To undertake through each patients individual care plan, those duties delegated by the Team Leader and Case Manager, which the Health Care Assistant has been trained and is competent and confident to perform. The Mid and South Essex Integrated Care System (ICS) brings together key health, care, community, and voluntary sector organisations from across our area. This will be achieved through undertaking clinical audits (e.g. use of antibiotics). Supporting quality improvement measures and contributing to the Quality and Outcomes Framework and locally commissioned enhanced services. Supervise practice reception teams in sorting and streaming general prescription requests, so as to allow GPs and Clinical Pharmacists to review the more clinically complex requests. Provide training and support on the legal, safe and secure handling of medicines, including the implementation of the Electronic Prescription Service (EPS) to the administration team. Perform medicines improvement work to maximise safe, cost-effective best practice in prescribing under the supervision of the GP or Clinical Pharmacist. Liaise with clinicians and administrative staff to resolve medicine-related queries. Support the Clinical Pharmacists in Structured Medication Reviews (e.g. organise necessary monitoring tests before SMR). Support medicine reconciliation of patients whose care is transferred back into primary care in a timely and effective manner liaising with patients and other providers to ensure patients receive appropriate medication post discharge. Assist Clinical Pharmacists with any searches, data collection and medicine safety tasks. Implement changes to medication that result from CAS/MHRA alerts, product withdrawal and other local/national guidance. Work collaboratively with the Practice team. Develop relationships and work closely with community, hospital and MMT pharmacy colleagues and the wider health system. Participate in practice meetings, patient participation groups and other local meetings to improve engagement with the role of pharmacy technician.

Main Responsibilities

• Operating Mobile Plant on site

Job Summary

• Supporting the Senior Management Team in the safe operation and maintenance of all plant, equipment and installations within the hospital estate, establishing efficient and effective compliance review procedures
• Primarily responsible for the electrical services operational equipment, decontamination, mechanical and building infrastructure. Control of contractors including permits to work on specialist and critical systems within the PFI hospitals as defined but not limited to those set out in HTM00 to HTM08 and any associated statutory and mandatory legislation

What you’ll do

• Fully competent (AP/CP Appointed) in the fault diagnosis, maintenance, repair assets to include but not limited to Mechanical systems such as LTHW Heating Systems, CHW Systems, Steam distribution systems, Hot Water Calorifiers, Refrigeration, Air Handling Units / Ventilation Systems, Compressed air and Medical Gas Systems
• Carry out Planned Maintenance of Mechanical Devices in line with current regulations and manufacturers recommendations
• Comparison and utilisation of technical documents as issued by the Department of Health
• Interpretation of technical terminology used in the Mechanical infrastructure
• The safe operation and maintenance of all plant and equipment within PFI Hospital site. Carrying out a range of repairs and new works to a high standard. Working on own initiative when applicable or as part of a team
• Supporting the Engineering Management team to ensure contractual response and rectification times are met. Producing required support documentation reports to meet contractual requirements.
• Undertaking Authorised Person and/or Competent Person responsibilities in relation to specified systems
• Ensuring services comply in accordance with the Project Agreement, legislative, statutory and HTM requirements
• Contributing to the commercial management of the operational element of the contract, including management of the Performing Monitoring systems and to ensure Contract and Commercial Compliance. Monitoring contractual response and rectification times to ensure compliance and avoidance of service failure and penalties
• Act when there are failures in compliance or initiatives and investigate the reasons behind these and address to build and develop performance
• The cost effective delivery of a Planned Preventative Maintenance Service
• The management of a reactive maintenance service during both normal operating hours and outside operating hours of the hospital. The working requirements will involve the need to take part in a roster out of hours call out system
• Provide technical and professional support to all staff, colleagues, engineering supervisors and to Trust officers in order to develop a team approach

• Provision of advice to the engineering manager for engineering materials procurement
• Management of a Workshop operation and activities for M&E craftsmen
• Provision of guidance and advice to craftsmen in order to develop skills and improve effectiveness

What you’ll bring

Essential

Support practice clinical pharmacists and practice clinical teams to provide a safe and effective repeat prescribing service. Comply with legal and professional requirements and with safe systems of work for re-authorising repeat prescriptions. Co-ordinate medication review appointments and any changes of medication as advised by the clinical team. Liaise with pharmacies on the supply of prescribed drugs, alert clinicians of any supply issues and forward recommendations of alternatives. Medicines reconciliation post discharge and on other transfers of care. Bring queries to clinical team to include under or over usage. Day to day handling of prescription queries within area of own competence. Delivery of training and development to practice administrative staff who process repeat prescription requests to support improved co-ordination and effective pharmacy administration. Use and maintain information reporting systems and databases relevant to the position including incident reporting, prescribing data and patient outcomes. Participate in practice meetings, and other meetings to improve engagement of the role. Utilise GP practice clinical systems to conduct prescribing audits. Support the clinical pharmacists and practices with the work required Support the clinical pharmacists in shared care monitoring and processes.
