The Role

Skills and Experience
The ideal candidate will hold a Nursing degree and have a minimum of 5 years industrial experience; it is desirable to hold a full teaching qualification or be working towards the teaching qualification, however for anyone new to teaching we can provide the training required. To be successful in this role, you’ll need to have excellent interpersonal and communication skills and be able to effectively motivate and inspire others. You’ll also have exceptional organisational skills combined with good IT knowledge and sound administration skills.

- You will have recent experience of dealing with student nurses either in a Higher Education Institution or in clinical practice.
- Have a good understanding of A/Level science.

Your role in the department:

You will be employed within the SEND Team within the student support services department structure under the overall management of the Head of SEND and High Needs.

You will report directly to the SENCO for professional, personal and organisational direction.

You will manage the EHCP process for the team ensuring that reviews are chaired, attended and noted with reports provided in line with guidance.

The health advisor will be part of a multi-professional/skilled team including clinical and support staff providing accessible health services to the population across Derbyshire and surrounding areas. The health advisor will be part of a multi-professional/skilled team including clinical and support staff providing accessible health services to the population across Derbyshire and surrounding areas. A commitment to quality service and excellence in all that DHU Health Care undertakes are integral to the role and the development of the service. The post holder will participate in promoting and advancing DHU Health Care. Answering calls from the general public in an efficient and courteous manner using agreed DHU, NHS Pathways and 111 policies, protocols and standards. Accurately recording, checking or amending details on the call logging computer system Adastra. Signpost patients to the appropriate care using the Directory of Services where appropriate. Triage patient calls using the NHS Pathways triage system. Deal appropriately with instructions and queries from surgery staff, clinicians and outside agencies. Maintain a tidy office/call centre at all times. Assist in general administrative and clerical duties and maintain manual/computerised records as required, in a professional timely manner. Adhere to all procedures, protocols and other relevant memorandum as appropriate. Provide assistance to new members of staff and assist in training and mentoring to ensure the maintenance of a high quality service. Attend training and meetings as and when required to do so by Management. Maintain strict confidentiality in relation to all issues concerned within the service and adhere to the requirements of the Data Protection Act 1984, Information Governance and Caldicott Principles. Maintaining equipment checking systems and printers as necessary and reporting any faults via the Electronic Shift Report to IT or senior member of staff. We are looking for a skilled and enthusiastic B5 Speech and Language Therapists to join our children's SLT team in the City team .

The post is in the Derby city team and based at Coleman Street Health centre .

We have a fantastic opportunity for a highly-motivated and enthusiastic Care Co-ordinator to join the community teams within Derby City.

Key aspects of the role will involve identifying and supporting patients who require additional support within the Community in line with the Ageing Well agenda, referring and signposting patients within the Health, Social and Voluntary Sectors.

The role will also include working closely with the aligned GP Surgery to coordinate meetings & Patient MDT meetings with Health and Social Care colleagues.

The successful applicants will have excellent communication skills, be able to work independently and be confident using IT systems.

This is a really exciting and challenging role for the right person; someone who is committed, has excellent IT and time management skills and really wants to make a difference to a high-functioning health care team.

Travel will be required across DCHS sites in Derby City within your working day.

Key aspects of the role will involve:

Identifying patients who require additional support within the community, using IT systems such as SystmOne reporting.

Co-ordinating monthly CST (Meetings and individual patient MDT meetings with external parties, such as Social Services and Community Mental Health teams and members of the Integrated team, to support complex patients.

Liaising with health care professionals, including GP, Advanced Specialist Practitioner/Community Specialist Practitioner, District Nurse, Therapies and other members of the health care team.

Referring patients to the voluntary sector as well as the health sector for additional support in and around their home and making telephone contact with patients; acting as liaison between the patient, health professionals and the social sector.

The Care Co-ordinator will have a holistic approach towards supporting patients within the community, helping them live well at home, working alongside an aligned GP surgery.

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.
