Assesses service and users and formulates, develops, implements and evaluates evidence based programmes of care with the aims of improving quality of life, reducing hospital admissions, empowering self-care and takes a lead in specialist area/complex caseload and providing expert advice and support to others. The specialist care/support plans may include the following: Promoting independence/self-management and maintaining safety Increased functional ability Assessment/treatment/management and implementation of care for individuals Interventions in disease/condition management Prevention and reducing of health inequalities Admission avoidance Post hospital discharge review Case management/treatment/care plans Palliative and End of Life planning Drug prescribing and titration. Specialised interventions around specific long term conditions and clinical need Improvement in health and wellbeing including mental health Ability to work without supervision or direction

Social Work Learning Academy (SWLA) On appointment, the post holder will be auto-enrolled in the SWLA and aligned to the appropriate British Association of Social Work Professional Capabilities Framework. The relevant SWLA stage (i.e. stage 1 (ASYE), 2 (Social Worker) or 3 (Advanced Social Worker) will also be agreed taking all prior Continuous Professional Development (CPD) and relevant experience into consideration. The post holder will be required to take responsibility for their own Continuous Professional Development.
