GENERAL OVERVIEW

As a member of the health care team and patient care advocate, the Medical Assistant II assists in coordinating the examination, treatment and health care planning for patients under the direction of the provider. Plays an integral role in the training and onboarding of new medical assistants. Also assists with other office functions to support patient care and office operations. Follows all established protocols, policies, procedures and standardized work flows. Effectively contributes to the managed health care needs of the patient. Assists in managing data so that patients can be effectively treated.

ESSENTIAL RESPONSIBILITIES

• Prepares and rooms patients within the clinical practice setting according to standardized work flows.
• Accurately obtains patient vital signs. Accurately documents patient information, patient history, and medication reconciliation. Prepares exam room for patient visit.
• Acts as an electronic health record (EHR) superuser. Accurately documents all pertinent details in the HER according to established standards and work flows including all pertinent patient interactions at the time of the encounter in the correct EHR note type. Reviews, updates and reconciles medication list in the EHR. Reviews problem list and social history with patient and updates record. Follows Meaningful Use standards and assists with population health management.
• Completes annual wellness visit assessments, requests orders for required age appropriate screenings and documents findings in the correct EHR note type.
• Assists in the coordination of patient care under supervision of the assigned health care provider. Assists providers with examination and procedures.
• Provides clinical leadership oversight, serving as a department resource for clinical questions.
• Assists with patient prescription needs based on established protocols.
• Reviews pre-visit planning documentation and patient chart in advance of appointment time to ensure that the provider is prepared to address required screenings and gaps in care.
• Performs PHQ-9 depression screening, tobacco screening, chronic disease screening and MU required screenings. Performs in-house testing following established protocols and policies.
• In collaboration with the care team nurse, provides follow up with patient at designated intervals via patient preferred method (telephone, electronic, written) in accordance with prescribed provider instructions. Notifies patients of test results under the direction of provider in a timely manner.
• Ensures adequate inventory of medical supplies. Ensures all patient treatment areas are at all times stocked with the appropriate supplies using established inventory standards.
• Cleans and sterilizes instruments per established standards. Accurately performs lab controls and equipment checks as assigned.
• Serves as a peer mentor. Responsibly orients new medical assistants in correct office workflows.
• Demonstrates proficiency in teaching pre-visit planning, meaningful use standards, appropriate documentation and workflows. Provides meaningful feedback and performance evaluations for orientees. Mentors students and interns as assigned.
• Accurately performs clerical office functions and other duties as assigned or required.
• Depending on location, may serve as office safety officer: maintains the office safety log, assures the accuracy and completion of all quality assurance testing for all POCT performed in the office environment.
• Depending on location, may perform additional duties such as minor wound care, splinting procedures, and 61 capsules procedures.
• May require travel between physician office locations.

QUALIFICATIONS

Minimum

• One year certificate from college or technical school, or one year of related experience and/or training, or equivalent combination of education and experience.
• Required: Newly hired medical assistants must complete training and/or attempt testing within 30 days of hire; newly hired medical assistants must obtain certification within 120 days of hire.
• CPR certification required.
• Medical assistants must achieve and maintain certification in accordance with CMS Meaningful Use requirements.

Preferred

• Prefer experience with practice management software and electronic health record applications.

GENERAL OVERVIEW

Nationally recognized surgical technologist certification required within 6 months of completion of Surgical Technologist program. -OR-
  • Be employed to practice surgical technology by the health care facility on or before December 29, 2020. -OR-
  • Been employed to practice surgical technology as the individual's primary function in another health care facility at any time during the two years immediately preceding December 29, 2020.

Operates CT equipment to produce cross-sectional images for diagnostic purposes after assessing and screening patients based on ACR and supporting governing bodies' regulations and requirements. Performs diagnostic imaging according to radiologist/medical director approved protocols and completes other duties as assigned. Serves as a physician liaison and department expert. Interacts with patients in a compassionate manner and demonstrates professionalism in a interactions. Works collaboratively with medical staff and other associates to ensure timely and thorough medical imaging.

ESSENTIAL RESPONSIBILITIES

• Utilizes a CT scanner and its associated equipment to perform diagnostic CT studies in an age-sensitive manner. Follows established CT protocols and consults radiologist or chief technologist when modifications to protocols are needed. Positions patients and selects anatomical and technical parameters accurately to independently perform CT imaging to include use of post processing equipment. Address age specific requirements and established time frames. Prepares for and assists the physicians in CT procedures if applicable.
• Communicates and interacts with patients with consideration for varying degrees of illness in order to elicit the necessary clinical history and to ensure patient understanding and cooperation. Reads and reviews all orders, patient clinical history, lab results and reason for exam prior to beginning patient exam. Verifies the accuracy of the patient order. Requests orders for and/or performs laboratory test when required. Utilizes multiple software applications to enter history and track exams.
• Notifies Radiologist of critical CT exams that require immediate interpretation. Reviews and post processes images ensuring that a diagnostic exam is available for interpretation. Prepares and administers contrast media and/or medications within the accepted scope of practice and applicable state and federal regulations using established procedures/protocols. Responds to adverse events or any change in patient's status while undergoing exam and in the department.
• May perform duties of Radiology Technologist as assigned or required. May assist with transportation of patients/families when necessary. Assists with orientation of new hires and clinical oversight requirements for students as applicable.
• Maintains adequate supplies for exam area and notifies supervisor when supplies need to be ordered.
• Completes daily calibrations and warm-up procedures in order to ensure optimal image and quality and to ensure that the CT scanner is operating in accordance with the equipment manufacture's specifications. Utilizes appropriate safety standards and performs quality control procedures when needed. Delivers positive patient experience. Cooperates with and maintains excellent working relationships with patients, AHN leadership and staff, physician offices and designated external agencies or vendors. Performs any written or verbal communication necessary to exchange information with designated contacts and promote working relationships.
• Has a daily focus on attaining productivity standards, recommending new approaches for enhancing performance and productivity when appropriate.
• Adheres to AHN organizational policies and procedures for relevant location and job scope. Completes and/or attends mandatory training and educational sessions within approved organizational guidelines and timeframes.

QUALIFICATIONS:

Minimum

• BA/BS degree or equivalent work experience in a multi-physician medical practice or facility billing/business office specifically related to revenue cycle management, or relevant experience and/or education as determined by the company in lieu of bachelor's degree
• 3-5 years' experience in a multi-physician medical practice or facility billing/business office specifically related to revenue cycle management (including staff supervision).
• Experience operating PC and using software applications.

Preferred

• Call Center experience.

GENERAL OVERVIEW:

The Social Work Case Manager in Care Management is a professional clinician that utilizes principles of care coordination to support patients and their families/caregivers.  The incumbent collaborates with the interdisciplinary team of providers, clinicians, health plans, and external partners while advocating for patients and families/caregivers to coordinate care across the continuum.  The incumbent is an integral member of the Care Management team that works to improve the quality of care, patient experience, and the health of populations and individuals by focusing on the social determinants of health impacting wellness. 

ESSENTIAL RESPONSIBILITIES

• Contributes to and/or completes initial and ongoing comprehensive assessment.  Provides interventions and implements recommendations after engaging patients and their caregivers/families.  Focuses on the individual's risk related to social determinants of health to assure successful coordination of care across the continuum. (30%)
• Collaborates to provide the safest transition plan for assigned patients (Inpatient/Observation/ED) in order to ensure a timely discharge and provide appropriate connection with post-discharge care providers and community based resources. (25%)
• Educates patient, family/caregiver and physician regarding most appropriate level of care post discharge and how to access community support.  Advocates for the patient, family/caregiver through effectively communicating with interdisciplinary team members, payers and post acute partners to assure the safest transition. (20%)
• Serves as a resource to provide counseling and intervention related to treatment decisions and end-of-life issues.  Drives collaborative conversations to establish goals of care.  Provides crisis interventions in cases involving Child Abuse and Neglect, Domestic Violence, Adult and Older Adult Abuse, Institutional Abuse Sexual Assault, Mental Health Disorders, Substance Use Disorders, and Identification of a Surrogate Decision Maker/Guardianship. (15%)
• Promotes individual professional growth and development through certification, mentoring/precepting, and/or participation on department/hospital/system committees. (5%)
• Supports Department based goals that contribute to the success of the organization. (5%)
• Other duties as assigned.

QUALIFICATIONS:

Minimum

• Master's degree in Social Work (Obtain within 6 months of hire)
• Experience in a hospital or health care setting
• LSW required within 1 year of hire.  Incumbents in the role on or before 12/31/2022 have until 12/31/2023 to obtain.

• Prepares equipment and supplies. Selects appropriate exposure factors and imaging parameters. Implements safety standards and performs appropriate quality control procedures on equipment. Troubleshoots, resolves, and/or reports equipment malfunctions as necessary. (10%)

QUALIFICATIONS

Minimum

• Associate’s Degree; or Certification from a technical school; or six months to one year related experience and/or training; or equivalent combination of education and experience.
• American Registry of Radiologic Technologists (ARRT) or Certified Nuclear Medicine Technologist (CNMT) - Nuclear Medicine Technology Certification Board or an American Registry for Diagnostic Medical Sonography (ARDMS) certification or American Registry of Magnetic Resonance Imaging Technologists (ARMRIT) certification.
• Magnetic Resonance Imaging (MRI) Certification is required within18 months of hire. – Employees in a MRI role (prior to July, 1 2022) who do not have a MRI certification are not required to obtain them; employees in a MRI role with existing MRI certification are required to maintain active certifications. Employees hired, or transferred into the MRI role after July 1, 2022 are also required to maintain active certifications.
• CPR certification.

• GED/High School diploma
• 2 years related experience
