Operational Delivery Identify the health needs of the teams 0-5 years population and establish priorities in line with the service specifications for Public Health Nursing, targeting vulnerable groups, in line with local and national initiatives. Actively engage with Family Centres to deliver agreed levels of service. Initiate, maintain, organise and manage programmes of health surveillance to include health reviews for Children Looked After. On a daily basis have due regard for personal and colleague safety in relation to physical aspects of the job; this would include lone working and carrying of equipment. Ensure all team members work within the policy. Have regard to the workload of, and the pressure of, professional colleagues and junior team members, taking appropriate action as necessary. Assess report and manage any risk, clinical and non-clinical for the benefit of self, colleagues and clients, in line with Risk Management Policy, particularly in relation to Lone Worker Policy and Health & Safety at Work. Ensure a team philosophy embraces a holistic approach to children, parents, families, and carers. Support team members to negotiate, agree, implement and evaluate planed care with clients, families, carers and other agencies. Patient / Customer Care Encourage and support team members to practice safely in Nurse Prescribing, demonstrating up-to-date knowledge of prescribed items including side effects and contra-indications and having due regard for security and administration. Empower and support team members to deliver a high standard of care to ensure individuals and their families reach their full potential in Health Promotion and Health Awareness, providing ongoing support to families with emotional, physical and life limiting conditions. Support team members to provide safe care in clients home including demanding/complex cases that may require advanced assessment and management skills. In addition team members will work within variable conditions, some undesirable and potentially hazardous, delivering care in health centres, GP surgeries, schools, community venues and Family centres as required. Maintain a high standard of infection control knowledge. Support team members in their continuing responsibility for the assessment of physical, emotional and social health needs and the development, implementation and evaluation of programmes of care for individuals and families. Oversee the level of Early Help assessments taking place within the team and encourage staff to act as Lead Professional where appropriate. Maintain a high standard of both written and computerised documentation within agreed guidelines. Ensure that team members have a clear understanding of the HCT record keeping policy At times act as the patients advocate raising their concerns about services of HCT or other agencies. Follow HCT and Hertfordshire Safeguarding Procedures, including the provision of assessments, reports and attendance at case conferences as required from the team. Have up to date knowledge and training on core group process and other identified work with children and families on the Child Protection Register and liaise with Safeguarding team as appropriate. Strategic Management Be prepared to participate in the HCT Emergency Plan in response to a major incident as requested. Service Development, Quality and Improvement Have a good working knowledge of the Quality Assurance framework ensuring sound leadership with Dip Tests (monthly checks/audits of patients electronic records), Peer reviews (yearly risk-based assessment of services, ensuring care is being given to a consistence high standard) and Quality Wheels (quarterly self-assessment of the service against key quality measures aligned with the five Clinical and Quality Strategy priorities). Incorporate the Trusts Making Things Better Approach to quality improvement through insight, involvement and improvements when leading on quality improvements and change ideas for staff, team service and service users. Lead in the development of innovative programmes to promote good health and participate in Public Health initiatives within the community, working collaboratively with other agencies, as agreed with commissioners. Support and guide clinicians in the development of their skills and knowledge base to effectively lead their respective teams. Lead projects as agreed with the Operational Manager, in line with HCT and national strategies, and personal skills and interest. Management and Leadership Attend regularly and participate actively in Restorative Supervision in line with HCT Policy. Manage, monitor and support all grades of staff in the team, managing the teams staff appraisal process. To act as a resource within the HCT concerning current development within the service, taking forward initiatives to share best practice, acting as a clinical role model within the team by demonstrating high levels of clinical practice. Take responsibility for own continuous professional development, maintaining a sound level of professional knowledge and competence in line with your Personal Development Plan agreed at annual appraisal. Ensure attendance at all required (mandatory) training. Provide effective mentorship by facilitating a learning environment to encourage the professional development of pre-registration and post-registration students on an ongoing basis. Ensure all team members are up to date with current mentorship training. Communication and Relationship Building Maintain confidentiality and manage information sensitively, acting as role model for colleagues. Demonstrate discretion and respect when communicating with clients, including those with special needs, colleagues and other agencies. Form and maintain strong communication links with GP practices within area of responsibility, working in partnership with GP colleagues in Practice Based Commissioning. Finance and Resource Management Ensure resources are used as efficiently and effectively as possible, taking into consideration allocation of staff, time management, clinical skills and equipment. Information Management Positively participate in and promote Clinical Governance, thus ensuring the highest quality of practice is maintained, within the service. Demonstrate the use of evidence based practice and participation in clinical audit. To actively participate in clinical audit, taking the lead as appropriate. General Maintain up to date registration and revalidation with the Nursing and Midwifery Council, in line with HCT Policy. Provide evidence on an annual basis to this effect. The post holder is involved for periods of prolonged concentration in the development of documentation relating to different incidents, complaints, individual staff members or work practices or procedures in order to achieve a suitable solution, future pathway or record as required. A. OVERALL JOB DESCRIPTION- SERVICE REQUIREMENTS
1. To provide clinical and administrative support to the Dental Officer to enable
maintenance of the surgery. This includes preparation/sterilisation of all instruments before
and after patients, reception duties, document handling, operation of dental computing
system, processing radiographs, shared maintenance tasks and working in the central
sterilisation area when in place.
2. To be fully aware of the responsibilities of a registered healthcare professional and
comply with the GDC Principles and Standards for Dental Professionals.
3. To ensure that not less than 6.5 hours per day in 2 sessions are set aside for
appointed patients
4. To be accountable to the Senior Dental Officer through their line management.
5. To comply with DPHC (Dental) policy on working hours.
6. To comply with all Health and Safety Policy directives and legislation.
7. To carry out preventive cross infection procedures including sterilisation of
instruments and materials in compliance with current legislation (HTM 01-05) and best
practice as described in DPHC (Dental) SOPs Chapter 13.
8. To adhere to local stock control procedures when removing items of stock from the
storeroom.
9. To manage the patient appointment system.
10. To manage dental records (manual and electronic) ensuring the correct receipt,
despatch, registration and storage of FMed 271 and hasten any outstanding documents.
11. To manage the receipt, despatch and documentation of dental prosthetic appliances,

B. STANDARDS TO WHICH A DENTAL NURSE MUST COMPLY WHEN PROVIDING

12. To comply with all relevant Authority policy including DPHC (Dental) policies,
regulations and orders as well as JSP 950 Leaflet 2-23-1 and other policies as notified to the
Dental Practitioner.
13. To ensure that surgery management, document administration (including the
rendering of dental returns and safe custody of F Med 271s) and maintenance of equipment
is undertaken to the standards laid out in JSP 950 2-23-1.
14. To ensure compliance with Radiation Safety Orders and adhere to current Control of
Substances Hazardous to Health (COSHH) regulations.
C. ESSENTIAL REQUIREMENTS / QUALIFICATIONS / EXPERIENCE
15. Fully registered with the General Dental Council (GDC) without restrictions.
16. Must hold valid suitable and appropriate indemnity insurance with a recognised
dental defence organisation.
17. Must provide evidence of clinical currency in accordance with JSP 950 Leaflet 4-1-4,
Returning to Clinical Practice, specifically a minimum of 2 clinical sessions per week over the
last 15 months.
18. To have completed GDC highly recommended Continuing Professional Development
(CPD) including medical emergencies, safeguarding children and vulnerable adults,
disinfection and decontamination, radiography and radiation protection.

To lead and supervise the members of the Public Health nursing team in the delivery of the 0-5 Healthy Child Programme (HCP) and Public Health Nursing Programme in line with the Trust guidelines and public health priorities. To work as part of an integrated 0-19 team and will be responsible for assessing, triaging, planning and implementing health care in accordance with Trust guidelines and Healthy Child Programme. This care may be delivered in a variety of settings and within a defined geographical area. To work with Partners to identify population health needs and determine the level of need and support required. They will represent the team at local liaison meetings e.g. Family Matters meetings and feedback information as required. To have an operational overview of the team and undertake delegated responsibility for reporting performance and data recording to the Team Leader. Maintain accountability for the assessment, triaging, planning and delivery of care. To allocate safeguarding and complex care cases that require targeted interventions from another team member, whilst retaining the case management responsibilities. RESPONSIBILITIES: The post holder will work within the HCT clinical governance framework, which is embedded in the day-to-day practice. Every clinical member of staff must work within the framework. Operational Delivery: Establish clear and effective communication links with children/young people, parents/carers, education staff, and a range of other professionals from both voluntary and statutory organizations. Identify the relevant health and social issues within the teams designated caseload. Recognise indicators of child abuse and initiate Safeguarding measures in accordance with recognised policies, and procedures of the Trust, and relevant Local Safeguarding Children Board and attend meetings as appropriate. Care Organise and undertake health surveillance and screening as determined by the HCP core programme of care, referring to appropriate services as necessary. Undertake health promotion/education activities that are congruent with identified needs and local and government targets. Demonstrate an evidence-based approach to practice to enable the development of a high standard of service provision. Demonstrate cultural awareness in relation to client needs.Undertake home visits in accordance with the Trusts Health & Safety Policy, in relation to lone working. Participate in the Health assessment of LAC, seeking advice and support from Designated Nurse for LAC as appropriate. Maintain professional records in accordance with both the Trust and NMC record keeping policies. Strategic Management Manage their time effectively to ensure prioritisation of workload in order to achieve agreed deadlines. Plan, organise and oversee the workload for both themselves and those for whom they are responsible within the team. Be responsive to fluctuations in workload and resources and take action as appropriate. Manage unpredictable and/or volatile situations effectively. Manage programmes of care to meet identified need. Assist in the delivery of local implementation plans for the Hertfordshire Children and Young Peoples Plan, etc. In undertaking this role, work in collaboration with other Senior Managers, Service Leaders and Stakeholders.
