Applications are invited for a Lead Pharmacist - Palliative and End of Life Care.
Working within the Havens Hospices Care settings and Southend Site pharmacy, you will lead the Palliative and End of Life pharmacy service in collaboration with the senior managers and clinical collegues from both Havens Hopices and the MSE Pharmacy Oncology team.
You will be responsible for supporting the medicines management agenda within Havens Hospices Care settings, providing a comprehensive clinical service for both adult and children & young people’s inpatient services.
You will support the Southend site oncology service with clinical pharmacy activity e.g. chemotherapy prescription verification.
You will need to be an experienced clinical pharmacist, with palliative, end of life and (preferably)oncology expertise, an excellent communicator with strong interpersonal/relationship building skills, have good service delivery and management experience, and a proven track record of delivering change within dynamic environments.
Ideally you will be a qualified independent prescriber.
We are looking for a highly motivated, innovative and dynamic individual who will develop and evolve this newly created post.
Main duties:
• To provide, with the support of senior colleagues, the delivery of a medicines optimisation and clinical pharmacy service to all areas within Havens Hospices; be a proactive and engaged member of Havens Hospices Medicines Management team.
• Closely liaise with multidisciplinary colleagues to facilitate a co-ordinated approach to palliative and end of life drug monitoring and control in line with formulary adherence.throughout the organisation.
• Be the point of contact for all hospice staff to aid in medicines-related problem solving and ensure supplies of end-of-life medicines.
• Monitor drug expenditure within specialty, produce budget statements and drug usage reports, interpret and provide analysis and recommendations.
• Develop treatment guidelines with clinicians to promote evidence-based, cost-effective prescribing, considering all relevant local and national policies and procedures.
• Act as an independent prescriber and prescribe as appropriate for patients within designated area of competence.
