An exciting opportunity has arisen to take the lead of the leg ulcer and lymphodema team at Tameside. This will involve acute and community working.

• The post holder has continuing daily Leg ulcer and Lymphedema team responsibility and accountability without direct supervision from the Tissue Viability lead nurse.

Community Gateway Clerical Officer

Band 2 - 37.5 hours - Permanent

We are looking for an enthusiastic individual to join our Community Gateway Service. The Gateway receives referrals to the District Nursing Neighbourhood, the Integrated Urgent Care Team and the Adult Social Care Neighbourhood teams.

What is a Community Gateway Clerical Officer?
This is the person at the end of the phone when a member of the public requires access to urgent and non-urgent health and social care interventions. They will gather as much information as possible, assess situations and provide clear, correct advice. This is a pressurised environment where you may be in contact with people in stressful situations; our callers can be upset, angry, anxious or hysterical. You would need to be confident in dealing with people in a variety of situations.

This post will be based at Crickets Lane Clinic, Ashton. The contracted hours will be 37.5 hours per week Monday to Friday, with the occasional Saturday. The service runs from 8.00am to 6.00pm therefore the role involves working a shift pattern, on a rota basis.

Main duties of the job

To be the first point of contact for service users making an initial contact to the Community Gateway regarding health and/or social care matters.

Based on set eligibility criteria and using agreed algorithms, decide whether a referral should be forwarded to Social Care or Health services, or another appropriate agency.

Principle Duties and Responsibilities

Assist the physiotherapist in the rehabilitation of patients using a range of delegated skills.

To manage the rehabilitation of patients as delegated by the physiotherapist. To be responsible for recognising patient progress or deterioration ( in particular taking and interpreting basic observations) and reporting back to physiotherapist.

Treat patients individually or in a group in a variety of settings with or without the direct supervision or presence of a physiotherapist, e.g. clinics or on the ward.

Encourage patients and their carers in an active approach to regaining and maintaining personal independence. This requires empathy, sensitivity and good interpersonal skills.

Use effective communication tools, verbal and non verbal to communicate treatment programmes to patients who may have barriers to understanding or communicating, e.g. blind, deaf, dysphasic or who do not have English as a first language.

Make accurate records of work undertaken and keep monthly statistics as required, including documenting patient contacts in line with professional and service standards.

The Role

The Financial Management team is a centralised finance service within the Resources Directorate using the Business Partnering approach to provide business and financial advice, guidance and expertise across the Council. A recently appointed Assistant Director who is due to commence in early December will lead the team.

You will play a crucial role supporting the transformation of financially sustainable services that will serve our residents to live healthy and independent lives whilst also advocating the continuous improvement journey within Financial Management.

Reporting to the Assistant Director of Finance, the roles will support the Council’s Corporate Finance responsibilities together with Service Directorates. The roles are key members of both the Finance and Service Directorate Leadership Teams bringing influence on all material business decisions, and ensuring the delivery of exceptional financial management and safeguard of public resources. You will be responsible for leading and developing teams to provide consistently high quality financial support and advice.

You will be responsible for leading the delivery of a number of essential responsibilities that include :

• Reporting the Council’s financial position, both revenue and capital, including the Council’s medium-term financial strategy, annual budget and savings programme, treasury management function, the collection fund and completion of the statutory statement of accounts accurately and within required timescales;
• Co-ordination of all revenue and capital monitoring across the business partnering Directorates;
• The development and assimilation of Directorate and Business performance data within financial reporting
• Leading the business partnering function for all corporate related financial activity;
• Evaluating invest to save / service transformation proposals to deliver cost efficiencies, together with the evaluation of future year service delivery models
• The provision of influential input to the decision making process, including objective challenge on strategy and policy development, efficiency and encouraging a culture of continual improvement
• The continuous review, development and implementation of financial systems, policies and procedures

• Advising and supporting directorate colleagues and key partners to deliver and transform business priorities with professional and influential challenge where appropriate

Job Summary

The successful applicant will work within the Pennine Care NHS Tameside & Glossop Talking Therapies service (based in Ashton-u-Lyne, which is in close proximity to Manchester city centre). It is a highly regarded, well-established provider of Talking Therapies services on behalf of the NHS. You will be offering high volume, low intensity Cognitive Behavioural Therapy (CBT) based interventions using a guided self-help model, to clients with depression and/or anxiety problems.

The PWP role is highly valued and seen as an integral part of what we do. Our PWP’s provide suitability assessments and evidence based Low Intensity CBT interventions for people with common mental health difficulties, such as anxiety and depression through face to face, telephone and digital methods. We offer 1:1, workshops and online treatments.

The post holder will work with people from different cultural backgrounds and ages, using interpreters when necessary and should be committed to recognising and valuing diversity and providing equal opportunities for all.

We are keen to develop the skills of our PWPs and there are opportunities to get involved in exciting and innovative projects, such as working with long-term conditions, and for career development.

Accept referrals via agreed protocols within the service and develop programmes of care packages.

Undertakes patient-centred interviews which identifies areas where the person wishes to see change and or recovery and makes an accurate assessment of risk to self and others.

Attend multi-disciplinary meetings relating to referrals or clients in treatment, where appropriate.

Complete all requirements relating to data collection within the service.

Keep coherent records of all clinical activity in line with service protocols and use these records and clinical outcome data in clinical decision making.

This is a diverse and challenging role that requires the development of specialist skills to deliver recovery focused interventions to adults in a mental health crisis. Our focus is to support service users to remain in their own home in order to aid their recovery during the crisis episode. You will be a qualified Mental Health Nurse, Occupational Therapist or Social Worker and will have worked in a multi-disciplinary team. You will need to have significant post qualification experience in mental health and preferably within crisis services.

We also provide clozapine titration in the community.

We are a dedicated team of nurses, OTs, Social workers, QAP and support workers who will work closely with Sector Consultants, our colleagues in the community mental health team, acute inpatients wards and the wider members of the MDT.
