The post holder will work as part of the Quality & Safety Management Team to assist the Directorate Managers and Department Heads to deliver the Trust Quality & Safety Management Strategy; to investigate incidents and make recommendations for quality improvement and safety initiatives. Specifically the post-holder will:- Investigate incidents in accordance with the Trust Policies. Provide the root cause analysis documents/reports, recommendations and draft reports for Directorate Managers to be able to respond to incident reports/complaint letters. Support the process for serious incident investigations to ensure deadlines are met and the quality of the incident investigation report (RCA) is exemplary. Work in partnership with the Quality & Safety Team; as well as the Patient Experience Team to ensure standard approaches are maintained during investigations; report writing and thematic analysis. Manage local access to the Safeguard Incident Management System to maintain records on progress with actions during investigations. Comply with the requirements for investigation by the Trust Rapid Review Group. Support Directorate Manager with audit compliance with Duty of Candour. Signpost staff to appropriate services for support if required during/after the investigation process. Provide information to the Medical Director/Director of Nursing and Quality for the revalidation of staff in accordance with the GMC/NMC requirements. Ensure lessons are shared across the Directorate and Trust following identification of the root cause of incidents/complaints to mitigate future risk. Work with multidisciplinary teams, leading the RCA process, collating responses and then communicating actions and any changes to practice. To work with the Clinical Governance Lead on agreeing any education/training requirements as a result of incidents/complaints. Promote a culture of incident reporting to facilitate thematic analysis to mitigate against risk/complaints. To comply with the trusts vision, values and behavioural compact. Actively encourage incident reporting.
