Job responsibilities Successful candidates need to hold MBBS or equivalent medical qualification, with evidence of 12 months experience in Emergency Medicine (6 months of which working in an Emergency Department in the UK). Candidates will be eligible for full registration with the GMC and hold a current license to practice. Candidates will need to have evidence of achievement of Foundation competences from a UKFPO affiliated Foundation Programme or equivalent by time of appointment in line with GMC standards/Good Medical Practice and will have need to have passed their MRCEM and Life Support Courses. Emergency Department operates 24 hours a day, seven days a week; therefore successful candidates need to be able to work a variety of shifts, including days, lates, twilights and nights.

The post holder will work with the clinical pharmacy team under the guidance and supervision of the clinical pharmacy technicians and clinical pharmacists in providing a Clinical Pharmacy Service in a ward environment. You will assist in the day to day operations of a ward pharmacy team ensuring an efficient and effective service is provided. This includes facilitation and re-use of the use of Patients Own drugs, one-stop dispensing, discharge planning and pod checking and dispensing eDNs at ward level. The role will involve being a practice supervisor in supporting trainee's at ward level.

The post holder will provide specialist advice and support for patients with suspected cancer during the journey from referral to diagnosis. The role will be part of a growing team of nurses who will focus on the diagnostic part of the cancer pathway, focusing on streamlining timed diagnostic pathways and meeting the new 28-day Faster Diagnosis Standard for cancer. We are looking for an experienced Clinical Nurse Specialist to join our team, to offer an individualised service and provide direct support and advice to patients with suspected cancer during various and often complex diagnostic tests with a focus on meeting the 28 day Faster Diagnosis Standard and other Cancer Waiting Times targets. To maintain nurse led clinic's whereby the concerns and needs of the patient's and their family/friends and carers are appropriately channelled.

Assesses patients, plans, implements care, provides specialist advice and maintains associated records. Working autonomously to manage a caseload of patients, acting as a lead assuming responsibility and accountability for assessing, implementing and evaluating care for the designated clinical area. Ensure that nursing care delivered within the ward / department is compliant with legal, professional and organisational standards. Responsible for the correct administration of prescribed medication including transfusion of blood. To provide leadership, support and guidance to staff in monitoring the planned delivery and evaluation of individualised care. Ensure safe and effective care for complex patients requiring advanced clinical skills. Be responsible for assessing and recognising emergencies in the speciality. To interpret information and take appropriate action and to lead others to do the same. Act as an advocate for patients within the department, to ensure a patient orientated approach to the delivery of care and to meet standards within the NHS Plan and other relevant guidelines. Ensure that patients receive high quality clinical care and a good patient experience, having regard for their customs, religious beliefs and doctrines. Recognise and avoid situations that may be detrimental to the health and wellbeing of the individuals.

Lead the Falls Service across specified areas of responsibility to ensure a unified approach to care, consistent standards of care and patient access. To lead on the development of appropriate policies and standards relating to services auditing results and responses. To review local services for falls and fractures to identify gaps and to establish a baseline for service delivery, including the identification of multi-agency resources currently supporting Falls and fractures Influence the care provided by other Health and Social care professionals through provision of guidance and involvement in formal teaching Demonstrates a highly developed specialist knowledge within own discipline underpinned by current evidence-based practice acting as a reference for colleagues and other agencies. To actively engage with clinicians and all key stakeholders and to foster good working relationships to ensure effective partnership approach to achieving multi agency outcomes To coordinate and manage the ambulatory falls care pathway, triaging patients onto an appropriate clinic or activity programme. The post holder is responsible for supporting the senior sister charge nurse and multi-professional teams In the day to day delivery of care, ensuring optimal high quality fall prevention and falls management is delivered and continuously monitored.
