The Assistant Chief/ Residency Co-Director dental officer serves as a practicing dentist who performs a full range of professional dental procedures in connection with the treatment of commonly encountered dental diseases or dental health problems requiring standard corrective, restorative, and/or preventive measures. The dental officer shall be proficient in all aspects of general dentistry including the ability to perform selected specialty services. The duties of the Assistant Chief/ Residency Co-Director include 70 percent clinical and 30 percent administrative responsibilities. The provider should possess the expertise and competency to perform the following:

• Makes oral and radiographic examinations of the oral cavity. Provide consultation and treatments within the scope of their specific clinical privileges. Providing outpatient, inpatient care for patients eligible for dental care.

The Community Living Center (CLC) Interim (short stay/long stay) unit is a 38-bed neighborhood that provides comprehensive, interdisciplinary care to Veterans with acute and chronic conditions. The focused population is short stay/long stay skilled and rehabilitation with primary focus on wound care, IV therapy, general deconditioning, general rehabilitation and post-surgical rehabilitation. Common diagnoses unit include diabetic neuropathy, peripheral vascular diseases, and impairment of skin integrity, complex wound care, congestive heart failure (CHF), and chronic renal insufficiency.

• Demonstrate a thorough understanding of personnel requiring film badges. Read and understand the film badge exposure report. Determine exposure patterns and recommend actions to reduce exposure levels to the public, radiation workers and self.
• Prepare studies for nuclear medicine physician's interpretation by insuring completeness of all forms involved in the procedure. Record appropriate data and related information on the proper patient worksheet and images.
• Work with closely with schedulers for patient procedures, taking into consideration camera availability, scan length, patient condition, the need for multiple nuclear medicine studies, other clinic appointments of the patient, contraindications for the test and urgency of the request. Inform patient of all pertinent preparations for his/her scan, i.e., NPO, follow certain diet, bowel prep, etc. Ensure any printed patient information sheets are updated and relevant. Follows VHA Radiology Orders Management Directives and conducts scheduling audits.

The hospitalist (MD) position is in the Medical Unit within the Acute Care Service Line at the Northern Arizona VA Healthcare System. The hospitalists' functions are to provide accurate, reliable, and comprehensive history and physical examination of the patients admitted to Acute Care Medical Unit for inpatient diagnosis and treatment, and provide routine daytime care for medical inpatients. The patients will be assessed in person in the Emergency Department or Medical Unit, and the hospitalist staff will make rounds on weekends and holidays.

Duties include but are not limited to:

• Whenever a Medical Officer of the Day (MOD) is not scheduled to be on site, at least one hospitalist will respond to and be part of the Rapid Response Team.
• Documents all clinical encounters at the time of visit.
• Records the patient's full and comprehensive history and physical within 24 hours of admission.
• Documents patients' history and physical examinations in a concise and thorough, manner.
• Initiates diagnostic modalities and stabilizes medical therapies while notifying the Acute Care staff, Nursing Officer of the Day, and the Transfer Coordinator in the event of a medical urgency or emergency,
• Maintains and revises, as appropriate, all medical aspects of the care plan including overall goals for management of all medical problems and specification of the methods by which goals will be attained.
• Places consult requests and pursue completion of the consult in a timely manner.
• Practices, always, within his/her scope of practice and assure all necessary consultation has occurred.
• Writes orders in the patient's chart that are timed, dated, and legible and supported by accompanying progress note information.
• Seeks necessary consultation as dictated by the patient's underlying diagnosis and condition.
• Builds and sustains collaborative relationships, works to resolves conflicts at lowest possible level.
• Is flexible about processes in the working environment, adapts well to change and is positive in accommodating new situations and realities.
• Effectively supports VHA, Network, and NAVAHCS initiatives, identifies unique opportunities to expand quality, access, and timeliness of care to veterans.
• Promotes safety with prescribed cautionary procedures and completes all accident reports within agency timeframes.
• Represents VHA programs, VHA staff, and supervisors in a professional manner that focuses on the needs of the VHA or NAVAHCS programs and not on personal or competing agendas.
• Completes continuing professional education required to maintain licensure/certification and keep current with state-of-the-art professional knowledge.
• Participates in quality improvement/quality assurance activities for the assigned work area and service or service line.
• Other duties as assigned.

Physician must be qualified to diagnose, treat, stabilize and manage Emergency Department and Hospital patients according to standard, usual, and acceptable methods and techniques. The successful candidate will also provide coverage as a Hospitalist to cover staff leave or vacancies. Physician must have an unrestricted medical license and be BLS and ACLS Certified. Duties to be performed include but are not limited to:

• Perform assessments/examinations on encounters with all presenting patients;
• Order and interpret appropriate diagnostic tests necessary to complete the ED evaluation;
• Provide Attending Physician oversight to residents, mid-level providers, and students;
• Stabilize all critically ill or injured patients;
• Admit patients to the hospital or discharge them from the ED with appropriate follow-up;
• Discuss treatment plan with patient and or family/caregiver (following COBRA guidelines);
• Perform medication reconciliation with all discharged patients;
• Participate in Quality Assurance, chart reviews, and support applicable ED performance measures;
• Coordinate patient care with consultants as deemed appropriate;
• Emergency Department physician will support the safety, occupational health and fire protection program by the application of approved standards, regulations, practices and work methods;
• Other duties as assigned.

The Nurse Assistant performs complex age specific nursing care for adult and geriatric patients with a variety of physical, mental, behavioral or psychological needs. The Nurse Assistant is learning to perform basic nursing assistant duties under close supervision in a variety of health care settings; outpatient, inpatient, community living care, residential, acute, or sub-acute. The Nurse Assistant will care for patients, residents with acute to chronic conditions. All duties performed will be completed under close supervision with appropriate training and competencies for assigned task.

• Will appropriately utilize glucose monitoring machine to collect and document appropriate glucose readings for assigned Veteran.
• Maintains positive therapeutic relationships by displaying respect for human dignity with patient, families and/or significant others utilizing learned communication skills.
• Begins to utilize observation skills for data collection, reporting and recording patient/resident observations.
• Performs aspects of nursing care to meet patient's needs for the following: personal hygiene, physical comfort, exercise, rest, nutrition, prevention of skin breakdown & rehabilitative aspects of care.
• Nurse Assistant begins to engage in-group activities such as off-unit trips, reality orientation, current events and other therapeutic groups.
• The Nurse Assistant learns to utilize principles of proper body mechanics while caring for patients.
• Reports patient observations to the RN and or other members of the treatment team.
• The Nurse Assistant Is trained to set up patient care related equipment and organizes and obtains routine supplies and equipment.
• Assists with orienting patient/family to unit routines and policies.
• Works as a cooperative, courteous member of the interdisciplinary team.
• The Nurse Assistant safeguards the patient's right to privacy by judiciously protecting information of a confidential nature.
• The Nurse Assistant uses proper safety practices.
• The Nurse Assistant utilize principles of proper body mechanics and follows procedures for use of patient care equipment to prevent injury to self or others in all settings such as utilization of lifting equipment.
