Band 6 - Senior Discharge Practitioner

Full-time hours, between:
08:00 - 16:30 Monday to Friday
08:00 - 16:00 Saturday and Sunday

Weekday - £25 per hour
Saturday / Night - £29 per hour
Sunday / Bank holiday - £33 per hour

Main responsibilities of a Senior Discharge Practitioner:

• Assist the Discharge Coordinator with setting standards of care
• To provide and co-ordinate clinical care and support staff in decision making
• To holistically assess patients in a hospital environment
• Create a personalised plan of care for all patients once assessed
• Responsible for managing staff within the Transfer of Care Hub (TOCH)
• Responsible for ensuring excellent clinical standards at all times / leading by example

Must hold valid NMC registration pin.
