Job Advert
You will manage a complex caseload and work in a small team dedicated to a high standard of practice for people with severe and enduring mental health difficulties.
Tasks include being responsible for assessing, planning and implementing clients’ care and support plans and ensuring that regular reviews are organised for your caseload.
You will work within client’s homes in the community and across NHS sites and private residential placements. You will directly liaise with other social work practitioners, health and social care staff to deliver a high standard of practice. Your involvement may also be related to progressing hospital discharges, therefore working to strict timescales is imperative.
A DipSw or equivalent and registration with SSSC are essential.
You must have the ability to be mobile across the Council area.
If you are the successful candidate, you will be required to gain/maintain PVG scheme membership.
