NMC standards. -To support the Clinical lead and other staff on the duty rota to manage, where appropriate, unplanned work. -To participate in the setting of quality standards, including the monitoring and reviewing in line with current clinical guidance practice and policy. - To support the Clinical lead, where appropriate, with practical interventions to de-escalate crisis situations. -To ensure the post holder understands and meets their professional role and responsibilities under the Care Programme Approach -To support the duty clinicians to manage the duty inbox effectively to ensure that requests for help are dealt with appropriately and in a timely manner. -To work from the team base alongside the rest of the duty team to provide immediate support to team members and admin staff dealing with unplanned work. --Where appropriate help to identify early signs of relapse by monitoring the users progress, level of functioning and mental state and alert the appropriate staff involved in the individuals care. - Develop good working relationships and work collaboratively with other organizations that are able to help individuals resolve crises, including vocational and educational providers, training providers, voluntary agencies, accommodation and financial services. -To work using assertive outreach approaches and manage your diary appropriately. -To support the Clinical lead to build more effective pathways with the acute services. -To ensure that the requirements of the Mental Health Act 2007 are observed, adhered to and implemented. -To promote carer and service user involvement within the service and for its provision.

To deliver mental health support work interventions in a community setting To contribute to the planning, implementation and evaluation of therapeutic programme of care for patients To work within the Clozapine Clinic at the Edith Cavell Campus To work within the Peterborough Adult Locality Team at 17 Bridge Street To arrange clinic appointments for the Clozapine clinic and to contact service users to ensure they are aware of their appointments Support the nurse in charge in maintaining essential clinical stocks To promote a positive service user experience through proactive listening, being responsive to issues raised and through resolving issues and concerns raised by service users with colleagues and senior members of the team. To be responsible for maintaining and conducting oneself in a professionalmanner towards service users, carers, colleagues and other agencies, at all times. To ensure high standard and accurate verbal/written communication about patient care with all team members To develop and maintain good relationships, communication and team work with all disciplines To comply with local and Trust policies and procedures including attending mandatory training and refreshers.
