The post holder will manage the day to day activities of the Infection Prevention and Control (IPC) Team. Ensuring that staff comply with IPC policies in order to reduce health care acquired infections. The postholder is responsible along with other members of the IPC team for prevention, surveillance, investigation and control of infection in Queen Elizabeth Hospital (QEH), Kings Lynn.

The post holder will provide advice and education to staff in order to promote best clinical practice across the QEH. They will provide advice and guidance to staff when required.

About the role -
Rehabilitation in the Armed Forces is delivered in three types of treatment centre.

Primary Care Rehabilitation Facilities (PCRFs):
Patients may be referred to see a physiotherapist at a PCRF which is normally based at Defence Primary Healthcare medical facilities. A physiotherapist will complete a detailed assessment within 10 working days of the first consultation and provide treatment and rehabilitation as required. Many PCRFs have an Exercise Rehabilitation Instructor who can also provide injury-specific exercise therapy and advice on gradual return to fitness.

Regional Rehabilitation Units (RRUs):
If a condition cannot be managed at PCRF level, the Patient is referred to an RRU unit in the UK. The RRUs provide specialist assessment and opinion through a Multi-disciplinary Injury Assessment Clinic staffed by specialist doctors, physiotherapists and ERIs. These clinics have access to a wide range of medical imaging to make an accurate diagnosis and work out a treatment plan, while also providing additional treatment options, such as pain management.

To perform as a specialist nurse, performing assessments, supporting diagnosis and management of heart failure using local and National guidelines.

· Working as a resource for patients, their carers and other staff.

· Working and liaising with the multi-disciplinary team (MDT) to provide high quality, evidence based patient care.

· Supporting the planning, co-ordinating, delivering and evaluating of care given to heart failure patients in the community.

· Support the development of the service across Primary Care and work collaboratively with the heart failure team in Secondary Care to promote a seamless service across the patient pathway.

· In reach into the secondary care trust to review patients

· Assessing, monitoring and reviewing patients on the ‘virtual ward’

· Working autonomously within own scope of practice under the guidance of the Band 7 Heart Failure Nurse and Clinical lead


· Constantly striving to improve heart failure care the role requires engagement with other healthcare professionals to support the modernisation of care delivery with supervision from the Clinical Lead.

Professional and Clinical

· Provide care to a specialist level in a variety of settings for heart failure patients referred into the service in line with agreed guidelines.

· To support the implementation and monitoring of a care pathway for heart failure patients, including rehabilitation and lifestyle changes.

· To liaise with GPs and Consultant Cardiologists on the initiation and titration of evidence based medications for patients on the heart failure service caseload.

· Using Independent Prescribing to enhance management of heart failure

· To accept referrals and manage a patient caseload according to agreed guidelines.

· To support the development and deliver a specialist nursing and support service across the care pathway with supervision from the Clinical Lead

· Supporting the improvement of Standards of Care for people with heart failure, in line with the National Institute of Clinical Excellence (NICE) Guidelines for Chronic Heart Failure.

· To provide a specialist resource for Primary Health Care Teams to provide care and support for people with heart failure with supervision from Band 7 and Clinical Lead.

· Working with the MDT to deliver the care pathway for people with heart failure, including supportive and palliative care.

· In reach into the secondary care trust

· Assessing, monitoring and reviewing patients on the ‘virtual ward’ - assessing suitable patients to step down from secondary care or step up from primary care

· Discharging from the virtual ward when appropriate and ensuring appropriate community follow up

· Using ‘telehealth’ to monitor community patients and promote self-management

· To assist in the provision of training and education for patients, carers and professionals to develop the skills required to manage heart failure effectively.

· Demonstrate specialist knowledge and skills through nursing practice in line with local and national guidelines.

· To maintain own levels of clinical competence.

· Working autonomously with support from Band 7 and Clinical lead and being accountable for own practice

· To promote and facilitate research and evidence based practice in heart failure nursing.

· To maintain a high profile in the clinical area, acting as a role model for other health care professionals.
