They will work as part of a multi-disciplinary team in a clinical role and will have senior leadership responsibilities. They will manage a team of Clinical Pharmacists and Pharmacy Technicians across the 3 practices. They will provide clinical supervision, training, support and mentorship. They will set objectives within the team that match both the professional development of each team member and the priorities of the three surgeries. They will develop the clinical skills of the Pharmacy Team within their scope of practice and allocate work accordingly, with direction from the 3 practices. Theywill take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy, especially for older people, people in residential care homes and those with multiple co-morbidities. They will provide primary support to general practice staff with regards to prescription and medication queries. They will help support the repeat prescriptions system, deal with acute prescription requests, medicine reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patients in the GP practices. They will provide clinical leadership on medicines optimisation and quality improvement, manage some aspects of the quality and outcomes framework and enhanced services and contribute to the delivery of the local medicines optimisation strategy. They will ensure that the practices liaise with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload. Primary Duties and Areas of Responsibility Long-term condition patients Telephone consultations with patients with multimorbidities and, in partnership with primary healthcare colleagues, implement improvements to patients medicines, including de-prescribing. The specialist nursing role will actively contribute to the on-going development of the Vascular Access Service across the trust and ensure that it delivers and promotes best practice in the care and management of all vascular access devices, promoting aseptic non touch technique to reduce the risk of device related complications and in particular infection. The post holder will provide specialist service and support and deputise for their line manager in the management of their clinical specialty and in conjunction with the Infection, prevention and control team. This will involve dealing with complex situations and having to take appropriate action in a variety of settings. Be expert in peripheral intravenous cannulation. Be expert in the insertion and removal of Peripherally Inserted Central Catheters (PICCs). Undertake training and become proficient in utilising ultrasound guided venous cannulation techniques (e.g. peripheral and central veins). Use ultrasound to identify common anatomical variations (e.g. thrombosis of target vein). Be able to verify tip catheter position and recognition of complications such as pneumothorax using image intensification, ECG technology and tracking devices. Ordering and interpreting radiological investigations such as CXR when necessary. Troubleshooting of vascular access related complications Supporting training and education for staff, patients and others. As part of the Rapid Response team, you'll offer high-quality mental health assessments & short-term intervention to young people experiencing mental health crisis. We work closely with the wider CAMH services, local authorities, education colleagues & other services.

The role is based at Erleigh Rd, Reading during the day & Royal Berkshire Hospital in Reading when working nights.

Current shift patterns are:
• Assessment Clinician 09:00 – 20:00
• Late Assessment Clinician 11:00 – 22:00
• Follow Up shift 09:00 – 17:00
• Shift Lead 08:00 – 19:00
• Weekend/Bank holidays 08:00 – 17:00
• Night Shift 20:15 – 08:15

You'll be allocated a combination of shifts on a rota basis to complete your contracted hours.

You'll use your excellent communication skills & a compassionate nature to to give full support to patients & their families.
