Identify Personal Training needs and actively partake in life-long learning, including Prep Requirements. Ensure skills are appropriately and effectively used to deliver care which is explicitly evidenced based according to the patients needs. Ensure that resources are used efficiently. Contribute towards the establishment of an environment conducive to good patient/staff morale and inter-personal relationships. Play an active role in developing and supporting the nursing team. Actively participate in educational programmes for the whole Primary Heath Care Team, including medical/nursing students and GP Registrar, in relation to areas of clinical and practice nursing expertise. General Practice independently in accordance with NMC code of conduct and with reference to Practice Guidelines, appropriately referring to GPs and other Health Professionals. Initiate safe and effective pharmacological interventions under current legislation (within scope of practice). Maintain accurate documentation of care and utilisation of I.T. Ensure continuation of own personal and professional development, facilitated by utilisation of clinical supervision networks. Will work effectively with individuals in other agencies to meet patients needs and effectively manage their own time, workload and resources. To ensure has basic life support training annually. Management of Chronic Disease (within area of speciality) Run chronic disease clinics according to area of post-holders speciality/responsibility ensuring that their practice is current with up to date practice and following current guidelines. Monitoring and development of individual management plan; agreeing these as appropriate with the patient and other health professionals. Recognising abnormalities and acting accordingly. Identifying the impact of treatment and implementing or recommending changes as appropriate. Check compliance and the adherence to appropriate treatments using an holistic patient-centred approach. To carry out diagnostic and therapeutic procedures. Wound Care and Management Assess, plan, implement and evaluate care for routine wounds including trauma. Provide ongoing care and support to aid good wound management in complex situations (i.e. leg ulcer management/removal of sutures/clips) Provide Patient Health Checks and Contraceptive care and Advice Undertake health checks as required by patients/practice guidelines/GMS contract. Obtain appropriate information using a lifestyle questioning approach and identify appropriate health promotion issues. Work with individual patients to develop a management plan where health problems or potential health problems are identified liaising with other health care professionals as necessary. Undertake smears and provide advice as appropriate following trust guidelines and to undertake annual self-assessment of practice. Assist Drs with coil fits and ensure follow up care for patients. Immunisation (Adult and Child) Ensure knowledge is kept up to date attend yearly immunisations /vaccinations update, including treatment/management of anaphylaxis. Maintain up to date knowledge and understanding of PGDs and respond to changes in legislation accordingly. Administer appropriate immunisations in accordance with current guidelines and practice. Work with patients and other professionals with more complex immunisation issues Travel Health Provide patient holistic approach to travel health including administering appropriate injections and providing guidance in accordance with guidelines. Provide evidence based knowledge to support the team in planning travel health care as appropriate. Use specialist advice/websites as required. Communication To generate and keep accurate and concise clinical records in accordance with NMC guidelines. To be aware of data protection issues particularly within a GP surgery. To liaise effectively with other health professionals both inside and outside the organisation, with regard to accepting and making clinical referrals. Communicate effectively with patients and relatives, considering differences in language, literacy and culture. As well as the nursing team there is a need to work closely with administration staff at the surgery. It will be necessary to attend and contribute to various practice meetings as requested. The only reason for not attending will be annual, study or sick leave.
