Specialty Doctor in Healthy Ageing Service
10 Sessions per week


Applications are invited to join The Healthy Ageing Service (HAS) and be part of a multidisciplinary team with Medical Staff, Registered Mental Health Nurses, Mental Health Practitioners, non-medical prescribers, Care Home Liaison Practitioner, Health Care Assistants, Clinical Psychologists, Occupational Therapist and administrative staff.

The successful candidate will be highly motivated, energetic and passionate about delivering high quality care to patients suffering with mental health problems. They should have experience of team working, possess sound organisation skills and be able to work on their own initiative. Effective communication and empathic skills are essential.

The post-holder will be part of a medical team consisting of a Consultant Psychiatrist and a Trainee Doctor. The position requires full GMC registration and we are looking for a candidate with experience in general adult psychiatry. Please note that active driving licence and access to a car is a necessary requirement for this role.
Interviews are scheduled to take place during week commencing 9th December 2024
• Initial assessment, investigation and management of patients for their mental health issues
• Follow up of patients to monitor the progress
• Comprehensive review of notes, making detailed and accurate but concise summaries of very complex mental health histories
• Initiating Psychotropic medications and titrating whilst monitoring for side effects and physical parameters
• Liaison with other teams, or other medical specialties to determine better treatment outcome
• Comprehensive electronic documentation of sufficient standard and good communication with primary care services, other secondary mental health services and medical specialties
• Carry out domiciliary visits with colleagues
• Attend in-patient unit TOPAS to support with in-patient elderly care as required.

§ To provide specialist psychological assessments to service users within the Prison based upon the appropriate use, interpretation and integration of complex data from a variety of sources including psychological and neuropsychological tests, self-report measures, rating scales, direct and indirect structured observations and semi-structured interviews with service users and others involved in the service users’ care.

§ To develop individual formulations and to lead team formulation planning applying theoretical models of the psychology of mental health and personality disorder and offending, to observed behaviour, cognitive, emotional and interpersonal patterns, for service users who present with complex clinical needs.

§ To formulate and implement plans for the psychological treatment and/or management of a patient’s mental health and personality problems, based upon an appropriate conceptual framework of the patient’s problems, and employing methods based upon evidence of efficacy.

· Be responsible for the day to day management of identified projects across the Community Health Services transformation agenda and will be a single point of contact, ensuring delivery against timelines; escalating issues and reporting highlights within the governance process as appropriate

· Agree tasks and resources required to meet project objectives and manage the project within and according to the agreed budget and timescales

· Produce robust and realistic project plans for driving forward transformation initiatives, using appropriate methodology and leading to the implementation of actions and outcomes required to deliver the projects in line with locally agreed timescales

· Maintain a full and accurate audit trail of key decisions, actions and outputs, including but not limited to risk and issue logs, project/milestone plans, update/highlight reports, and action plans, and make adjustments if targets/deadlines are not met

· Produce documents, reports and presentations on a regular basis for meetings, Committees and Boards in accordance with the agreed governance arrangements.

· Use appropriate software to create and maintain appropriate project documentation including project initiation documents, business cases, project plans, project briefs, risk and issue logs and communication plans

· Ensure that the clinical perspective and clinical staff are central to the work of the projects, working with clinical teams, managers and existing Business & Transformation Managers and Service Improvement Managers to develop new ways of working

· Ensure the patient voice is at the heart of transformation planning and delivery from initial scoping discussions all the way through to agreeing evaluation and the desired outcomes of change

· Make audience-appropriate presentations, be open to challenge and opposition, raise awareness, provide training on own project etc.

· Provide support and relevant updates to project/programme boards as required

· Chair project and or workstream meetings as appropriate

· Attend programme meetings to provide updates and facilitate discussions to seek key decisions

· Manage internal and external barriers to change

· Ensure deadlines are met and delivered to a high standard

· Interpret and analyse complex information and make decisions on a range of programme issues where there may be more than one course of action
• Support the Programme Lead in coordinating and facilitating relationships between the programme and other local and other oversight and strategic group
• Work with service colleagues to champion digital innovation
• Investigate use of automation and AI to improve processes and scheduling
• Use service improvements projects as opportunities to digitise further parts of the care pathway
• Investigate use of AI (e.g chat GPT/ Google Gemini) in the production of reports, bearing in mind the need to maintain confidentiality.

· Work collaboratively across the Transformation landscape when other programmes or projects are in a resource intensive period, providing flexible support and project management as required. This may be outside of the community health portfolio.

The post involves working in Milton Keynes Specialist CAMHS, a Tier 3 specialist service, incorporating the Single Point of Access (referrals), Core CAMHS (with a mood and anxiety pathway), and an ADHD pathway. The specialist service works closely with the Liaison & Intensive Support (crisis and home treatment) team, Mental Health Support Team (schools-based support), and the Looked After Children service, as well as other partners in education, social care and the third sector. You will join a Consultant Psychologist in Looked After Children team, Senior Psychologists, Assistant Psychologists, as well as Psychotherapists, and Family, Arts, CBT, Occupational and EMDR therapists in delivering specialist psychological care to, and in partnership with, diverse young people with moderate to severe mental health difficulties and their carers and networks.

The post holder will be responsible for the development and delivery of psychological care; overseeing pathways and waiting lists, innovating where flexibility and creativity are required, and supporting and developing staff our wide multi-disciplinary team in an empowering way.

You will play a key role in the strategic development of services, translating policy into practice and ensuring high quality of care. Providing specialist Occupational Therapy (OT) assessment and intervention to children and young people of the Core CAMHS pathway across Milton Keynes, using evidenced based practice to assess, plan, implement, and evaluate interventions.

To provide intensive home treatment support for up to eight weeks to children/young people and their families in a time of crisis, and equip them with skills and confidence, through solution and function focused interventions to enable a safe discharge from the service.
