The Medical Day Unit has 2 treatment rooms, both with 10 chairs, working alongside clinical haematology outpatient department.

We specialise in intravenous therapies with a specific focus on haematological conditions. We work primarily to conduct day case treatments and investigation, preventing inpatient admissions and administering chemotherapy for haematological malignancies, as well as treatments for General Medicine.

We would love an experienced SACT trained nurse to join our team, or a nurse looking for a new challenge and willing to complete the necessary training.

An exciting opportunity has arisen to appoint a permanent Band 7Children’s & Young People’s Epilepsy Nurse Specialist.

The post holder will be responsible for meeting the needs of children, young people and their families with epilepsy. The post holder will support the Consultant to deliver holistic care to meet the needs of this patient group. This service is provided in accordance with national, local and regional guidance, standards, protocols and policies in order to ensure children and young people reach their full potential throughout childhood and into adult life. We are looking for a motivated and dedicated nurse to join our friendly team at Ormskirk Hospital

Registered children’s nurses who have significant post registration experience and who can demonstrate a commitment to date to nursing Children’s & Young People with Epilepsy are welcome to apply for this post.

Applicants must hold a recognised British Paediatric Neurology Association (BPNA) qualification and an Independent and Supplementary None Medical Prescribing (NMP) qualification.

· To advise as required on in-patient clinical management following appropriate protocols and with consultant support.

· To arrange appropriate on-going management, individualised to meet each family’s needs, at Out-Patients and through home visits and telephone support.

The Chase Heys New Directions, Discharge to Assess Therapy Team, offers a dedicated seven-day enhanced therapy service to 14 beds located within Chase Heys providing specialist assessment and intervention including discharge planning to adults following their acute stay in hospital.

Our team work closely with Trust colleagues, patients and families to achieve the best possible level of independence and quality of life, always supporting a discharge home where possible. Thus promoting the HomeFirst pathway and encouraging people's functional independence to stay safe in their own environment.

The successful applicant will be part of a motivated, forward thinking, multidisciplinary team of Occupational Therapists, Physiotherapists, Therapy Assistants and Assistant Practitioners. The team work closely with the wider MDT to include the Care Team at Chase Heys, Medical and Social Care colleague to provide excellent care and outcomes.

Continuous evidence-based practice and service development is fundamental to the team, and you will be passionate in identifying, leading and driving forward new projects.

You will be joining a recently established team and will have access to support and supervision from a Band 7 Team Lead, with wider clinical supervision provided through peer support from Trust colleagues. 2 x 37.5 hours per week. Current operating hours at Southport and Ormskirk sites are Sunday – Saturday 8.00am – midnight, including bank holidays. There is a concurrent on-call rota covering the Ormskirk site from midnight – 7.30am Sunday – Saturday. The post holder will be expected to rotate across these shifts, in the first instance covering mainly Tuesdays – Saturdays inclusive. The department is moving towards becoming operational 24/7 which will include night shift work.

The primary role of the post is to receive samples for laboratory testing to check that all details of the request forms and samples match and to enter and receive them into our laboratory information system. It also includes the pre-analytical preparation required for analysis and ready to be processed on the analysers.

The responsibilities of this job are to perform basic clerical and reception duties within the laboratory, receive and log specimens into the department and record tests requested on the laboratory computer system.

Working independently although under the technical supervision of a qualified Biomedical Scientist, to undertake pre-analytical preparation of specimens received and to act as the first point of reference for enquiries into the department.

This will involve working closely with Consultant and junior medical staff, General Practitioners, Laboratory and other hospital/professional staff both in this organisation and outside agencies and assisting patients with general enquiries both on the telephone and face to face.

KEY DUTIES

Technical
• Duties may vary slightly from site to site so may or may not be applicable on all sites.
• Receive and barcode all samples and request forms as appropriate
• Enter Patient and Sample details into Laboratory Information System. Data entry of all patient identifiable information, clinical details and request details for all disciplines with the exception of Blood Transfusion into the Pathology (Telepath) system (accuracy is vitally important).
• Sort and deliver bar-coded samples/requests to appropriate department in a timely manner
• Receive requests into Telepath via the order communications systems
• Process samples through pre-analytical stages such as centrifugation, aliquoting into additional tubes, removal of clots and loading onto the analytical track to be analysed.
• Numbering, Telepath entry and loading of Blood Cultures onto the analyser in Microbiology
• Take and communicate as required all telephone enquires
• Prioritise urgent samples for fast tracking
• Process add-on Requests
• To use the Patient Administration System and other systems to check patient details.
• To carry out LIMS housekeeping – manual patient merges in Haematology, Biochemistry and Microbiology once patient information has been checked in the above systems and to notify Blood Transfusion of any mismatches.
• Pneumatic Air Tube- Pathology Reception is responsible for receiving fault calls regarding the air tube that must be dealt with immediately as per policy.
• Purge air tube system and clear simple faults as required
• Report ongoing faults to Facilities Maintenance
• Respond to telephone enquiries from service users, patients, relatives, and departmental colleagues with correct use of telephone and the giving out authorised results from Telepath as required.
• Use of fax, photocopier, according to the manufacturer’s instructions, as required.
• Understand and follow storage and stock rotation requirements of relevant stock in various storerooms.
• Distribution of GP orders (including blood forms) to surgeries in a timely manner.
• To ensure that all reports are sorted and distributed to the relevant bodies through internal and external posting systems in a timely and efficient manner.
• Accurate filing and retrieval of reports. Scanning of request forms.
• To work closely with all departments within the Pathology Directorate to ensure that a timely accurate service is delivered.

Workflow Management

• To promote an efficient, courteous telephone manner to our service users.

• Ensure risks to service such as faulty equipment or unexpected staff shortages are reported to the supervisors or manager as soon as possible.
• Replenish stock of blood bottles as required and prepare and despatch orders to community locations.

If so, the North West Regional Spinal Injuries Centre is offering the opportunity to apply for a Band 6 Sister / Charge Nurse temporary 12 month post within the department to cover Secondment. This will enable existing staff to gain experience in a line management role whilst supporting the multi-professional team with day to day care provision. The aim of the role will be:
• To promote, assist & maintain the standards of professional practices in order to enhance the quality of care for patients with spinal cord injury.
• To assist in the effective ward management of the North West Regional Spinal Injuries Centre resources & services.
• To carry out the functions required to fulfil the goals & services of the ward / department.
• To participate in the Trust approach to Resource Management initiative.
• To ensure the ward is clean & provides a safe environment for patients & staff

Excellent clinical knowledge and skills are required with high level of communication skills. Previous experience within the speciality is essential.

This post will be subject to an Enhanced DBS (with barred list for vulnerable adults and children disclosure).

To assist the Senior Sister / Charge Nurse with effective day to day management of patient care whilst ensuring efficient staff line management. This will include daily staff allocation to ensure appropriate skill mix, completion of sickness absence reviews & staff appraisals, participation in clinical audits such as Tendable (Perfect Ward) & education & supervision of junior staff in order to develop a skilled workforce.

The North West Regional Spinal Injuries Centre at Southport hospital is a 43-bedded in-patient & 8-bedded community-based facility with a catchment population of more than 6,000,000 covering the North West of England, North Wales and the Isle of Man. We specialise in rehabilitation for people with varying degrees of disability, ranging from those with high cervical lesions requiring mechanical ventilation to others with incomplete neurology & cauda equina injury. The clinical team encompasses a variety of different professions including nurses / health care assistants, physio / occupational therapists, speech & language therapist, counsellor & medics.

KEY DUTIES

· Provide leadership, management and team development as delegated by the

· Department/Ward Manager to the team, allocating workloads.

· Contribute to setting goals and benchmarking standards of care.

· Develop tools to improve the quality of the service and co-ordinate clinical services across the ward.

· To co-ordinate and manage patient flow within the unit including timely admission and discharge, ensuring national targets are met where possible.

· To inform/escalate to Department/Ward Manager / Matron if acceptable levels of care, standards, practice, and patient safety is compromised and develop appropriate actions.

· To assist and or lead in the development/implementation of local and trust policies and protocols relevant to the specialty, ensuring effective implementation in local area.

· To manage control and quality of levels of stock and equipment within the clinical and non-clinical areas and promote and ensure economic use of all resources.

· To attend corporate groups as a representative of the directorate and team.

· To identify areas for self-development, areas of interest and training requirements within your personal performance planning and Development Review.

CLINICAL & PROFESSIONAL RESPONSIBILITIES

· Provide senior nursing advice and support to facilitate high quality, individualised care to patients/ carers in collaboration with the multi-disciplinary team.

· Manage and coordinate complex clinical situations, taking appropriate action in emergency or unforeseen situations.

· To be clinically competent and confident in the use of specialised equipment in varied clinical settings.

· Act as a clinical role model and leader for all members of staff providing daily unit management continuity of care and a high standard of excellent nursing care.

· To assess, plan, implement and evaluate programmes of care, actively seeking evidence-based practice which will improve the quality of care and meet the needs of the patient. · To maintain safe custody of drugs, ensuring that checking, witnessing, and administration procedures are compatible with statutory and trust requirements.

The Pharmacy Department has recently benefited from a significant investment by the Trust to further enhance the quality of service provided to our patients which includes more specialist posts to support our junior staff and increased ward-based technicians to enhance medicines management on wards.

To further benefit our patients, the Trust has also recently embarked on the implementation of EPMA throughout the hospital.

The pharmacist will be ward based within a department committed to a pharmacist free dispensary for most of the day and will be rotated through different specialities including acute admissions, cardiology, respiratory, surgery and Women's' and Children's' services.

We support pharmacists undertaking the Foundation Pharmacist Framework and continuously review our training and education to give our Foundation Pharmacists a positive learning experience during their early career in hospital pharmacy.

We actively encourage career development for our Foundation Pharmacists into specialist roles, over the past 5 years our band 6 pharmacists have progressed to a band 7 post on average 16 months after taking a post at Southport and Ormskirk Hospitals.

Specialist Medical Engineering Technician

Full Time: 37.5 hours

As part of a multi-disciplinary team of specialist medical engineers, your duties will include the commissioning and acceptance testing of new medical devices, carrying out planned maintenance, repairs, fault finding and electrical safety testing on medical equipment of a highly complex nature.

Equipment includes: life support, patient monitoring, patient therapy, diagnostic, ventilatory and anaesthetic equipment which at times may be connected to a patient.

You will be required to work without supervision and conduct diagnostic procedures.

You will take responsibility for the supervision of junior medical engineering staff under your technical control, and the supervision of contract staff carrying out maintenance on Trust premises; these staff could be working with highly complex medical equipment.

You will be required to provide training to nurses, clinical and care staff in the safe use of medical equipment, and keep appropriate records.

You will also be required to participate in an on-call rota providing emergency 24-hour cover to Life support ventilation in Spinal Injuries Unit, ITU and Home Based Patients (HBPs). HBP’s medical devices are located on premises outside of the Acute Trust, in HBPs’ homes.

All staff will be expected to work in any site location around the Trust, to meet the demands of the service.

Technical/Specialist Knowledge

1.1 Technical expertise across a broad range of medical devices and utilise specialist knowledge within a designated field of medical device technology. This specialist engineering knowledge will be gained through formal qualifications (HNC or above), attendance at Manufacturer approved training courses, self study, continuing professional development and practical experience of maintaining and repairing highly complex medical equipment.

1.2 Attend training courses and update individual technical knowledge of specialist equipment and knowledge in respect of specialised clinical use of such equipment. To disseminate this information to the relevant healthcare specialists as required

1.3 Attend mandatory training, to enable the Trust to meet its obligations under its policies and legislation. The post holder will identify personal and departmental training and development delivery needs.

Southport and Ormskirk Hospital have an exciting opportunity to join our team as a lead specialist HIV and Sexual Health pharmacist. We are looking recruit an experienced pharmacist with a keen interest in antiretroviral medicine and sexual health clinical pharmacy to become an integral part of our well established multidisciplinary team.

The successful applicant will take lead responsibility to support and implement quality-driven Sexual Health services alongside our lead consultant and specialist nurses. This will ensure antiretrovirals and antimicrobials and are used in a safe, effective, prudent, patient-centred and efficient manner.

The post holder will promote safe and cost-effective antiretroviral prescribing across the Trust in collaboration with the consultant and lead nurse in sexual health. You will lead in drug usage analysis, identification of budget pressures arising from new products, co-ordination of projects and audits and the development and maintenance of guidelines, policies and protocols relating to HIV and sexual health treatment. You will have significant input into the choice of therapy prescribed for people with HIV based upon their comorbidities and response to treatment, and prescribing of appropriate treatment.

In addition to supporting our HIV and sexual health service, the post holder will provide a clinical pharmacy service to our acute medical wards.

The Trust is currently undergoing the implementation of EPMA and the post-holder will have a significant input into improved prescribing and therapeutic drug monitoring using electronic systems.

Clinical Role
• To assist the Clinical Pharmacy Services Manager to plan, manage, develop and deliver the pharmaceutical service toHIV and Sexual Health patients within the Trust in line with National strategy, in accordance with the changing needs of the NHS, the requirements of the Trust and taking into account feedback from users of the service.
• Act as a pharmaceutical resource for the Trust’s patients, medical staff, pharmacy staff, nurses, midwives, non-medical prescribers etc.
• To work closely with pharmacy and other colleagues within local and national networks
• To work closely with clinical pharmacy colleagues to support the safe use of medicines.
• As a Trust approved Independent Non-medical prescriber, prescribe medication where appropriate in accordance with personal formulary
• To assist the department in producing and reviewing guidelines and polices
• To analyse medicines expenditure across an allocated directorate as directed by Clinical Pharmacy Services Manager and to identify prescribing trends and potential prescribing cost saving initiatives.
• To produce reports of and present such findings at Directorate level as required.
• To be involved in audit of current practice to identify compliance with e.g. HIV and Sexual Health Services prescribing or adherence to Trust guidelines, producing reports of results for presentation at a suitable forum.
• To critically evaluate and interpret all information obtained from all sources and to apply professional judgement, to provide clinically sound, unbiased, up to date, evaluated information.
• To apply sound professional judgement to complex legal and ethical issues and dilemmas.
• Be aware of changes to national policies, guidelines, NICE guidance and other medicines/health directives and recommend amendments to the formulary and medicines management policies as appropriate.
• Scan national medical and pharmaceutical news by using recognised medical and pharmaceutical databases and websites to identify current and future developments in the pharmaceutical care of surgical patients.



CLINICAL & PROFESSIONAL RESPONSIBILITIES

· To provide an advanced clinical pharmacy service to specified wards in accordance with professional, departmental and Trust policies. This will include:

- Taking drug histories on admission

- Checking medical notes of each new patient to ascertain presenting complaint, past medical history, working diagnosis and relevant biochemical, haematological and microbiological test results to ensure safe and effective medicines use.

- Ensuring each medicine has an appropriate indication and that each diagnosis has appropriate therapy.

- Ensuring that existing therapy is safe to take with, and has not contributed to, the presenting complaint.

- Ensuring that new treatments are compatible with the patients’ conditions.

- Making recommendations on changes to drug therapy in line with the above.

- Reviewing recommendations in the light of new results and treatment plans.

- Advising on which medications to continue and which to stop during the peri-operative period, if appropriate

- Monitoring for adverse drug reactions and submit ‘yellow cards’ to the CSM where appropriate.

- Advising on therapeutic drug level monitoring for specific drugs where appropriate.

- Ensuring compliance with formularies, policies and procedures.

- Educating patients about their drug therapy.

- Developing evidence-based treatment protocols for use within specialities or across the Trust where appropriate.

- Providing advice on the availability of alternative formulations suitable for use in patients who are nil by mouth.

- Providing information on the use of drugs outside their licensed indications

- Monitoring use and ensure adequate stock levels of crucial drugs on a daily basis.
• To participate in consultant ward rounds and multidisciplinary meetings where appropriate
• Provide specialist pharmaceutical information to medical and nursing staff and patients in your speciality and to General Practitioners and other staff where appropriate.
• Liaise with General Practitioners and other primary care professional to obtain accurate drug histories.
• Record clinical interventions in line with departmental policy.
• Report any drug errors you detect to the Head of Pharmacy or their deputy. Ensure that the appropriate error form and prescriber feedback are completed.
• Participate in the departmental therapeutic drug monitoring service, interpreting drug levels and providing advice to medical staff on appropriate dosage, including on call.
• Liaise with appropriate healthcare professionals within local Primary Care Trusts to ensure the effective and efficient use of resources across the primary/secondary care interface and where necessary develop shared care protocols.
• Provide professional and legal supervision to the dispensary. Duties will include:
• Professional checking of inpatient requisitions and TTO’s
• Ensuring all prescriptions comply with the Medicines Act
• Ensuring all prescriptions comply Misuse of Drugs Act where appropriate.
• Final checking of prescriptions for accuracy prior to release
• Labelling and dispensing of medication as required.
• Providing advice to patients on how to take their medicines and potential side effects.
• Advice to Doctors, nurses and other professionals as required.
• Order, receive and dispense controlled drugs in accordance with the Misuse of Drugs Act
• To develop competencies in designated specialty and deliver ward based clinical teaching to band 6/7 pharmacists & pre–registration pharmacists within the Trust.
• Supervision of pre-registration graduates and junior pharmacists
• Comply with the legal and other requirements related to the purchase, supply, use safe custody and destruction of drugs within pharmacy and other areas of the hospital.
• To assist medical staff in undertaking of such clinical trials as have been approved by the Ethical Committee and to arrange under the direction of senior members of staff of the Pharmacy department the necessary supplies and aspects of clinical trials protocol affecting the pharmaceutical service.
• Assist in the development, implementation and monitoring of The Clinical Pharmacy Standards across the pharmacy department.
• To act as a clinical tutor for pharmacists undertaking the Diploma in Clinical Pharmacy
• To assist the Clinical Pharmacy Services Manager in the recruitment of staff including participation on interview panels
• To assist the department in the review of patient group directions
• Return unwanted or patients own Controlled drugs from the wards in accordance with the departmental policy and the Misuse of Drugs Act

Pharmacy Technician – Information Governance / Dispensary Checker

We have an exciting opportunity for a registered pharmacy technician to join our small but essential Audit and High-cost Drugs / Homecare team.

You will be working alongside the lead pharmacy technician to support the provision of medication via homecare services in line with National Standards. This will involve liaising with patients, clinical teams across the Trust and homecare providers to maintain current relations, provide expertise and ensure a high quality of care for the increasing number of patients who receive their medicines via these services.

You will be based at Southport & Formby DGH however duties will entail travelling to other sites within Mersey & West Lancashire Teaching Hospitals NHS Trust as required.

The successful candidate will be expected to participate in the department's checking, late night, weekend working & Bank Holiday rotas. Our core working hours are 8.30 to 5pm Monday to Friday plus our weekend service 8.30am to 4.30pm on Saturdays and Sundays.

You will undertake a variety of detailed audits in Information Governance areas such as safe & secure handling of medicines, controlled drugs and patient safety. This will involve the input & analysis of data, formulation of action plans and ensuring all recommendations are followed through to completion.

An exciting opportunity has arisen for a motivated and dynamic individual Band 6 Occupational Therapist to join our team. TheNorth West Regional Spinal Injuries Centre at Southport hospital provides specialist care for spinal patients from across the North West, North Wales and the Isle of Man. We invite applications for a WTE permanent post within the spinal cord injury/neurological rehabilitation therapy team.

The candidate will need to be an experienced occupational therapist clinician who has had experience working with spinal cord injuries and neurological conditions.

You will receive support and clinical supervision from experienced Occupational Therapists and have opportunities to attend internal and external training linked with your personal development plan.

A keen interest in service development and involving/encouraging more junior staff is essential as is the need to be innovative.

• To assess and treat patients within the ward or Community setting.
• To assist Clinical Therapy Manager/Team Leader in service development and the provision of a quality service.
• To take responsibility for junior members of staff and delegate duties as necessary, providing appropriate level of support at all times.
• To meet the Therapy and Community and Continued Care Service objectives.
• To cover for absence when required within the Community and Departments of Southport & Ormskirk Hospital Trust
• To take part in the weekend rota if applicable.
