JOB SUMMARY This role is an amalgam of the CCC co-ordinator and Call handling role, therefore the duties will be a composite of these two roles. The two activities will be carried out simultaneously. You will be supporting a clinically lead team whose main role is to coordinate and facilitate the care of patients being admitted to hospital from their GP surgeries and also act as a single point of contact for GPs accessing emergency secondary care. You will be dealing directly with health care professionals and patients over the phone, via email and electronic fax. You will be responsible for dealing with confidential patient information and ensuring all information entered into our systems is accurate. You will receive incoming calls promptly and efficiently from Patients, Health Care Professionals and services, processing onto Shropdocs operational and clinical application (internal computerized database) or other as appropriate, in accordance with laid down guidelines, fast tracking to the appropriate health care professional or service. To carry out general administration duties The role has no fixed pattern as you will be expected to provide absence cover, with a minimum of two weeks notice. It will involve a mix of in-hours and out of hours shifts (including some overnights) and a mix of weekdays, weekends including bank holidays. 3. KEY TASKS AND RESPONSIBILITIES 1. To receive incoming calls promptly and efficiently from Patients, Health Care professionals and services, process onto Shropdocs operational and clinical application (internal computerised database) or other as appropriate, in accordance with laid down guidelines, fast tracking to the appropriate health care professional or service. 2. To ensure all calls are input into our Clinical software in a precise and timely manner. 3. To arrange and send all the relevant paperwork to healthcare providers in a timely manner. 4. To ensure that all associated administration is completed including scanning/photocopying. 5. To book patient transport using the appropriate service, enabling patients calls with ambulance if emergency transport is required as defined by our 999 trigger list. 6. To support clinicians in liaising with the various medical, social and transport services to ensure the delivery of agreed care. 7. To conference call with healthcare providers where relevant to provide the most appropriate patient care. 8. To ensure that you remain up-to-date and familiar with all non-clinical CCC and Call handling protocols and processes on the Shropdoc Hub, reading regular updates and keeping up to date with Bluestream training modules. 9. To make outbound calls to patients to arrange outpatient appointments. 10. To update information on all of Shropdocs databases / systems ensuring clear and concise notes are made for handover purposes. 11. Arrange and administrate care plans for patients undergoing long term treatment. 12. To enter appointments in relevant clinics. 13. To receive incoming calls from lone workers, recording information and escalating risks in accordance with laid down guidelines. 14. To search for correct demographics using Summary Care record, verify patients details and update records if required. 15. To ensure that all prescriptions are faxed or emailed to the appropriate chemist, entered on to relevant spreadsheet and filed during shift. Obtaining a clinician signature before faxing. 16. To attend and participate in training and Mandatory Training sessions as directed 17. To attend and participate in Operational Team Meetings. 18. To maintain a constant and continuous interest in Health and Safety matters applicable to Shropdoc activities, and strive to ensure that all staff avoid risks, and discourage irresponsible behaviour. Ensuring that in the event of incident/accident these are reported to the appropriate bodies/personnel and in the appropriate accident books. 19. To actively participate in the training and mentoring of current team and any new members of staff. 20. To be fully aware and able to implement all or any contingency plans as required. 21. To liaise with patients and the wider Primary Health Care Team as required to ensure the efficient management of patient care. 22. To undertake any duties of a similar nature consistent with the responsibilities of the post in order to provide a quality service. 23. To have a supportive role to the Rheumatology Nurse Specialist team of 7 nurses and provide a nursing contribution to the clinical management of patients with Rheumatological Disease. Perform basic clinical skills and provide support in drug monitoring. Administer Intravenous Infusions of infliximab, rituximab and other rheumatological medications, alongside bisphosphonates on a day-case basis. Provide safe, accurate, comprehensive monitoring of a patients and recognise changes in patients conditions which require the intervention of others and ensure that timely escalations are enacted as appropriate. Respond to adverse drug events and abnormal blood results, adjusting the patients treatment plan, under supervision. Ensure patient safety is maintained at all times and record and report adverse events and near misses in line with local policy. Duties and Responsibilities HOME VISIT 4.01 To ensure that the clinician is always transported to home visits safely and with all clinical kit available 4.02 To maintain contact with despatch throughout home visits, informing them of any delays/detours and always ensuring they are contacted before returning to base 4.03 Comprehensively complete vehicle checks as per the Driver and Vehicle Policy and the Vehicle and Kit Log before driving anywhere on shift 4.04 Ensure full clinical kit is available on all home visits and that it is regularly checked and restocked in accordance with procedure 4.05 To always use the TomTom satnav in the vehicle, even on short, local journeys 4.06 Support clinician in locating the front door of properties and assist with carrying of kit 4.07 Report all arrival and departure times using the working times function of the TomTom 4.08 Provide an escort into the patients home at the request of the clinician if you feel safe and comfortable doing so. Never witness a consultation, remain in the hall/by front door and be available as required 4.09 Record all drugs/sundries dispensed out of the vehicles using relevant stock control recording method and restock as required 4.10 Record details of home visits on Vehicle and Kit log 4.11 Maintain security of vehicle at all times 4.12 To always abide by the rules of the Highway Code and the Driver and Vehicle Policy 4.13 To ensure Toughbooks are charged and switched on and that you are aware of how to resolve basic faults 5. Job Summary To act as receptionist at the base, managing patient appointments, supporting clinicians with administrative tasks, and supporting and managing patients through their experience with Shropdoc To liaise with despatch and your clinician to ensure that base appointments and home visits are completed in a timely fashion To drive the Shropdoc vehicle to home visits in a safe and competent manner To ensure that all kit is checked and available for both home and base appointments To ensure that vehicles are checked prior to any visits so that they are safe and roadworthy 3. Duties and Responsibilities - BASE 3.01 To ensure that the base is set up and ready for patients at the beginning of the shift. All computers to be logged in and all essential kits ready and available. 3.02 To ensure that base email account is checked and anything relevant is actioned, all phone lines are connected, and printers are operational. 3.03 All clinical kit is checked in conjunction with the Kit Checking and Sealing Process 3.04 To appropriately record all drug and sundry usage across the shift using the relevant stock control method and in conjunction with Medicines Management Policy 3.05 To support your clinician with administrative tasks, such as scanning paperwork, emailing information or making phone calls 3.06 To be responsible for greeting patients at bases and promptly recording their arrival on clinical software and ensure that should a patient not attend that it is promptly reported to despatch 3.07 To ensure that all infection control procedures are stringently followed in line with relevant policy and maintain a continuously clean and tidy environment 3.08 To carry out basic first response and fault-finding following failure of systems/technical equipment 3.09 To provide clear and specific details to despatch when reporting issues that require further investigation 3.10 To log any incidents that occur on shift via Radar (for example, health and safety incidents, safe-guarding concerns etc.) 3.11 To advise clinicians of any drug or sundry shortages 3.12 To witness the removal and re-stock of controlled drugs and sign your agreement 3.13 To check the presence and function of all diagnostic equipment, replacing batteries or reporting any faulty items 3.14 To keep despatch appraised of clinical workload and any delay in seeing patients 3.15 To always maintain security of the base, including ensuring that the drugs cupboard remains locked, and that no patient identifiable data is accessible 3.16 To follow the process for patients without appointments should people unexpectedly turn up at base looking for clinical support 3.17 To ensure prescriptions are in printers when the doctor is seeing patients but stored in drugs cupboard when on home visit or base is closed 4. 1. Provide administrative support to the Business Support Officer in the retrieval of information in relation to key performance indicators.2. To accurately maintain and keep secure patient health and staff records/systems in line with policy and standard operating procedure, to ensure data quality, confidentiality and compliance with the current Data Protection Law.3. Provide a full range of office support systems including photo-copying and scanning.4. Collate and input data in a timely manner into both manual and electronic systems.5.
