We are looking for a Band 3 Health Care Assistant ideally with community nursing or caring experience to join our community nursing team, working between the hours of 8.30 am and 22.00 pm. This service is provided 7 days per week, and you will work alongside a team of experienced Nurses and Paramedics delivering care to the people of Swindon.
In this busy environment, you must be keen to learn and adapt to a changing environment as well as beingexpectedto complete the Care Certificate training if you have not already achieved this and ideally have an NVQ level 3. You will need to be IT literate as there is an expectation to be competent in the use of an electronic patient record system following full training.
Patients may require interventions for the administration of medication, wound, catheter care, and support and advice during the last stages of life. We will provide a training in wound care, care of urethral catheters, caring for the palliative patient who is nearing the end of their life, obtaining and recording accurately the physiological signs and symptoms of the unwell or deteriorating patient and a variety of other clinical skills.
To fulfill the needs of service delivery to patients/service users and in line with our current team of clinicians, the post holder will work as part of a flexible rota over seven days a week, which will include working evenings, weekends and Bank Holidays.
A clean driving license and access to a vehicle is essential for this role.

2. To provide treatment as delegated by a registered practitioner and following an appropriate by care plan.
3. To keep the patient at the centre of his or her care, ensuring patient and carer participation in decisionmaking.
4. To undertake the daily care and support of delegated patients.
5. Deliver treatment/care within a community setting as specified in a written care plan, under the guidance of a registered practitioner.
6. To practice the principles and philosophy of individualised patient care in accordance with policy, procedures and regulatory body’s guidelines.

1. Wherever possible to avoid hospital admission and reduce inpatient length of stay to ensure that patient receive the appropriate care in the most appropriate setting.
2. To be proficient in the use of an electronic patient record system, have excellent communication and documentation skills that are clear, concise and accurate.
3. To work independently within the community setting visiting patients within their own homes.
4. To be aware of those patients whose health maybe deteriorating and escalate appropriately.Patient Client Care
5. Responsibility for the day-to-day care of patients including monitoring.
6. Support the taking of medication under the authorisation of the qualified practitioner.
7. To undertake specific tasks as delegated.
