The job holder will be responsible for initiating, undertaking and ensuring the cardio metabolic screening is recorded and completed.
Providing evidence-based interventions from an agreed care plan and working closely with care co-ordinators and other professionals, providing feedback and ensuring referrals are signposted to other agencies if required. For promoting safe practice, supporting service users to value and encourage meaningful change and promoting mental health physical health & wellbeing. They will support and assist qualified nurses with Wellbeing and Clozapine Clinics and will demonstrate an attitude that respects and values service users and carers.
1. The ability to prioritise workload according to needs and risk, and will alert the appropriate professional should problems arise. Will deliver care to service users in a community setting, sometimes working alone to an agreed care plan. Liaising with Care Co-ordinators / Consultants and other CMHT members.
2. Ensure cardio metabolic screening is undertaken and recorded.
3. Undertake venepuncture training to enable participation in Clozapine clinic.
4. Completion of training / exam in the use of POCHI machine.
5. Be accountable for aspects of own clinical practice.
6. Ensure the dignity and respect of service users is maintained.
7. Will be proactive in maximising physical health, mental wellbeing and social inclusion. Will support service users to maintain own personal care needs.
8. Will obtain informal consent prior to undertaking any intervention.
9. Participation in mental health promotion aspects of the service.
10. Will support qualified nurses with Wellbeing and Clozapine Clinics.
11. To monitor and report any changes, both positive and negative within service user’s lives to the Community Mental Health Team.

All new clinical support staff will be required to have completed or working towards the Care Certificate (CWP In-house 2 day training to be completed within 6 months of start date).
Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. You are encouraged to enrol for the DBS Update Service. A small annual fee of £13 per year applies.
New entrants to the NHS will commence on the minimum of the scale stated above.
