Goal:
• To provide care to people with intellectual and development disabilities in a supported community setting.
Responsibilities:
Participant Support
• Supporting the participant with their daily routines in a person centred way including, but not restricted to-
o Personal care including feeding (PEG), showering, toileting and dressing, whilst ensuring the safety of the participant, including transfers/hoisting between toilet, wheelchair, motor vehicle etc
o Taking medication, documenting, and monitoring responses.
o Meal preparation and shopping
o Cleaning and laundry
o Pet care
o Other routine home jobs
Lifestyle Support
• Working in consultation with other Health Professionals including Behavioural Support Practitioners, Occupational Therapists and other members, in coordinating and delivering the care of the person. This includes-
o Supporting the participant to feel calm and when they feel upset. For some people this will mean initiating the person’s Positive Behaviour Support Plan. This may also include providing feedback.
o Ensure that the person is supported and encouraged to persue their own interests, hobbies and friendships, to participate and be involved within the community
o Ensuring that participants are provided with privacy, dignity and confidentiality
o Providing information to the participant and their families about care in a person centred way.
• Promoting health and assisting in preventing ill health by participating in continuous education and other health promotion activities.
o Ensuring the participant has access to, and participates in community activities, and is supported to access any health care to ensure good health
o Ensuring that all required participant records are accurately followed and maintained.
Administration-
• Administrative tasks relate to both service operations and records of individual participants.
o File notes
o Shift reports
o Communication books
o Learning logs/Monthly reports
o Customer Incident reports and WHS incidents - Medication forms
o Minutes
o Checking emails
o Other required documentation
o Implement the incident report protocol as required
Staff will work to a level that ensures participants receive the highest standard of care possible.
Essential:
• Registered Nurse (Developmental Disability)
• First Aid Certificate
• Driver’s license
• Cleared NDIS worker screening check or willingness to apply
• Where mandatory Health orders apply, applicants must comply with vaccination requirements.
• WWCC (where applicable)
• Police Check
