The Physical Therapist is assigned to the Caregiver Support Program. The primary function of the PT Clinical Assessor is direct patient care, including completing functional assessments to a diverse population in outpatient and community settings within CSP Program of Comprehensive Assistance for Family Caregivers (PCAFC). The incumbent will complete functional assessments of Veteran applicants utilizing a prescribed instrument The Veteran Functional Assessment Instrument (VFAI) and other assessment tools to determine if a Veteran applicant requires personal care services from a caregiver due to inability to perform an activity of daily living (ADL) and/or due to the need for supervision, protection or instruction. In addition to completing the VFAI and other assessments, the incumbent will engage in inter-professional collaboration with: social work, nursing, PT, OT, PACT team, MD, PA, DO, NP, psychologists, counselors and a variety of other medical professionals.

Complete home visits in the community to determine Veterans' and caregivers' needs within the home. The incumbent will support the staffing needs of the medical center and will complete other duties as assigned.

Assists in the day-to-day operations of the Caregiver Support Program specific to his/her area of assignment and ensures compliance and collaboration with local facility and the VA Central Office Caregiver Support Program initiatives.

The Clinical Assessor advises and collaborates with interdisciplinary teams throughout the medical center on caregiver issues,

Utilize and assist in maintaining a data monitoring system to measure the efficacy of the program and assists in collecting and analyzing local data for reports for: local management, VISN Caregiver Support Program Leads, Centralized Eligibility and Appeal Teams, the National Caregiver Support Program, Care Management and Office of Patient Care Services.

Conducts Veteran, caregiver and family clinical interventions to facilitate the identification of caregiver stress, psychiatric illness, emotional distress and social needs as treatment issues.

Develops and fosters relationships with community partners to assist with system navigation, maintain collaboration and coordinate services Maintains basic knowledge of local VA medical center programs, community services and VA Central Office caregiver support initiatives.

The Clinical Assessor, in collaboration with CSP staff, provides referrals to VA medical and mental health treatment and to other VA services including VA benefits, community-based social services and other non-VA entitlement programs.

Receives the referral and schedules the patient for evaluation and treatment in a timely fashion; chart review will allow the incumbent an opportunity to assess appropriateness of the referral and possible contraindications for treatment.

This position independently performs duties that require proficiency in special procedures and broad knowledge of service operations, such as food and beverage preparation, modified diets, computer proficiency, dishwashing, dry and refrigerated storage, and safety and sanitation.

Major duties and responsibilities include (but are not limited to):

• Sets up and breaks down assigned station with the correct supplies and food items
• Portions food items into standard serving sizes using the proper utensils and specified dishware
• Decides what food items to serve for the most common diets when the diet card or tray ticket identifies only the kind of diet called for
• Provides patients with basic information about modified diets and the nutrition services that are available
• Identifies obvious discrepancies between the prescribed diets and the food items designated by the menu
• Follows directions in assembling, measuring, weighing, or mixing ingredients for basic formulas and supplemental feedings following standardized recipes and using sanitary techniques
• Delivers meal trays to the patients' bedsides; reporting the patients' comments and complaints to the supervisor or dietitian
• Prepares fresh fruit/cold salads/dressings/sandwich fillings and cold sandwiches and simple cold desserts along with individual and bulk nourishments and supplement foods and beverages for patients
• Makes final check of diet trays assembled by lower grade workers for completeness, correct food temperatures
• Works in one or more functional areas of the kitchen such as food preparation, dish and pot washing, dry and refrigerated storage and receiving, and the serving line
• Performs heavy-duty cleaning tasks throughout the food service and related areas
• Performing other duties as assigned

This position is part of the Pathways Internship Program and is located at the Central Office, a Regional Office, or a federal correctional facility in the Federal Bureau of Prisons (BOP).

Provides administrative support to the Health Service Department.

Assists the Health Services staff in programs, such as transcribing correspondence for Health Services, to include transfer referrals, minutes for the Health Services Committee and staff meetings, letters, and other reports as necessary.

Work assignments provide the incumbent with the opportunity to learn about the overall mission, functions and organization of the Bureau of Prisons (BOP).

Major Duties will consist of but not limited to:

• Serves as a Consult Coordinator responsible for coordination of assigned consults to include scheduling, discontinuation of duplicate consults, correction of consult errors, handling of no-show consults, ensuring providers have correct consult title set-ups in the system, preparation of consult management reports, contacting patients to schedule appointments, etc
• Monitors patient-access performance metrics, assures timely scheduling and access to care provided by the section.
• The incumbent assists the senior management and attending staff in the educational endeavors of the program to include performing extensive literature searches, preparing Power Point presentations and assembling lecture material.
• Performs a variety of administration functions and assignments to include timekeeping, patient scheduling, answering phones, and check-in desk(s) for coverage.
• Maintains an automated system of program-specific data to track dates on items such as progress reports, funding accomplishments, compliance strategies, correspondence, etc.

Major duties and responsibilities will include:

• Has a broad knowledge of routing modified diets, combinations of diet modifications and commercial off-the-shelf (COTS) dietetics software
• Handles patient meal service orders in person or by phone
• Interacts with Veterans, dietitians, nursing staff, and other medical center employees
• Receives patient meal orders and preferences, meal and nourishment requests from nursing, clinical staff, and other members of the interdisciplinary team
• Takes whatever action necessary, within authority, to maximize patient satisfaction with meal service
• Completes select and non-select menus as required
• Retrieves computerized information to assist with tray and/or nourishment assembly such as standing orders, nourishment labels, tabulated recipe lists, and other simple information to assist in meal delivery
• Provides education and information on local facility therapeutic diets to patients, guests, and nursing staff
• Assists patients in selection of the proper foods according to their individual diet
• Performs clerical and administrative duties in support of the service

Operates a personal computer (PC) and peripheral equipment to produce a variety of civilian and military correspondence, spreadsheets, charts, reports and graphs, using a wide variety of software programs.  Transmits and receives documents and messages electronically using PC network or links to other computers.  Proofreads all prepared correspondence and checks for completeness and conformance to applicable instructions and regulations.  Operates PC using graphics applications to produce various graphic materials as required.  May type and prepare statistical data sheets.

Duties include but not limited to:

Incumbent reports to technical supervisor or Pathologist. Evaluates and solves problems related to collection and processing of biological specimens for analysis considering physiologic processes and requirements. Accepts and/or documents incoming patient specimens using the designated computer system (or backup system) as per established policies and procedures.

Controls physical conditions and responds to time factors to ensure that the physiological state of the specimen and the quality of patient care is maintained. Reports all specimen problems according to established guidelines. Verifies accuracy, completeness and validity in the testing process. Logs rejected specimens according to laboratory/departmental policy and delete accessions according to laboratory policy. Adheres to requirements to accomplish plans to assure successful accreditation by regulatory agencies such as Joint Commission, CAP, CUA, FDA, American Association of Blood Banks and other governing bodies.

In microbiology, specializes in performing gram stains, infectious disease testing, immunochemical fecal occult blood testing, vaginosis DNA testing, and complex PCR testing. Performs a variety of diagnostic tests and makes macroscopic examinations of cultures of numerous species of bacteria for the purpose of positive identification when performing pharmacy aseptic technique assessments.

In hematology, performs complex testing such as coagulation studies including prothrombin testing, activated partial thromboplastin testing, fibrinogen testing, and D-Dimer testing. Performs body fluid crystal analysis and microscopic urinalysis reviews. n chemistry (typically in the specialized chemistry unit of the section), performs blood gas testing.

Maintains and documents the optimal functioning of laboratory equipment. Maintains laboratory in a clean and safe condition according to agency and accreditation requirements. Troubleshoots and repairs instrumentation. Recognizes indicators of malfunction and evaluates validity of data.

While performing a full range of biochemical tests, must have an extensive knowledge of instrumentation to install, calibrate, troubleshoot and resolve problems. Calibrates at timely and/or established intervals to minimize break downs and delay of procedures. Performs and monitors quality control. When controls fall outside acceptable limits, employee uses independent judgment, in conjunction with standard operating procedures, to evaluate the validity of data in relation to the test system and assay procedures, correlating and integrating information and perform additional testing to confirm abnormal results. Monitors specimens and the environment for acceptability.

Helps to maintain an effective liaison with biomedical engineering personnel from Healthcare Technology Management and/or manufacturer's technical service representatives to help assure optimal uninterrupted services. Ensures that sufficient supply of reagents, standards and controls are available at all times to assure that the inventory is adequate for uninterrupted functioning of all instruments. Is responsible for preparation of reagents, controls. etc. as required. Recommends changes in procedure to improve efficiency, accuracy and safety. Conducts all work according to laboratory safety regulations. Assists in training employees in laboratory procedures and laboratory safety. Accountable to technical supervisor or Pathologist to report conditions and progress on eliminating discrepancies.

• The technologist must demonstrate skills in performing complex ultrasound procedures including color flow, Doppler carotid, and arterial Doppler exams.
• Knowledge of anatomy, cross sectional anatomy, and physiology such as the location, appearance, and function of the various major and minor systems applicable to sonographic illustration to interpret the requested examination accurately.

• Participates in the training of physicians, students, and radiology technologists so these individuals may acquire an understanding of the principles of ultrasound and how it relates to other diagnostic exams. Instructs students while performing the full range of ultrasound examinations; oversees the student as he/she performs examinations, providing immediate assistance or guidance.
