Lead and co-ordinate an effective service to ensure there are safe and effective pathways in place for all young people across the footprint into specialised inpatient settings within the NHS and independent sector. Maintain and co-ordinate the Single Point of Access for all referrals into regional specialised inpatient beds on a daily basis. To ensure the provision of a 5 day, high quality clinical service to young people before, during and after admission to a specialist inpatient setting, co-ordinating bed management for the whole of the NHS Greater Manchester GAU and ED beds. To also work closely with our providers so the out of hours offer is consistent and meets the needs for those young people. To manage and register all referrals into the team and co-ordinate full and timely responses offering alternatives to admission where appropriate. To remain committed to ensuring a clear, timely and optimum outcome and resolution for all referrals. To promote and maintain partnerships and interface with other professionals and agencies that work within the child and adolescent mental health settings. To provide case management role for young people when their pathways have become disrupted, or have complex needs. Key requirements of Person Specification You will have a relevant core professional qualification in mental health or social care plus evidence of continued professional development in a CAMHS related area. You will have evidence of professional registration. You will have substantial experience of risk assessment and risk management. Band 6 Mental Health Practitioner

This role is as a band 6 Mental Health Practitioner within the CAMHS Crisis team working alongside senior clinical staff you will respond rapidly to requests for assistance when a young person is presenting in crisis.

You will contribute to the assessment and management of young people experiencing episodes of crisis within a clear and structured Risk Assessment and Risk Reduction framework and identify therapeutic goals to support the young person and families/carers. You will refer on to specialist in-patient services as necessary or work collaboratively with other services involved to reduce the need for inpatient care.

You will develop holistic, safety and recovery focused care plans and mobilise support around the child and family to promote resilience and reduce the likelihood of future crisis episodes.

As a member of the multi-disciplinary team the service will intensively support young people and families under a number of different pathways including an inreach and assessment model where a young person has been admitted to a paediatric ward, rapid intervention on 72 hour care pathways and longer term intervention on 4-6 week care pathways.

Direct work with young people will take place in clinical environments including emergency care settings and paediatric wards; community settings such as schools as well as home

environments.

To lead and support the Digital Health Systems Team including the training function of the department. Carry out day to day management of the Digital Health Systems function including annual appraisals & development plans for team members. Interpret and communicate highly complex systems information and processes within the organisation and external system suppliers.
