**To be considered for this position you must be qualified as a Registered General Nurse with a current active NMC Pin**

As the Clinical Lead your key responsibilities include:
• To promote, monitor and implement standards for resident care within the unit environment, evaluating through clinical governance and audit
• Provide clinical leadership, positive role modelling to the unit team, ensuring effective communication across the multidisciplinary team, through supervision, mentoring and appraisal. Lead on formal care reviews with commissioning Trust
• Ensure all Care records, risk assessment, audit and any other agreed outcome measures are an integral part of the patients care and that these are monitored and updated on a regular basis
• Ensure that all nursing staff provide an appropriate care plan which is written with the resident and that the resident’s progression against these are documented within care notes

• Caring & compassionate with an excellent clinical track record
• Passionate about delivering outstanding care to older people
• Excellent communication, organisation and leadership skills

The successful Clinical Lead will receive an excellent salary of £21.25 per hour and the annual salary is £42,542.50 per annum. An amazing job opportunity has arisen for motivated Senior Staff Nurse to work in an exceptional mental health service based in the Hook, Hampshire area. You will be working for one of UK's leading healthcare providers

This special service aim to support patients in a range of community-based activities as part of a full treatment programme and to prepare them in moving through their discharge pathway towards a community- based placement

**To be considered for this position you must be qualified as a Registered Nurse (RMN or RNLD) with a current active NMC Pin**

As the Senior Staff Nurse your key duties include:
• Ensure that all patients within the ward have their needs assessed, that programmes of care are developed to meet those needs, and are delivered in accordance with agreed policy during their span of duty
• The post holder will assist the Ward Managers in the evaluation of care offered by nursing staff on nights
• Ensure that staff complete the nursing records for each patient in accordance with agreed policy
• Ensure that advice/support and information to patients and their relatives/friends is provided as required
• Evaluates the effectiveness of care within the ward and takes appropriate action if required
• To facilitate liaison with other disciplines as necessary to meet the needs of patients
• Responsible for setting the standards of care, at nights, to be achieved by example and direction

Undertake full psychosocial assessments with those in need of such. Ensuring thorough, precise and timely documentation and dissemination of information in line with Trust policy and procedure. Undertake a supervisory role to junior staff in line with trust policy and procedure. Undertake acute risk assessment and safety planning for extremely vulnerable client groups during times of crisis. Deliver High quality, evidence based care to a defined team caseload Proactively support the prevention of YP requiring long-term mental health hospital beds, also encouraging and engaging in early discharge from same, should the need had been identified.
