Bank Planner / Day Controller

We are looking for a dynamic thinker to support our local planning of patient journeys. The role involves effectively planning patient journeys to and from home / health care setting using our cleric system and against set key performance indicators.

Our patients have varying clinical and mobility needs, some of our patients travel regularly with us into renal and oncology units. Our patients and staff are our priority, ensuring we show care, compassion and empathy.

You will need a calm approach under pressure, have a general geographical knowledge of the Bedfordshire, Hertfordshire, Cambridgeshire, Essex and outlying areas. The role is focused on the development and implementation of the council's approach to quality improvement of the adult social care market across Essex. It ensures that the Adult Social Care market adheres to regulatory and contractual requirements and provides a consultancy service to ensure contract management and quality improvement can be delivered.

Providing early identification and intervention to manage risk effectively is key to ensure working systems function effectively; regular auditing of suppliers to ensure a good standard of quality is being met and monitoring improvement work.

Accountabilities:

• Responsible for completing comprehensive assessments and production of reports.
• Responsible for providing consultancy services to the Social Care market on how to improve the quality of their services to improve customer outcomes and experiences.
• Working collaboratively with partners and stakeholders to maximise potential opportunities for quality improvement; through the development of innovative approaches to service delivery to deliver greater choice to adults.
• Responsible for identifying and challenging care delivery which falls below standards set, providing support to improve the quality-of-service provision.
• Provide expert advice and support to providers during periods of Safeguarding activity.
• Provide reports on expected standards of delivery across Essex to inform and support ECC's safeguarding responsibilities and duty of care.
• Ensure providers are compliant with all external regulation and contractual requirements to achieve required standards and ensure where there are any issues these are addressed in a timely, effective manner.

Develop and manage a system for the identification and management of serious incidents and escalation of them to the appropriate senior members of staff within the organisation in order to comply with HCS policy and procedures to support patient safety and governance policy. Oversee all aspects of Serious Incident management. Support the Head of Patient Safety in the transition to PSIRF. Leading on the development and delivery of a training programme for Patient Safety/ Patient Safety Incidents to all staff across HCS. Providing both training and advice to clinical staff on a 1:1 or group basis around the processes. Lead on the performance management process for investigation and analysis of Serious Incidents, ensuring all documentation and associated reports are in place for Review Panel Meetings in order for action to be agreed. Communicate information to patients or staff members to inform people (and their families) when they have been harmed as a result of the care or treatment they have received to ensure adherence to Caldicott principles and thereby ensure that HCS meets legal requirements. Provide guidance and direction to managers and clinicians at all levels of the organisation on key aspects of the quality and safety strategy and supporting documents e.g. Mortality, Clinical Audit, Effectiveness and Quality Improvement Strategy, Duty of Candour Policy, Safety Event Learning Policy, Serious Event Framework; to ensure the achievement of care group and organisational quality and safety governance objectives; alongside clear monitoring and assurance reporting within a culture of openness and civility.

Develop a policy planning framework(s) within HCS and ensure this remains effective and relevant by pro-actively identifying when action is needed. Work closely with key stakeholders, partner departments including the Jersey Care Commission and Law Officers Department, to ensure HCS policies are appropriate, robust and achieve the intended outcomes. Drive, facilitate and monitor the development and implementation of up to date HCS policies, procedures and good practice guidelines, based on a range of evidence from sources such as National Institute for Health and Care Excellence (NICE), professional colleges and bodies, local quality improvement activities, patient and user experience/ feedback, external and internal reviews and regulation. We have an exciting opportunity to become part of an established and friendly team within Burton-on-Trent!

You will be working 36 hours per week, with the opportunity for overtime if required, as part of a team caring for a younger adult with an acquired brain injury. Working alongside some of the resident’s family members, as well as other care professionals, you will be ensuring that the service user has the best quality of life possible.

The service user likes to be active and takes part in trips within the local community including bowling, trips to the cinema. We are looking for someone who will engage with the service user throughout the day and take an active role in making their life the best possible!

Your duties will include:
- Following the care plan to ensure that the service users’ routines are followed to prevent any distress.
- Conduct personal care activities – inc. showering, catheter and bowel care
- Remaining engaged and patient with the service user
- Domestic duties
- Administrating medication
