GENERAL SUMMARY:

The Speech-Language Pathologist is an independent health care professional who provides individualized patient care that assesses and treats speech, language, social communication, cognitive communication, voice, fluency and swallowing disorders in children and adults per physician referral. The Speech Language pathologist is responsible to evaluate referred patients, establish patient and family centered goals and develop and execute treatment plans to promote or maximize independence in the discharged setting. The staff speech language pathologist also participates in performance improvement and educational opportunities that promote the growth and development of the department and profession.

ESSENTIAL DUTIES:

1. Provide initial evaluations and re-evaluations of the patient’s status related to speech, language, social and cognitive communication, voice, fluency and/or swallowing consistent with physician referral.

2. Perform evaluations consisting of a review of the patient’s medical, surgical and social history. Interpretation of appropriate formal and informal tests in the following areas: Motor Speech, Verbal Expression, Written Expression, Voice and Resonance, Auditory Comprehension, Reading Comprehension, Hearing as it impacts speech and language, Swallow function, Non-Verbal Communication, Orientation, Attention, Problem Solving and Reasoning, Memory, Oral Motor Function, Pragmatics, and Visual Perceptual Skills. Objective tests can be complex and may need to be adapted due to medical comorbidities.

3. Create customized plans of care that coordinate therapist and patient and family directed goals to obtain achievable outcomes for prevention, intervention and sustainable wellness over the patient’s lifespan.

4. Implement and progress evidence-based therapy interventions consistent with established and modified goals and plans of care according to the patient’s response to treatment. Utilizes and supervises support staff as available and appropriate.

5. Develop and coordinate an integrated discharge plan that includes recommendations specific to the patient’s needs with other healthcare providers.

6. Consulting with peers regarding patient care is appropriate and supported. Formal or informal mentoring is required of more experienced and specialty therapists.

7. Maintains educational requirements by providing and attending in-services and staff meetings on a regular basis. Other educational responsibilities include assistance with orientation of new therapy staff, functioning as a Clinical Instructor with students, acting as clinical fellowship supervisor and serving as a mentor or Friend-at-Work to other clinicians. Communicates with participating Universities regarding skill level of students.

8. Assesses patient’s educational needs, addresses any patient barriers to understanding or learning and involves the patient, family and/or caregiver in the entire therapy experience per the patient’s preference. In addition, collaborates and communicates with other healthcare team members to promote optimal benefit from therapy and quality care.

9. Performs medically necessary documentation consisting of initial evaluations, re-evaluations, daily notes, plans of care with associated certification dates, physician letters, insurance authorizations and discharge notes which provide evidence of skilled care according to professional, departmental and payor standards.

10. Fulfills administrative duties associated with patient care which may include:

a. scheduling assistance

b. follow through with patient attendance

c. compliance with accurate and timely charging practices

d. assurance of appropriate referral and diagnostic coding

e. utilization of only the medically necessary services and number of visits

f. compliance with insurance authorizations and tracking of insurance limitations

g. participation in peer reviews

h. performance and process improvement projects

k. any other related activities

11. Promotes the financial success of Beaumont Health. Meets department productivity metric expectations.

12. Compliance with work schedule expectations based on site location needs and hours of operation as required for patient care. This may involve working at other sites depending on patient volume, or other factors. Inpatient work schedule requirements are based on a seven day per week operation which includes availability for weekend and holiday rotations.

C. Certification, Licensure, Registration:

About the Department

The Corewell Health West Ambulatory Care Management Department is made up of Social Workers, RN Care Managers, Community Health Workers and Medical Assistants. We have a large team of Social Workers and RN Care Managers who are embedded in Primary Care offices across the system. Care Management work is focused on identifying and connecting with patients who have behavioral health concerns or chronic medical conditions on an outpatient basis and assisting these patients with developing healthy behaviors and coping skills that will improve overall health outcomes. Our Ambulatory Care Managers play an active role on the patient’s care team and partner with primary care providers in developing an appropriate treatment approach for patients experiencing chronic disease and/or behavioral health concerns to promote self-management and symptom improvement. The Ambulatory Care Management team also provides comprehensive transitions of care support to patients who have been admitted to the hospital in an effort to keep patients well in the community, prevent readmissions and reduce overall cost of care. Resources and support for barriers related to Social Determinants of Health (SDoH) is also provided to primary care patients by our remote team of Community Health Workers. The Ambulatory Care Management Department has consistently demonstrated successful outcomes through the interventions provided to patients.

Scope of work

Plans and prioritizes care for individuals and population of clients, focusing on strategies that will promote optimal health within populations. Provides peer support and role modeling related to the development of life skills and health/wellness to individuals and families during home visits, if applicable. Demonstrates expertise, current knowledge in patient care and management of a caseload of clients of varying complexity and seeks to improve patient, family and health systems/community outcomes through the application of educational concepts/skills and preventive care in a managed care environment.

• Assesses internal referrals and may provide individual, short-term therapy services or care management services and monitors patient progress.
• Develops plan of care and makes recommendations to PCPs, specialists, and other members of the health care team regarding mental health treatment strategies and identifies strategies to maximize continuity of care across the continuum.
• Assesses and provides follow-up for the Systematic Case Review process which includes consultation with psychiatrist and serves as liaison between PCP and consulting psychiatrist to assist in psychotropic medication management for patients.
• Responds to patients’ urgent mental health concerns, including suicide risk assessments and assists patient in accessing appropriate level of care.

About this unit:

The Cardiothoracic Intensive Care Unit (CTICU) is a 22-bed ICU located in the Meijer Heart Center specializing in highly acute post-cardiac and advanced heart failure patients. Nurse techs are able to care for a wide range of patients and devices including heart/lung transplant patients, post-operative open-heart patients, and more.  We are experts in caring for the most critically ill patients in our area and throughout Michigan. We are at the forefront of cardiac medicine, engaging in cutting-edge innovation and research. Top preferred skills we seek in our Nurse Techs include experience in roles that require constant use of critical thinking skills, resilience, a very high level of self-motivation and drive, flexibility with availability, commitment to excellence, receptive to constructive feedback.

Essential Functions:

• Collects and reports physiological, psychological, sociocultural and spiritual needs of patients and families in coordination with and as delineated by the RN or Provider.
• Ensures that required documentation is performed in an accurate and timely manner.
• Provides direct patient care under the direction of the RN or Provider, consistent with clinical policies and procedures, and department standard work. Provides input in the development of an individualized plan of care. Ensures that required documentation is performed in an accurate and timely manner.
• Articulates the nursing technician role to the patient, family and health team members. Provides support to facilitate safe, quality patient care in a cost-effective manner.
• Cleans patient’s environment / work area / equipment and communicates any equipment malfunctions to appropriate department / unit personnel.
• Demonstrates a high standard of moral and ethical behavior. Advocates for privacy, confidentiality and security of patient, staff and organization data.

Orientation and Training

Past the point of the required orientation classes, the Nurse Tech role has a schedule that is variable, with rotating weekends. 

Available 2024 start dates occur once per month and are based on successful onboarding into Corewell Health, and availability of our Nurse Tech Orientation Classes:

• February: 2/12,2/13 & 2/14            ASC: 2/26
• March: 3/11 , 3/12, and 3/13           ASC:  3/25
• April: 4/8, 4/9, & 4/10           ASC: 4/22

The role will begin with 2 days of training in a simulation center and 1 day of EPIC training with our Nurse Educators.  These are prescheduled classes from 8 am to 4:30 pm.  Because these classes are prescheduled, you must attend all full days, and we cannot reschedule any of the required dates if you cannot attend.  Attendance is compensated, as it is required for compliance. The remainder of your training is done on your unit, during the hours of the position you accepted and is also compensated.
