The post holder will carry out the following duties for the PCN: - oversee the day to day running of the Enhanced Health in Care Home Team, PCN Cancer Services and the Digital Clinical Hub, together with ad hoc support to the PCN Pharmacy Team Lead to ensure services delivered are of high quality, safe and caring and value for money. - collation and preparation of performance reports, KPI data (using EMIS), outcomes, briefing papers for each of the teams in preparation for all relevant review meetings with Network members and / or external stakeholders, ensuring professional standards are maintained. - standard operating procedure compliance, preparing and distribution policies for the above teams. - meet regularly with the PCN Manager and PCN Project Manager to keep them updated on service delivery, identifying any areas of risk, mitigating factors and any areas for quality improvement. - attend regular service operational meetings with Southport and Formby Health Ltd (SFHL) to share updates, learning and keep PCN teams up to date with wider developments across the PCN and Southport and Formby Health. - attend regular Governance committee meetings with SFHL to ensure PCN services are compliant with governance requirements for the specific PCN teams. - assisting with line management of staff such as agreeing weekly work plan for the PCN staff where applicable. - support the HR Manager with inductions and onboarding of new starters and identify training and development needs across the teams and escalate to the PCN leadership team for consideration. - co-ordinate record keeping of PCN initiatives including mechanisms to record and monitor progress, identify risk areas and action planning. - support HR Manager and Team leads to manage rota systems for staff within PCN to ensure staffing levels are sufficient to meet the service needs on a daily basis. Ensuring appropriate desk space/room space available and the effective communication of such rotas network wide. - build external relationships and identify opportunities to work with wider system partners. - ensure compliance with the GDPR and FOI along with other relevant legislation and professional information management standards, such as GMC/BMA and LMC guidance, across the PCN. - take an active role in the development and embedding of the PCN culture, values, and reputation as providers of high-quality services. The post holder will carry out the following duties for both SFHL and the PCN: - act as first point of contact to oversee teams and offer support and trouble shooting, - responding / reporting operational incidents - act as digital champion for IT related issues, liaising with IT provider and colleagues. - build key relationships with clinical leads, team leads and staff as a visible presence in the main PCN/SFHL office. - available for team leaders to answer queries. - manage and facilitate appropriate communication channels to support the sharing of information and best practice through effective evidence gathering and updates to key internal and external stakeholders. - building awareness and embedding Employment Practices across the organisation. - work with our Governance Officer and Operations Assistant to manage and monitor our estate ensuring we are compliant, meeting the needs of service users and ensuring our physical buildings are maintained to a high standard. This is inclusive of, but not limited to, the safety of the site, ordering and maintaining equipment and completing all relevant documentation.

Duties to include the following; Follow a set protocol to decide which documents to process Code and file documents received by GP practices Code documents using an EMIS template created by the digital clinical hub to ensure the EMIS health record is clear and easy to understand. Refer documents that require any action to the relevant GP practice team using the EMIS appointment booking page Complete a daily audit by running a search in EMIS web to support data quality. Complete a daily log of work done in Microsoft Excel. You will work to a daily target for the number of documents completed daily, which we will support you to achieve. Engaging in weekly huddles to collaborate with team members, share knowledge and skills, and find ways to improve the service. Attending relevant training to support both the development of the Digital Clinical Hub and your own personal growth. Build and develop relationships with team colleagues, wider PCN colleagues and with practices across the PCN. Undertake other duties as may be reasonably required within the general terms of the job description. The main duties of the role include but are not limited to; Undertake consultations with patients taking a comprehensive patient history, carry out physical examination use their expert knowledge and clinical judgement to identify the potential diagnosis Exercise a higher degree of personal and professional autonomy using advanced clinical practice skills to include clinical interventions Refer patients for investigations when necessary/appropriate Make a final diagnosis, decide on and carry out treatment, including the prescribing of medicine or referral to primary or secondary care specialist services within local agreed pathways and protocols as appropriate Ensure that all treatments and care provided are based on best practice/evidence-based care Plan and provide skilled competent care to meet the patients health and social care needs, involving other members of the healthcare team as appropriate Work with multi-disciplinary and multi-agency teams in order to ensure patient needs are met Asses and evaluate, with the patient, the effectiveness of the treatment and care provided and make changes as needed Use independent/supplementary prescribing skills for benefits of patient care Work independently and as part of the 7-Day GP Service team Assist in clinical audit and setting and monitoring of service and unscheduled care Identify and take appropriate actions to address risk to the patient, this will include: Identification of emergency care needs requiring despatch of a 999 ambulance Safeguarding children and protection of vulnerable adult issues which require referral on to other agencies in line with local policies Work independently and at the same time contribute to the work of the whole team to ensure the delivery of a consistently high level of service, this will include: Taking responsibility and being accountable for managing own workload, risk assessment and management, acting within professional codes and appropriately to the clinical team leader for advice and support Undertaking other duties when required, such as audits or administrative tasks Identifying issues which may negatively impact upon service delivery and reporting these on to the clinical support manager/clinical manager operational team leader/shift manager in a timely fashion Acting as a clinical resource for non-clinical staff who may require advice and support Maintain contemporaneous records of the consultation and action using clinical software and where necessary written records, in line with NMC guidance and local policies, guidelines and procedures Remain focused on the delivery of an excellent service within an unpredictable, diverse, and challenging workload Work remotely using EMIS Web following the Working At Home Policy ensuring records and patient information is protected following GDPR.

This role is an integrated position working between Mersey Care NHS Foundation Trust and Mersey and West Lancs NHS Trust to support the early discharge, within the Care Transfer Hub, putting emphasis on valuing patient time. The post holder will work with the multidisciplinary teams in identifying and delivering a wide variety of quality improvements methods that will support identification and interventions required to reduce wasted time for patients in their journey of care and treatments through the hospital system, back out into community settings. The role will work closely with staff in clinical areas using appropriate tools, systems and processes that support the trusts capacity and flow ‘home first’ programme. The role for wards and clinical areas will have an emphasis on quality improvement methodology to assure delivery of excellence in patient and carers care provision which will ensure
at the centre of the system is the patient receiving acute care and/or their carers whose roles should be involvement at all stages of the journey and decision making from admission to discharge.

The post holder will provide leadership in support creating a culture of continuous improvement and excellence promoting high expectations and ambitions together high achievement by providing advice, guidance and support and coaching to ward staff and their teams on quality and safety.
To provide effective and visible clinical leadership that will support wards and clinical teams on their quality improvement plans with specific focus on patient flow leading to safe, timely and effective discharges.
Initiate and develop implementation of data collection and subsequent
improvements to measure and enhance quality outcomes related to SAFER care bundles and home first methodology for patients and their carer’s.
Work in partnership with the wider system, to develop discharge processes and to escalate any challenges / issues appropriately.
Identify areas for improvement and make recommendations utilising improvement methodology which will drive service improvement.
Provide learning opportunities for staff on safe, timely discharge for wards and clinical areas both formally and informally.

The successful candidate will support the development of pathways out of M&WL NHS Trust ensuring appropriate use of all community resources including Intermediate Care and Discharge to recover beds.
