Print ward lists and handover sheets to Identify new patients admitted since the previous pharmacy visit. Requesting and recording consent from the patient to refer if needed to the discharge medicine service and trust pharmacy community-based service. Transfer medicines, between wards if patients have been transferred without them. This will include medicines dispensed by the hospital pharmacy as well as patients own drugs (PODs) and patients own controlled drugs. Support the ward pharmacists and technicians to record accurate medication histories by: Ascertaining if the patient has brought supplies of their medicines from home. Ascertaining and documenting how the patient has their medications supplied, eg: compliance aids. Ascertaining and documenting if the patient has been admitted from a care home. Ensure patients have enough medicines available to avoid missed doses by: Assessing PODs as suitable for use on the hospital according to standard operating procedures. Contacting relatives or carers with patient consent, to bring in the patients PODs into hospital if necessary. Screening prescription charts to identify missed doses, new items prescribed and any changes to medicines, and requesting a supply from pharmacy if required. Liaising with ward staff to identify any additional stock medicines that they require. Identify patients who have been prescribed new medicines and counsel on side effects, dose, frequency, food interactions etc, using the relevant systems/training available to support with this. Support clinical areas to maintain safe and secure handling of medicines by: Ensuring that all medicines for individual patients are stored correctly in the bedside locker, or elsewhere on the ward according to the standards within the Trust Medicines Charter. Delivering patient labelled medicines to individual bedside lockers. Participate in pharmacy-led audits as required. Report any incidents of inappropriate storage and handling of medicines to the appropriate person. Ensure that information collected is communicated to the relevant pharmacist or technician in a timely manner. Minimise medicines waste by managing medicines returns from clinical areas according to standard operating procedures. Dispense medicines for in-patients (including discharges) following guidelines, under the supervision of the Pharmacy Technician or Pharmacist, with the final check by a Checking Technician or Pharmacist. Ensure that all information needed for accurate stock control and prescription issues data is recorded using both paper systems and the computerised Pharmacy Information System (PIS). Liaise with appropriate staff member about urgently required medicines and inform patients, and other health care staff about supply problems / delays. Dispense specialist hospital-only medicines and unlicensed medicines which require accurate, detailed records. Dispense oral chemotherapy medicines taking necessary precautions detailed in SOPs. Act as the Medication Safety Officer for the Trust, becoming a clinical expert for medication safety. Provide a highly specialised pharmacy service in the field of medication safety and risk management. Promote an open and honest culture to encourage, promote and increase the reporting of medication events and learning. To be responsible for ensuring copies of correspondence are stored in accordance with standard operating procedures. To attend meetings and appropriate Committees to provide secretarial support by taking minutes and arranging meetings, as and when required. To be responsible for the confidential management of records and ensure that all documentation is returned to the appropriate department in a timely manner and within standard operating procedures. The post holder will give relevant assistance and training as and when required to support all colleagues. To carry out general administration duties and provide clerical assistance, as required. To send and receive faxes in a safe haven environment as per Trust policy.

An exciting opportunity has arisen for a highly motivated and enthusiastic individual with a passion for fitness to join our Health and Wellbeing team. We are seeking a passionate, energetic and inspiring individual to join the team and provide motivational support, advice and guidance to our staff. The applicant must hold Level 2 Fitness Instructor qualification, Level 3 Personal Trainer qualification and be registered with CIMSPA (The Chartered Institute for the Management of Sports and Physical Activity) at time of application.

You will lead the development and delivery of the physical competency assessments programme for the Trust working closely with respective teams ensuring compliance to local and national policy and standards. Using your extensive experience in the field you will also provide training advice and support with a focus on lifestyle behavior change. You will be an innovative thinker with a flexible and pragmatic approach to work, be a people person who believes in, champions, and supports improvements in physical wellbeing.

You will be required to work across the whole Yorkshire region supporting all staff.

The post holder will be an excellent Communicator, Empathetic, Compassionate, non-Judgmental and be willing to support the health and fitness needs of our diverse staff. This can range from supporting rehabilitation, undertaking assessments, inductions and designing personalised programmes to help our staff achieve their goals.

You will be people focused with a passion for health and fitness. Working within a multidisciplinary team including occupational health service providers you will use evidence base to develop new preventative approaches to raise awareness of and inform positive lifestyle and wellbeing improvements. You will use a variety of communication platforms and approaches to deliver health and fitness related interventions.
