We are thrilled to announce an opening for a Band 5 Registered Nurse in our dedicated Children’s Continuing Care Team. This is a fantastic opportunity for a passionate and motivated nurse to make a meaningful impact in the lives of children and their families.
The team comprises currently of 1 Band 7 Team Leader and 10 Health Care Support Workers, working with children requiring commissioned health care packages in their home.
The staff within our continuing care team work closely with our children’s community nursing team and within children’s targeted community services in Salford.
The post holder will be responsible for supporting the holistic assessment of children’s care needs and development, implementation and evaluation of programmes of care within a Continuing healthcare caseload.
The postholder will support the management of a caseload of children and young people acting as a link between primary and secondary care with guidance, support from senior members of the team. This involves working unsupervised in patient’s homes, and community settings, with regular supervision from senior team members including safeguarding and clinical psychology.
Attendance of mutli agency meetings with senior staff, such as discharge planning meetings, case conferences and meetings as required whilst ensuring that all relevant information is exchanged and communicated to the appropriate participants and agencies.
The staff member must be able to recognise and monitor acute exacerbations of illness and to initiate appropriate nursing interventions that will maintain the safety of the child.
The post holder will support the team leader to develop, deliver and evaluate care packages.
They will also support the team leader in the provision and coordination of the service, which includes the day to day management of the staff rotas, ensuring appropriate levels of staff and clinical safety is maintained across continuing care packages.

This vacancy is open to those who have a current NMC PIN to practice as a Registered Nurse or Midwife in the UK, or those who are currently enrolled on a course which means they expect to receive their full NMC Pin within the next 2-3 months.
Please note an NMC PRN Pin is not a full NMC Pin and would not fulfil the essential criteria for this role.
