1. Assist in the maintenance and development of an integrated governance framework for qualityimprovement and innovation in The Life Rooms Social Model of Health.2. Provide a full range of administrative support duties to the Quality & Research Lead in relationto quality, governance, and innovation for The Life Rooms Social Model of Health. This willinclude the compilation of a variety of documents using Word, Microsoft Office and other ITpackages.3. Organise and facilitate service user-led task and finish groups, taking formal minutes andtranscribing them within an agreed timescale.4. Provide a comprehensive and high-quality surveillance and information analysis system tocapture all reporting sources and providing timely and accurate reports including trend analysisthrough the Quality Information Governance (QIG) process.5. Liaise with the Quality & Research Lead and the Senior Leadership Team to ensure reports aretimely, accurate and fit for purpose, utilising innovation in statistical, graphical and technicalexpertise to present information in formats suited to a varied audience.6. Collating Quality Data from across the service to be utilised for promotional communications.7. Develop, in consultation with the Quality & Research Lead systems and processes to support thedevelopment of performance management of quality improvement and governance throughoutall of the services within The Life Rooms Social Model of Health, liaising closely with operationalmanagers.8. Liaise with the Trust QRV Team to arrange inspections.9. Undertake the preparatory work to prepare for QRV inspections as directed by the Quality &Research Lead; this will include working with operational staff, coordinating visits, collatingmaterial, minuting visits and drafting post visit action plans.10. Work autonomously and liaise with the Trust quality improvement co-ordinators when necessary.11. Support service improvement and quality improvement projects including the preparation ofcomprehensive project plans and ensuring these are actioned, managed, and evaluatedappropriately.12. Undertake surveys, audits, or research to support service developments with regard to qualityissues.13. Co-ordinate, monitor and evaluate the evidence for quality and governance across The Life RoomsSocial Model of Health to ensure the agreed standards and outcomes are achieved.14. Contribute to the development of the Trust quality improvement programmes identifying theactivity required to improve or assure compliance with standards or reviews.24. Oversight in order to report to QIG of all service user feedback in the form of SWEMWBS andoutcome measure ensuring that trends are reported within the SLT reports and external evaluationreports.25. Lead on the review and updating of all Life Rooms documentation; drafting and managing an annualdocument review schedule e.g. business continuity plan, Standard Operating Procedures and TeamCanvas.

Co-ordinate clinical audit activity for all clinical divisions Provide advice and guidance on the completion of all audit projects To ensure all local audit projects are conducted in line with the Trusts Clinical Audit Policy To assist, support and coordinate the Trust participation in national clinical audits To be responsible for the preparation of the annual Clinical Audit Forward Plan To ensure that the registration system for clinical audit operates efficiently and to provide regular feedback to the clinical audit and governance committees on progress. To ensure local clinical audit projects have a SMART action plan in place (where appropriate) To ensure all national audit recommendations are converted into a SMART action plan in a timely way Responsible for the dissemination & sharing of actions from both local and national audits across the Trust Maintain an actions tracker identifying outstanding actions and ensuring evidence is provided for all completed actions To develop working relationships with the Information Services and clinical coding team to assist in obtaining patient lists and data for Quality Accounts preparation To assist in pulling and verifying data in preparation of the annual Trust Quality Account To assist and monitor processes for the dissemination, implementation and monitoring of NICE guidance and NCEPOD recommendations including coordinating audits with Clinical Audit leads as required To ensure non-compliance is escalated to the divisional leads and the Head of QG&C for entry onto the risk register where required and for reporting to relevant Trust wide committees. To assist in the compilation of evidence collation in relation to the Care Quality Commission, NHS Resolution or other external visits as required To line manage members of the Quality Governance & Compliance and Datix teams as required To undertake other ad-hoc Quality Governance projects as required To lead on the delivery of Clinical Audit training including presentations; posters and quality events to supporting the trust strategy on clinical quality.

To provide a high standard of administrative support to the Head of Quality Governance & Compliance. Provision of day-to-day coordination of NICE guidance assessment and compliance process including communicating information to staff at all levels of the organisation. Identify and review newly published NICE guidance and determine which clinical areas are required to provide a response. Issuing newly published guidance in a timely way providing a clear timeframe for response in line with the Trusts Best Practice Policy. Act as the first point of contact and provide advice and guidance on the completion of assessments and be able to resolve issues or queries relating to compliance with NICE guidance. To train new members of staff, including clinical staff, on the completion of NICE assessment forms, expectations for these to be completed to timescale and process for non-compliance to guidance. To collate completed assessments and accurately record the compliance level on the Trust database, flagging any assessments which indicate partial or non-compliance, to the Head of Quality Governance & Compliance. To act with autonomy to attempt to resolve any issues of non-compliance with guidance in assessments. Maintain an action tracker identifying reasons for all partial compliance and actions required to support the full implementation of the guidance with the identification of an action owner and completion date. Monitor responses and chase for any past the agreed response date, escalating any issues in obtaining a response in line with departmental process. Assist in project planning and take responsibility for key effectiveness and compliance projects, under the guidance of the Head of Quality Governance & Compliance. Extract data and prepare and present reports to divisional committees, to assist the Head of Quality Governance & Compliance in the production of Trust level reports for the Quality Governance Oversight Committee (QGOC), the Quality Assurance Committee (QAC) and for the Integrated Care Boards (ICBs). Work with the Head of Quality Governance & Compliance and the Associate Medical Director for Audit & Effectiveness to prepare the agenda, gather and distribute papers, monitor actions, and take minutes at the Clinical Audit Forum (CAF). Act as minute taker for the Clinical Audit Forum (CAF) and provide cover for the minute taker for the Mortality Surveillance Committee (MSC). To coordinate National Confidential Enquiries into Patient Outcome and Death (NCEPOD), including liaising with clinicians and the Information Services team to obtain patient lists, liaising with NCEPOD in the collation and return of clinician questionnaires, organisational questionnaires and copies of patient records as required. To maintain accurate records of NCEPOD study participation including a rolling schedule for presentation of published studies at QGOC and other committees as directed by the Head of Quality Governance & Compliance. Assist in processing clinical audit registrations and completed projects as required by the Head of Quality Governance & Compliance. Responsible for maintaining appropriate stationary supplies including submitting new orders for supplies.
